The purpose of this study was to develop a model of osteoarthrosis of the temporomandibular joint in monkeys, which is remarkably similar in structure and function to that of humans. Nine juvenile monkeys, two as controls and seven as an experimental group, were used in this study. In the experimental group, the articular eminence on both sides was surgically made steeper. Two animals were killed at 1 week, four at 6 months, and one at 1 year postoperatively and the temporomandibular joints were examined macroscopically and microscopically. Typical changes of osteoarthrosis were observed in th

PURPOSE: The rabbit model has been established to mimic the effect of temporomandibular joint (TMJ) arthroplasty of ankylosis, and distraction at the level of the condylar neck is used to elongate the ascending ramus. The histomorphologic changes of TMJ and distraction gap were investigated.
MATERIALS AND METHODS: The unilateral condyles and articular discs were extirpated, and the experimental mandibular rami were shortened by 5 mm. An embedded distracter was used to restore the height of the mandibular ramus by unilateral condylar neck distraction (0.8 mm daily for 7 days). A total of 12 ad

Collaborative care pathways (CCPs) provide a framework for multidisciplinary patient care. They provide guidelines and a mechanism for audit, and were first introduced at the Regional Unit, Walton Hospital, Liverpool, in November 1994. They have been designed for many surgical groups. Between August 1996 and 31 July 1997, 955 patients were admitted on to the nine established pathways: fractured mandible (n=213), fractured zygoma (n=117), minor oral surgery (n=244), abscess (n=18), examination under anaesthesia (n=73), nasal surgery (n=73), osteotomy (n=80), salivary (n=63), and temporomandibul

BACKGROUND: Orthognathic surgery depends on the accurate transfer of virtual surgical planning to the operative field. Although surgical splints play an important role in this process, information about intraoperative adaptation error is limited.
PURPOSE: The purpose of this study was to define and quantify interarch discrepancy during splint-guided orthognathic surgery and to evaluate whether its magnitude differs according to facial symmetry and surgical sequence.
STUDY DESIGN, SETTING, SAMPLE: A retrospective cohort study was conducted at Seoul National University Dental Hospital. Patient

INTRODUCTION: True ankylosis of the temporo-mandibular joint must be differentiated from other reasons why a patient is unable to open his mouth properly. It can be treated by various methods. It is the purpose of this paper to (a) Present various cases with different disorders that had lead to a restricted mouth opening and (b) Report upon the long-term post-operative results achieved by having applied various treatment options for true ankylosis of the temporomandibular joint (TMJ).
MATERIAL AND METHODS: In 40 patients a true ankylosis was treated surgically by applying one of the two proto

OBJECTIVE: The infratemporal fossa (ITF) is a continuation of the temporal fossa between the internal surface of the zygoma and the external surface of the temporal bone and greater wing of the sphenoid bone that is sitting deep to the ramus of the mandible. The principal structure to understanding its relationships is the lateral pterygoid muscle. Other important structures are the medial pterygoid muscle, the maxillary artery, the pterygoid venous plexus, the otic ganglion, the chorda tympani nerve and the mandibular nerve. In this study, we describe the microsurgical anatomy of the ITF, as

Injuries to peripheral branches (IAN, LN, LBN) of the trigeminal nerve during the removal of M3s are known and accepted risks in oral and maxillofacial surgery practice. These risks might be reduced by modifications of evaluation or surgical techniques, depending on the surgeon's judgment in individual patients. If a nerve injury does occur, prompt recognition, subjective and objective evaluation, and development of a treatment plan, if the sensory deficit fails to resolve in a reasonable period and is unacceptable to the patient, give the patient the best chance of achieving improvement or rec

Chronic Recurrent Dislocation of the jaw (CRD) is a relatively frequent occurrence, with a multifactorial aetiology. Both conservative and surgical treatments have been applied to this entity. This paper seeks to review the results of surgical treatment applied according to Norman's technique (1984), with certain modifications, applied to CRD. Ten patients were treated by glenotemporal osteotomy and interposition of bone grafts, (four cranial bone grafts and six iliac crest grafts). Previously, all of the patients had suffered numerous episodes, requiring hospital care on more than one occasio

Coronectomy is a surgical procedure for the treatment of mandibular third molars in close proximity to the mandibular canal. Unfortunately, often the surgical protocol is not described step by step and it is difficult for the clinician to assess the key factors that are important for the success of this procedure. The aim of this paper is to propose and describe a standardized surgical protocol to improve the success of the technique. The treatment approach, for the most common types of third molars impaction is analysed. Each step of the surgical procedure is described in details and a new ty

BACKGROUND: The aggressive type lesions of central giant cell granuloma (CGCG) require wide resection that leads to major defects in the jaws. This form of surgical treatment can be particularly disfiguring. A number of alternative non-surgical therapies have been advocated in recent years for the management of the central giant cell granuloma (CGCG). These include calcitonin injections and nasal spray, intralesional steroid injections and subcutaneous interferon injections.
MATERIALS AND METHODS: A large central giant cell granuloma aggressive type lesion in the mandible of a 24-year-old pat

This study is a randomized control split-mouth non-blinded prospective trial, the aim of which was to evaluate clinically the frequency and type of injury to the inferior alveolar nerve (IAN) following mandibular third molar surgery using or not using the tooth section technique. The sample consisted of 50 lower third molars in 25 patients, in the control group the tooth section technique was not used, and it was used in the experimental group. The outcomes have shown a frequency of 8% of IAN injury for both groups (in both genders), and there were no statistically significant associations bet

OBJECTIVE: To obtain accurate occlusion in mandibular reconstruction when the tumor is exophytic, which makes prebending the plate impossible.
DESIGN AND SETTING: Use of computed tomography-based resin models and external fixation devices is combined in a tertiary academic center. The exophytic part of the tumor on the model is burred off, and the reconstruction plate is bent on the model before surgery. Temporary external fixation is applied before resection, and a precontoured plate is applied following segmental resection before the release of external fixation.
RESULTS: Six patients unde

Ameloblastoma, a benign tumor of odontogenic type, represents 10% of all tumors of the jawbone. It is localized in the mandible in 80% of cases and in the upper jaw in the remaining 20%. In every case, the selection of the surgical treatment to be applied must consider some fundamental elements, including the age and general state of health of the patient, the clinicopathological variant, and the localization and extent of the tumor. In addition, it is necessary to evaluate whether the neoplasm to be treated is a primitive lesion or a recurrence. Although ameloblastoma has relative histologica
