Bronchodilators for bronchiolitis.

BACKGROUND: Bronchiolitis is an acute, highly infectious lower respiratory tract infection. Bronchodilators are commonly used in the management of bronchiolitis in North America but not in the United Kingdom. OBJECTIVES: The objective of this review was to assess the effects of bronchodilators on clinical outcomes in infants with acute bronchiolitis. SEARCH STRATEGY: In this updated review, we searched the Cochrane Central Register of Controlled Trials (CENTRAL) and the Database of Abstracts of Reviews of Effects (DARE) (The Cochrane Library, Issue 1, 2006), MEDLINE (1966 to October 2005), E

Partial liquid ventilation for the prevention of mortality and morbidity in paediatric acute lung injury and acute respiratory distress syndrome.

BACKGROUND: Acute lung injury, and acute respiratory distress syndrome, are syndromes of severe respiratory failure. Children with acute lung injury or acute respiratory syndrome have high mortality and significant morbidity. Partial liquid ventilation is proposed as a less injurious form of respiratory support for these children. Uncontrolled studies in adults have shown improvement in gas exchange and lung compliance with partial liquid ventilation A single uncontrolled study in six children with acute respiratory syndrome showed some improvement in gas exchange during three hours of partial

Diazepam for treating tetanus.

BACKGROUND: Clinical management of the muscle spasms and rigidity of tetanus poses a difficult therapeutic problem to physicians everywhere, especially in resource poor countries. There are wide variations in therapeutic regimens commonly used in clinical practice due to uncertainties about effectiveness of conventional drugs. Diazepam compared to other drugs (eg phenobarbitone and chlorpromazine) may have advantages because of combined anticonvulsant, muscle relaxant, sedative and anxiolytic effects. OBJECTIVES: To compare diazepam to other drugs in treating the muscle spasms and rigidity of

Antibiotic prophylaxis for surgery for proximal femoral and other closed long bone fractures.

BACKGROUND: Wound infection and other hospital-acquired infections cause significant morbidity after internal fixation of fractures (osteosynthesis). The administration of antimicrobial agents (antibiotics) may reduce the frequency of infections. OBJECTIVES: To determine whether the prophylactic administration of antibiotics in patients undergoing surgical management of hip or other long bone fractures reduces the incidence of wound and other hospital acquired infections. SEARCH STRATEGY: We searched the The Cochrane Library, Issue 3 2000; MEDLINE, EMBASE, LILACS, Current Contents, Dissertat

Clinical Surveillance Technologies in Nonintensive Care Unit Hospital Settings: Systematic Review and Bayesian Network Meta-Analysis of Randomized Trials.

BACKGROUND: Failure to recognize clinical deterioration in hospitalized patients has prompted the development of rule-based electronic surveillance (RB-ES), predictive model-based electronic surveillance (PM-ES), and continuous physiologic monitoring (CPM). However, their comparative effects on patient-centered outcomes remain uncertain. OBJECTIVE: The aim of this study is to compare the clinical effects of emergent intensive RB-ES, PM-ES, CPM, and local standard care in nonintensive care unit hospital settings. METHODS: We searched PubMed, Embase via online, the Cochrane Central Register of

Is whole-colon investigation by colonoscopy, computerised tomography colonography or barium enema necessary for all patients with colorectal cancer symptoms, and for which patients would flexible sigmoidoscopy suffice? A retrospective cohort study.

BACKGROUND: For patients referred to hospital with suspected colorectal cancer (CRC), it is current standard clinical practice to conduct an examination of the whole colon and rectum. However, studies have shown that an examination of the distal colorectum using flexible sigmoidoscopy (FS) can be a safe and clinically effective investigation for some patients. These findings require validation in a multicentre study. OBJECTIVES: To investigate the links between patient symptoms at presentation and CRC risk by subsite, and to provide evidence of whether or not FS is an effective alternative to

Emergency intubation for acutely ill and injured patients.

BACKGROUND: Emergency intubation has been widely advocated as a life saving procedure in severe acute illness and injury associated with real or potential compromises to the patient's airway and ventilation. However, some initial data have suggested a lack of observed benefit. OBJECTIVES: To determine in acutely ill and injured patients who have real or anticipated problems in maintaining an adequate airway whether emergency endotracheal intubation, as opposed to other airway management techniques, improves the outcome in terms of survival, degree of disability at discharge or length of stay

The role of midwife in postpartum hemorrhage.

