Occlusal reconstruction of a patient with ameloblastoma ablation using alveolar distraction osteogenesis: a case report.

BACKGROUND: Ameloblastoma is one of the most common benign odontogenic neoplasms. Its surgical excision has the potential to lead to postoperative malocclusion. In this case report, we describe the successful interdisciplinary orthodontic treatment of a patient with ameloblastoma who underwent marginal mandibulectomy. CASE PRESENTATION: A woman of 20-year-old was diagnosed with ameloblastoma, and underwent marginal mandibulectomy when she was 8 years of age. She had an excessive overjet (11.5 mm) and a mild open bite (- 1.5 mm) with a severely resorbed atrophic edentulous ridge in the area ar

Mandibular Movement Restoration in a Child with Bilateral Coronoid Hyperplasia: A Case Report.

BACKGROUND: Coronoid process hyperplasia is an uncommon finding, characterized by an enlargement of the coronoid process, causing a mechanical obstacle by its interposing in the posterior portion of the maxilla or zygomatic arch. CASE PRESENTATION: The article presents a case report of a bilateral coronoid process hyperplasia in a 3-year-old girl demonstrated with inability to open the mouth and restricted jaw movement. Panoramic x-ray and 3-dimensional computed tomographic reconstruction showed bilateral elongation of the coronoid processes associated with deformation of the mandibular condy

Multidisciplinary treatment approach in Treacher Collins syndrome.

Treacher Collins syndrome (TCS) is a common genetic disorder with high penetrance and phenotypic variability. First and second branchial arches are affected in TCS, resulting in craniofacial and intraoral anomalies such as: severe convex facial profile; mid-face hypoplasia; microtia; eyelid colobomas; mandibular retrognathism; cleft palate; dental hypoplasia; heterotopic teeth; maxillary transverse hypoplasia; anterior open bite; and Angle Class II molar relationship. A high incidence of caries is also a typical finding in TCS patients. Nonetheless, even simple dental restorative procedures ca

SAPHO syndrome with TMJ involvement: review of the literature and case presentation.

Synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome is a rare group of sterile, inflammatory osteoarticular disorders classically associated with skin manifestations. The etiology is unknown but probably involves genetic, infectious, and immunological components. The characteristic feature of the disease is found in the bone lesions, which typically involve the anterior chest wall and axial skeleton. In the literature review, six case reports discussed involvement of the TMJ. Treatment of SAPHO is geared toward symptom management as there is no cure. Surgery for mandibular les

[Position of the disc in collum mandibulae fractures--a magnetic resonance tomography study].

39 condylar mandibular fractures were examined by magnetic resonance imaging (MRI) within the first 12 hours after trauma prior to therapy. A new classification has been applied on the traumatic disc displacements. Three types A, B, C can be distinguished. Type A means no displacement in relation to the condylar fragment and the fossa. Type B means a displacement in the transversal plane exceeding the physiologic range of movement. Type C means displacement in the sagittal as well as in the transversal plane which exceeds the physiologic range of movement of the disc. The classified type of di

Prosthodontic considerations for patients with cleft lip and palate.

The prosthodontist provides the final active treatment for the patient with a cleft. He must anticipate and decide upon the prosthodontic procedures in collaboration with the plastic surgeon and/or the orthodontist during the period of their interventions. The purpose of prosthodontic treatment is to prevent the relapse of the maxillary segments and the teeth after surgical and/or orthodontic correction, as well as the rehabilitation of mastication, speech and aesthetics. The individual peculiarities of the cleft patient should be taken into consideration from the beginning. They are: discrepa

Osseous anatomy of unilateral coronal synostosis.

High resolution, thin slice computerized tomography (CT) scans with paraxial and three-dimensional surface reconstructions were utilized to document the endocranial, exocranial, and orbital anatomy of non-syndromal unicoronal synostosis (UCS). Eighteen patients with UCS were evaluated qualitatively and quantitatively. Of these, 10 were studied both preoperatively and 1 year postoperatively. The endocranial base in UCS is characterized by a 9 degree angulation toward the synostosis of the anterior cranial base with respect to the posterior cranial base. The exocranial base has a 7 degree angula

[Understanding and comments of the series of uncertain viewpoints on the diagnosis and treatment of AAOMS medication-related osteonecrosis of the jaw].

This article focuses on the series of viewpoints on the prevention and treatment of medication-related osteonecrosis of the jaw issued by the American Association of Oral and Maxillofacial Surgery (AAOMS), and puts forward several uncertain issues from a clinical perspective. The points of doubt in diagnosis include the definition of drug categories and the threshold of bone exposure duration, as well as the significance of imaging features of bone lesions for diagnostic and treatment decisions. The concerns in terms of prevention include drug risks and disease incidence, tooth extraction and

Psycho-social impact of orthogathic sugery.

