Reconstruction of the medial patellofemoral ligament with nonresorbable suture tape normalizes patellar maltracking independent of patella-side fixation technique.

PURPOSE: Patellar maltracking caused by a rupture of the medial patellofemoral ligament (MPFL) can be improved by MPFL reconstruction (MPFL-R) with a tendon graft. Nonresorbable suture tape (FiberTape®, FT) is possibly becoming an option to tendon grafts for MPFL-R. Patella-side fixation of FT can be performed with suture anchors or via soft-tissue fixation. The aim of this study was to investigate patellar tracking considering soft-tissue-based and anchor-based patella-side fixation techniques. METHODS: In eight fresh-frozen human knee joint specimens (m/f 4/4; age 75 ± 10 years), the MPFL w

Routine knee MRI: how common are peripheral nerve abnormalities, and why does it matter?

OBJECTIVE: To evaluate the frequency, MRI appearance, and clinical significance of peripheral nerve abnormalities encountered on routine knee MRI. MATERIALS AND METHODS: A retrospective review was performed to identify consecutive patients who underwent routine knee MRI from March 2015-2018 and had peripheral nerve abnormalities. MRIs were reviewed for the presence of tibial (TN) and common peroneal nerve (CPN) abnormalities (including hyperintensity, bulbous enlargement, discontinuity, architectural distortion, skeletal muscle denervation). The presence or absence of concomitant meniscal, cr

An Updated Overview of the Anatomy and Function of the Proximal Medial Patellar Restraints (Medial Patellofemoral Ligament and the Medial Quadriceps Tendon Femoral Ligament).

The medial patellofemoral ligament (MPFL) has been widely accepted to function as "the primary static restraint to lateral patellar displacement." However, current growing evidence suggests that there is a complex of medial patellofemoral/tibial ligaments, both proximal [MPFL, and medial quadriceps tendon femoral ligament (MQTFL)], and distal (medial patellotibial ligament and the medial patellomeniscal ligament) which restrain lateral patellar translation at different degrees of knee flexion. Specifically, the MQTFL has gained popularity over the last decade because of pure soft tissue attach

Medial Patellar Instability: A Systematic Review of the Literature of Outcomes After Surgical Treatment.

PURPOSE: To perform a systematic review of literature reporting on outcomes after surgical treatment of medial patellar instability. METHODS: A systematic review was performed according to PRISMA guidelines. Inclusion criteria were as follows: the outcomes and complications of medial patellar instability repair with a follow-up greater than 12 months, English language, and human studies. We excluded cadaveric studies, animal studies, basic science articles, editorial articles, review articles, and surveys. RESULTS: Searches identified 1,116 individual titles. After inclusion and exclusion cr

[Complete rupture of the peroneal nerve by knee distortion].

This report describes a case of complete rupture of the peroneal nerve as a consequence of low velocity trauma. A 54-year-old physically fit patient suffered a complex trauma with complete nerve discontinuity as a result of knee joint distortion without external force. The initial medical findings were unremarkable, in particular neither the accident medical history nor the initial sensitivity impairment suggested the presence of serious knee damage; however, during clinical diagnostics a complex trauma with rupture of the peroneal nerve was found. Accordingly, an extensive revision with nerve

Surgical management of neglected bilateral obligatory patella dislocation.

Congenital dislocation of the patella may present as an isolated anomaly or may be associated with other lower limb deformities as part of a more complex malformative or dystrophic syndrome. One type is the congenitally dislocated patella fixed irreducibly from birth in a lateral position, and second is the obligatory patella dislocation that is hypoplastic to start with and gradually gets fixed into the lateral position as the child grows. We report the surgical management of a 16-year-old boy with neglected obligatory dislocation of bilateral patellae with 2-year follow-up results.

Fully arthroscopic stabilization of the patella.

The authors present a new fully arthroscopic technique for the treatment of patellofemoral instability consisting of plication of the medial patellar retinaculum and release of the lateral patellar retinaculum. The indication for this procedure is not only acute patellar luxation, but also recurrent patellar luxation and subluxation. The procedure has been performed on 17 patients, 6 male and 11 female, between the ages of 14 and 27 years. The indication for surgical arthroscopic treatment was patellar instability in 3 patients, acute patellar luxation in 4, and recurrent patellar luxation in

[Anterior knee pain associated with sports and work].

Anterior knee pain is not a disease, but a syndrome with numerous causes. This paper describes its appearance in the patellofemoral joint in sportsmen and in untrained people. Chondromalacia patellae is a condition of the cartilage, not a disease, that is, it is never diagnosed alone. Relative muscular insufficiency, especially of the knee extensors, may occur in children and adolescents, as they grow rapidly. The consequence is a unbalance of active stabilizers and the disturbance of the slippery trail of patella, particularly in the presence of dysplasia of patellofemoral joint. The impingem

MR imaging of the patellofemoral compartment.

This article reviews the applications of MR imaging of the patellofemoral compartment. Axial plane images are the most informative for abnormalities of this compartment. The role of MR imaging in the evaluation of the medial synovial plica and in the detection of chondromalacia is discussed. MR imaging can reliably detect and delineate the complex of injuries associated with patellar dislocations and valgus hyperextension.

[MRI evaluation of the patellar articular cartilage in patients with subluxation of the patella].

In patients with subluxation of the patella, injury of the patellar articular cartilage is frequently observed and correct evaluation is important to manage these patients. We examined 11 patients with subluxation of the patella and five normal volunteers. In 12 patellofemoral joints of seven patients with subluxation of the patella, the abnormalities observed on MRI were compared with those on arthroscopy and/or at operation. MRI was performed with a Magnetom 1.5 T (Siemens) using the round surface coil. Pulse sequences were SE (TR 400 ms/TE 19 ms), FLASH (TR 320 ms/TE 15 ms FA 90 degrees and

Strapping of joints.

Strapping with adhesive tape plays a significant part in the prevention of joint injuries, particularly at the ankle and hand, and is important during early management of injury and rehabilitation. Recurrence of injury when the athlete recommences training is less likely when supportive strapping is applied.

An evaluation of the Elmslie-Trillat procedure for management of patellar dislocations and subluxations: a preliminary report.

The Elmslie-Trillat procedure involves lateral retinacular release, medial capsular reefing, and medial transposition of the anterior tibial tubercle hinged on a distal periosteal attachment. It is an easily performed procedure and is an excellent method for realignment of the extensor mechanism in cases of dislocation and subluxation of the patella. In our series of 52 cases with 6-24 months postsurgical evaluation, 46 cases, or 88 percent were rated as good or excellent. It must be emphasized that this technique is used only if the epiphyseal plates are closed.
