Administration Timing of Respiratory Syncytial Virus Preventatives Among Commercially Insured Populations in the United States: 2024-2025 RSV Season.

Background/Objectives: Respiratory syncytial virus (RSV) is the leading cause of infant hospitalizations in the United States. Prevention strategies are recommended to mitigate severe RSV outcomes. In addition to identifying potential coverage gaps, preventative administration timing is important for estimating product effectiveness. This study characterized administration timing of maternal and infant immunization against RSV across the United States during the 2024-2025 RSV season. Methods: A retrospective cross-sectional study was conducted using administrative claims of a commercially insu

Effects of a "Food Is Medicine" Intervention on Glucose Control Among Medicaid-Insured Patients With Type 2 Diabetes: A Randomized Controlled Trial.

BACKGROUND: Although medically tailored groceries (MTG) hold promise to improve food and nutrition security and health among patients with diet-related illnesses and social needs, few randomized trials have been performed. METHODS: We enrolled 460 Medicaid-insured adults with type 2 diabetes and elevated hemoglobin A1c (HbA1c) between November 2021 and July 2022 in a randomized controlled trial of MTG in southern California. Eligibility was based on at least 2 HbA1c measurements ≥7.5% in the past year. Participants were randomized 1:1:1 to usual-care (N=153), lower-dose MTG (N=153), or higher

The effectiveness of care coordination on medication adherence among high-need, high-cost commercially insured beneficiaries: A randomized controlled trial.

BACKGROUND: High-need, high-cost (HNHC) beneficiaries experience life stressors that complicate their medication self-management. Care coordination programs may improve medication-taking behavior, including for chronic conditions that require daily medication adherence. OBJECTIVE: To determine whether nurse-led care coordination is associated with improved adherence among an HNHC commercial population with a chronic condition. METHODS: We analyzed data from a pragmatic, national randomized controlled trial of a comprehensive care coordination program conducted by a large national insurer and

Freedom to Ride, Responsibility to Insure: Florida's Motorcycle Trauma Coverage Gap When Private Risk Produces Publicly Financed Rescue.

Florida's motorcycle insurance framework can separate individual risk-taking from the financing of catastrophic trauma care. When meaningful first-party medical coverage is absent or inadequate, unreimbursed costs may be shifted to hospitals, physicians, public programs, insured families, and taxpayers. This editorial argues that the resulting arrangement can function as an implicit subsidy of a privately chosen motor-vehicle risk and reviews historical Florida and contemporary out-of-state evidence describing that potential burden. Because the current statewide magnitude, payer distribution, …

The Association of Medical Periodicity Adherence and Early Preventive Dental Care With Subsequent Dental Utilization Among Medicaid-Insured Ohio Children.

Purpose: To evaluate the association between preventive medical and dental visits before age 30 months with dental utilization after age 30 months. Methods: This retrospective cohort study used Medicaid enrollment and claims data from Partners for Kids, a pediatric accountable care organization in Ohio. Medical periodicity adherence was defined by the HEDIS W30 measure, and early preventive dental care was defined as having a preventive dental visit by age 30 months. Dental utilization outcomes were measured from ages 30 months to eight years. Descriptive statistics, chi-square tests of indepe

Relationship Between Socioeconomic Deprivation and Healthy Days Among Commercially Insured Adults Using a Digital Health Application: An Observational Study.

Population health research has long demonstrated that where someone lives is highly correlated with health outcomes and quality of life. This study explored if this relationship remained between zip code socioeconomic deprivation index (SDI) and member-reported healthy days among commercially insured adults interacting with virtual care and navigation services between May 1, 2023 and May 31, 2024, offered by Included Health. Members received an SMS-based survey that included the 4 Centers for Disease Control and Prevention Healthy Days questions after interacting with the digital health applic

Trends in Initiation of Disease-Modifying Antirheumatic Drugs for Rheumatoid Arthritis Among Commercially Insured Adults in the United States, 2001-2021.

OBJECTIVE: To evaluate patterns in disease-modifying antirheumatic drugs (DMARDs) initiations between 2001 and 2021 among adults with Rheumatoid Arthritis (RA) in the United States METHODS: This retrospective cohort study used a US commercial claims database (2001-2021) to identify patients (≥ 18 years) with RA newly initiating a DMARD. We calculated the annual proportion of initiations for 22 DMARDs, categorized as conventional synthetic (csDMARDs), biologic (bDMARDs), and targeted synthetic (tsDMARDs). Secondary analyses examined trends in first-line non-csDMARD initiation and biosimilar upt

COVID-19 Incidence and Mortality in Patients Recovered from Tuberculosis: a Retrospective Cohort Analysis of the National Health Insurance in the Republic of Korea.

Tuberculosis (TB) is an endemic respiratory disease in several countries, including South Korea. The coronavirus disease 2019 (COVID-19) may pose greater risks to individuals with pre-existing respiratory diseases, but there are few reports on how the state after recovery from TB affects COVID-19 infection and mortality. This study aimed to investigate the susceptibility and mortality of COVID-19 in patients with a history of TB. We retrospectively analyzed data from the National Health Insurance Service of Korea. We extracted individuals with TB from 2011 to 2019 and matched them with a popul…

An explainable machine learning framework for predicting the risk of buprenorphine treatment discontinuation for opioid use disorder among commercially insured individuals.

OBJECTIVES: Buprenorphine is an effective evidence-based medication for opioid use disorder (OUD). Yet premature discontinuation undermines treatment effectiveness, increasing the risk of mortality and overdose. We developed and evaluated a machine learning (ML) framework for predicting buprenorphine care discontinuity within 12 months following treatment initiation. METHODS: This retrospective study used United States (US) 2018-2021 MarketScan commercial claims data of insured individuals aged 18-64 who initiated buprenorphine between July 2018 and December 2020 with no buprenorphine prescri

Equity in the Developmental Dysplasia of the Hip (DDH) Diagnosis and Treatment: A Retrospective Cohort Study to Unravel the Effect of Area Deprivation and the Insurance Type.

Background Timely diagnosis of developmental dysplasia of the hip (DDH) is crucial for implementing less invasive treatment. However, socioeconomic barriers may lead to late diagnoses. The Area Deprivation Index (ADI) is an indicator of the socioeconomic challenges experienced by patients and their families. The primary objective is to investigate if the age at which DDH is diagnosed and the treatment protocol are influenced by the ADI or the insurance type. Materials and methods Using International Classification of Diseases-Tenth Edition (ICD-10) codes, newly diagnosed DDH patients (age unde…
