Short-term relugolix use for rapidly growing uterine fibroids before uterus-preserving surgery in women seeking to conceive: Two case reports.

Large uterine fibroids that grow rapidly over a short period should be differentiated from uterine leiomyosarcoma, and their treatment remains controversial in women seeking to conceive. Here, we report two cases of uterine fibroids treated with short-term relugolix administration followed by myomectomy. Needle biopsy specimens obtained before relugolix administration showed no malignancy, and significant tumor shrinkage was observed following treatment. Both patients subsequently underwent myomectomy. One patient achieved a live birth 14 months after myomectomy, whereas the other experienced …

Use of a magnetic intramedullary nail for axial compression in endoprosthetic reconstruction of the humerus: a description of technique.

Pathological fractures, both completed and impending, of the humeral diaphysis from primary or metastatic disease demand a reconstruction that restores stability while preserving shoulder function and alleviating pain. Conventional reconstructive approaches include the use of long-stemmed endoprosthetic devices, plate-and-screw fixation or intramedullary nailing, all of which can be limited in their success when residual bone is short or biologically weak. This case study details the management of a patient with metastatic leiomyosarcoma who presented with refracture through a previously cemen…

Rare serosal cystic gastric gastrointestinal stromal tumor with extensive intestinal metaplasia in an adherent gastric mucosa; a case report in a 65-year-old male.

Gastrointestinal (GI) intestinal stromal tumors account for 60% of mesenchymal GI tract tumors commonly located in the stomach and small int … c transformation. The primary differential diagnoses of spindle cell neoplasm are GISTs, Leiomyoma, Leiomyosarcoma and Schwannoma. These spindle cell neoplasms are differentiated by applying a panel of Immunohistochemical stains, CD117, SMA and S100.

Management of Soft-tissue Tumors With a Size of 2-5 cm, Including Malignancy.

BACKGROUND/AIM: The management of soft-tissue tumors of 2-5 cm is not specified. We investigated the management of these cases. PATIENTS AND METHODS: Among 105 cases with soft-tissue tumors from 2014 to 2021, 38 with tumors of 2-5 cm were retrospectively reviewed. The clinical characteristics, diagnosis, and management were investigated. RESULTS: The study population included 14 males and 24 females (mean age=57 years). Common histological diagnoses included lipoma, epidermal cyst, and hemangioma as benign tumors, and leiomyosarcoma (n=2) and metastatic soft-tissue carcinoma (n=1) as maligna…

Cutaneous mesenchymal tumors treated with Mohs micrographic surgery: a comprehensive review.

Cutaneous mesenchymal tumors (CMT) are rare tumors with wide clinicopathologic heterogeneity. Treatment of malignant cutaneous mesenchymal t … d pleomorphic sarcoma treated with MMS recurred in 32% with an unknown metastatic rate. Superficial leiomyosarcoma treated with MMS recurred in 3.8% with no reported metastasis. Cutaneous angiosarcoma and myofibrosarcoma treated with MMS have shown no recurrence or metastatic disease, but literature is sparse. The rarity of malignant CMT and the lack of comparat …

Atypical presentation of intravascular leiomyomatosis mimicking advanced uterine sarcoma: modified laterally extended endopelvic resection with preservation of pelvic neural structures.

Intravascular leiomyomatosis is a rare, benign tumour of myometrial smooth muscle. Despite being non-invasive, these tumours can proliferate aggressively within vascular structures including pelvic vessels, the vena cava and the heart. We discuss a 77-year-old woman presenting with a 9 cm uterine mass extending into the right adnexa and ovarian vein. Following hysteroscopic biopsy, palliative radical surgical resection was performed for suspected stage IV leiomyosarcoma. Tumour extension into the pelvic sidewall and obturator fossa indicated a modified laterally extended endopelvic resection c…

Myofibroblastic, fibroblastic and myoid lesions of the breast.

Myofibroblastic, fibroblastic and/or myoid lesions are rare in the breast but comprise the majority of mammary mesenchymal spindle cell lesi … entiation include benign leiomyoma, myoid hamartoma and leiomyomatous myofibroblastoma, but primary leiomyosarcoma and rhabdomyosarcoma may also rarely arise in the breast. Furthermore, fibroepithelial lesions and metaplastic carcinomas can demonstrate myoid metaplasia. Diagnosis can be challenging, particularly on core biopsy, but benign lesions with or without …

Uterine sarcoma - current perspectives.

Uterine sarcomas comprise a group of rare tumors with differing tumor biology, natural history and response to treatment. Diagnosis is often made following surgery for presumed benign disease. Currently, preoperative imaging does not reliably distinguish between benign leiomyomas and other malignant pathology. Uterine leiomyosarcoma is the most common sarcoma, but other subtypes include endometrial stromal sarcoma (low grade and high grade), undifferentiated uterine sarcoma and adenosarcoma. Clinical trials have shown no definite survival benefit of adjuvant radiotherapy or chemotherapy and ha…

Vulvar sarcomas: Short guideline for histopathological recognition and clinical management. Part 2.

Malignant tumors of the female reproductive system are a serious health and social problem, as they are the second most common cause of deat … ortality rate. In Part 1 of this paper, we presented the most common forms of sarcoma of the vulva: leiomyosarcoma, epithelioid sarcoma, malignant rhabdoid tumor, and rhabdomyosarcoma. The second part of this review will focus mainly on the rarest variants of vulvar sarcoma: low-grade fibromyxoid sarcoma, synovial sarcoma, monophasic synovial sarcoma, carcinosar …

Multimodality imaging of splenic lesions and the role of non-vascular, image-guided intervention.

Splenic lesions are often incidentally detected on abdominal-computed tomography (CT), ultrasound, or magnetic resonance imaging (MRI), and … nic cysts, and pseudocysts, and malignant processes such as metastasis, lymphoma, angiosarcoma, and leiomyosarcoma. While several of these splenic pathologies have characteristic imaging features that are helpful in diagnosis, others have nonspecific findings. In such clinical dilemmas, image-guided intervention may be essential, and we therefore discuss the rol …

Abdominal malignant fibrous histiocytoma infiltrating stomach with Chilaiditi's sign manifestation (a rare case report).

The incidence and mortality rate of gastric cancer are decreasing within last 60 years. Approximately 85% of gastric cancer is carcinoma where as the rest 15% is lymphoma and sarcoma. Leiomyosarcoma is the most frequently encountered sarcoma in stomach and represents 1-3% gastric cancer. This tumor is frequently located in the anterior and posterior wall of gastric fundus accompanied by ulceration and bleeding. Malignant fibrous histiocytoma (MFH) is a soft tissue tumor with poor prognosis, rarely found in stomach. This tumor is most frequently originated from inner fascia or skeletal muscle (…