BACKGROUND: COVID 19 is the most recent cause of adult respiratory distress syndrome (ARDS). Extracorporeal membrane oxygenation (ECMO) can support gas exchange in patients failing conventional mechanical ventilation, but its role is still controversial. We conducted a systematic review and meta-analysis on ECMO for COVID-associated ARDS to study its outcome. MAIN BODY: CENTRAL, MEDLINE/PubMed, Cochrane Library, and Scopus were systematically searched from inception to May 28, 2020. Studies reporting five or more patients with COVID-19 infection treated venovenous with ECMO were included. The

[P323L Mutation in a Case with Prolonged SARS-CoV-2 PCR Positivity].

Coronavirus disease-2019 (COVID-19) emerged in the last months of 2019 and caused a pandemic effecting the whole world. Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), the causative agent of COVID-19 has changed by various mutations since the day it was first identified, causing the pandemic to continue. Age, male gender, obesity, and comorbidity, which are general risk factors for COVID-19, can also cause prolonged PCR positivity. In this report, a case of 37-year-old male who is working in the hospital's COVID-19 molecular diagnostics laboratory was presented. He was vaccinated

Laparoscopy for the management of acute lower abdominal pain in women of childbearing age.

BACKGROUND: This is an updated version of the original review, published in Issue 1, 2011, of The Cochrane Library. Acute lower abdominal pain is common, and making a diagnosis is particularly challenging in premenopausal women, as ovulation and menstruation symptoms overlap with symptoms of appendicitis, early pregnancy complications and pelvic infection. A management strategy involving early laparoscopy could potentially provide a more accurate diagnosis, earlier treatment and reduced risk of complications. OBJECTIVES: To evaluate the effectiveness and harms of laparoscopy for the managemen

Estimating the effect of maternal viral load on perinatal and postnatal HIV transmission: a systematic review and meta-analysis.

BACKGROUND: Although a growing body of evidence supports zero risk of sexual HIV transmission from a person with sustained virological suppression, known as U=U (undetectable equals untransmittable), data have been insufficient to determine whether this is also true for vertical HIV transmission. We conducted a systematic review and meta-analysis to quantify vertical transmission risk by maternal HIV viral load (mHVL) and to evaluate the applicability of U=U to perinatal and postnatal HIV transmission. METHODS: In this systematic review and meta-analysis, we searched PubMed, Embase, Web of Sc

Postnatal phenobarbital for the prevention of intraventricular hemorrhage in preterm infants.

BACKGROUND: Intraventricular hemorrhage (IVH) is a major complication of preterm birth. Large hemorrhages are associated with a high risk of disability and hydrocephalus. Instability of blood pressure and cerebral blood flow are postulated as causative factors. Another mechanism may involve reperfusion damage from oxygen free radicals. Phenobarbital has been suggested as a safe treatment that stabilises blood pressure and may protect against free radicals. OBJECTIVES: To determine the effect of postnatal administration of phenobarbital on the risk of intraventricular hemorrhage (IVH), neurode

Genetic predisposition to cardiogenic shock complicating acute myocardial infarction: a systematic review.

BACKGROUND: Cardiogenic shock (CS) complicates 5-10% of acute myocardial infarctions (AMI) and remains associated with in-hospital mortality of approximately 40-50%. The clinical predictors established do not fully explain why patients with comparable infarct severity have divergent outcomes, raising the hypothesis that host genetic factors modulate susceptibility to CS. Therefore, we conducted this systematic review to synthesize published evidence describing genetic variants associated with the development of CS complicating AMI. METHODS: PubMed, Embase, Scopus, and the Cochrane Library wer

Bayesian network meta-analysis comparing efficacy of surgical gastrojejunostomy, endoscopic stenting, and EUS-guided gastroenterostomy for malignant gastric outlet obstruction.

BACKGROUND AND AIMS: Malignant gastric outlet obstruction (GOO) substantially impairs quality of life in patients with advanced malignancies. Surgical gastrojejunostomy (S-GJ), endoscopic stenting (ES), and EUS-guided gastroenterostomy (EUS-GE) are the principal palliative options. We performed a Bayesian network meta-analysis to compare their efficacy and safety. METHODS: PubMed, Embase, Web of Science, Scopus, and the Cochrane Library were searched through April 13, 2026. Impact of Donor Obesity on Outcomes in Living Donor Liver Transplantation: A Systematic Review and Meta-analysis.

BACKGROUND: Living donor liver transplantation increasingly faces the question of whether otherwise healthy candidates with a body mass index ≥30 kg/m2 can be safely accepted. Available evidence is uneven: large registry datasets sit alongside heterogeneous single-center series, none quantifying the risk accepted by the donors themselves. We therefore synthesized contemporary comparative evidence in living donor liver transplantation, evaluating donor obesity against donor, recipient, and graft outcomes. METHODS: We searched PubMed/MEDLINE, Cochrane Library, Scopus, Web of Science, and Google