BACKGROUND: Orthognathic surgery is a branch of maxillofacial surgery. It carries out the treatment of the facial skeleton asymmetries and deformities. The patients who ask for this surgery are often young people who usually refer symptoms related to dental malocclusion, difficulty eating and temporo-mandibular pain. These physical symptoms are often accompanied by psychological symptoms triggered by their physical appearance such as low self-esteem, self-confidence and negativism about their social and emotional future. MATERIAL AND METHODS: Patients with skeletal malformation of facial bone

Sleep apnea and sleep-related breathing disorders in patients with craniofacial synostosis.

Seventeen patients with craniofacial synostosis (CFS) have been treated at the Department of Plastic and Reconstructive Surgery at Osaka Medical College during the past 10 years. Six patients were thoroughly evaluated at the Department of Otolaryngology by polysomnography (PSG), cephalometric X-ray and nasopharyngoscopy during sleep. In 4 of the patients PSG showed obstructive sleep apnea syndromes (OSAS). Heavy snoring without apnea and paradoxical respiration were noted in the other 2 patients. Thus, all of the patients had sleep-related breathing disorders (SRBD). It is assumed that the inc

The Sternocleidomastoid Flap for Oral Cavity Reconstruction: Extended Indications and Technical Modifications.

PURPOSE: Several investigators have found that preserving the superior thyroid artery flap can considerably increase the survival rate of the sternocleidomastoid (SCM) flap. Nevertheless, they have recommended not cleaning the lymph nodes above level II for occipital artery preservation, which can leave risky metastatic lymph nodes and restrict the application of the SCM flap in patients with at least stage N1. The authors propose that preserving only the superior thyroid arteriovenous system is sufficient to ensure survival of the SCM flap and preserving the occipital artery is not necessary.

The Fibula Free Flap as Used for Reconstruction in Patients With Oral Cavity Malignancy: A Systematic Review.

OBJECTIVES: The fibula free flap is a widely used means of reconstructing the maxilla and mandible. It is used for a variety of indications, including for reconstruction after resection of a benign or malignant tumor, osteoradionecrosis, or trauma. The current literature reports outcomes for the fibula free flap across all indications, but there is a paucity of information regarding outcomes when the flap is performed exclusively after resection of malignancy. The authors report a systematic review of the literature and report flap survival rates, flap complications, donor site complications,

Management of refractured mandibular condyle after open reduction and internal fixation.

The purpose of the present case is to highlight the surgical management of the refractured condylar segment of the mandible after open reduction and internal fixation. A man in his 20s reported to the department with an alleged history of road traffic injury (RTI). He was diagnosed with a fracture of the right parasymphysis and left subcondylar region of the mandible. He was managed surgically using 2 mini plates of 2.0 mm in relation to the condyle, one 5-hole miniplate in relation to the posterior border of the mandible, one 3-hole miniplate in relation to the anterior border of the condyle.

Endonasal Free Flap Reconstruction Combined With Draf Frontal Sinusotomy for Complex Cerebrospinal Fluid Leak: A Technical Report & Case Series.

BACKGROUND: Frontal sinus cranialization with closure via bifrontal pericranial flaps is the gold standard for separating the nasofrontal recess from the intracranial cavity for posterior table defects. Despite the high success rate, cerebrospinal fluid (CSF) leak may persist and is particularly challenging when vascularized reconstructive options from the bicoronal incision are exhausted. OBJECTIVE: To assess a novel endonasal technique using an adipofascial radial forearm free flap delivered to the frontal recess through a Draf sinusotomy to repair complex CSF leaks from the frontal sinus.

A prospective study of implant-supported full-arch yttria-stabilized tetragonal zirconia polycrystal mandibular fixed dental prostheses: three-year results.

PURPOSE: The development of high-strength oxide ceramic materials has led to increased interest in all-ceramic fixed dental prostheses (FDPs). Success rates reported in clinical studies for all-ceramic FDPs based on high-strength oxide ceramic materials reportedly approach those of porcelain-fused-to-metal FDPs. These reconstructions, however, are still of limited size and have mainly concerned FDPs supported by natural teeth. The purpose of this study was to evaluate the clinical performance of multiunit all-ceramic FDPs supported by dental implants. MATERIALS AND METHODS: Ten patients recei

Culture and differentiation of osteoblasts on coral scaffold from human bone marrow mesenchymal stem cells.

In this paper we describe an approach that aims to provide fundamental information towards a scientific, biomechanical basis for the use of … lications on over a thousand patients with different bone pathologies including spinal stenosis and mandibular reconstruction. It is well accepted that human bone marrow (hBM) is an exceptional source of mesenchymal stem cells (MSCs), which may differentiate into different cell phenotypes such as osteoblasts, chondrocytes, adipocytes, myocytes, cardiomyocytes and neurons. …
