Acute oral toxicity in mice of a new palytoxin analog: 42-hydroxy-palytoxin.

The acute oral toxicity of a new palytoxin congener, 42-hydroxy-palytoxin (42-OH-PLTX), was investigated in female CD-1 mice. The toxin (300-1697 μg/kg), administered by gavage, induced scratching, jumping, respiratory distress, cyanosis, paralysis and death of mice, with an LD₅₀ of 651 μg/kg (95% confidence limits: 384-1018 μg/kg) within 24 h. Hematoclinical analyses showed increased plasma levels of lactate dehydrogenase and aspartate-aminotransferase at doses of 600 μg/kg and above, as well as of alanine-aminotransferase, creatine phosphokinase and potassium ions at ≥ 848 μg/kg. Histology r

Laterofixation of the vocal fold using an endo-extralaryngeal needle carrier for bilateral vocal fold paralysis.

OBJECTIVES: Laterofixation of the vocal fold is a simple and reliable surgical intervention for laryngeal obstruction due to bilateral vocal fold fixation to obtain sufficient glottal space. Nevertheless, it has some technical disadvantages. This report summarizes the surgical outcomes in patients who underwent laterofixation of the vocal fold with or without the use of an endo-extralaryngeal needle carrier (EENC). METHODS: A prospective study of 11 consecutive patients with bilateral vocal fold paralysis. All of the patients underwent unilateral vocal fold laterofixation. Six patients were a

Laryngeal reflex before and after placement of airway interventions: endotracheal tube and laryngeal mask airway.

BACKGROUND: Previous reports indicate that detrimental laryngeal function persists over several hours after tracheal extubation even in patients who have regained full consciousness from anesthesia. The authors hypothesize that even after minor surgery, the presence of an endotracheal tube (ETT) impairs the receptors at the vocal cord and diminishes the defensive laryngeal function. The hypothesis was tested by comparing types of experimentally induced laryngeal airway reflexes before and after surgery in anesthetized patients with use of either an ETT or a Laryngeal Mask Airway. METHODS: Twe

A novel type of encephalopathy associated with mushroom Sugihiratake ingestion in patients with chronic kidney diseases.

BACKGROUND: The etiology of encephalopathy in uremic patients is multiple. We recently encountered a novel type of encephalopathy which occurred exclusively in patients with chronic kidney diseases after ingestion of a mushroom called Sugihiratake. While the exact etiology of this encephalopathy remained mysterious, we aimed to describe its clinical features. METHODS: A total of 32 patients with chronic kidney diseases who had presented with encephalopathy following ingestion of Sugihiratake were enrolled from seven prefectures in Japan., with 24 of the 32 patients undergoing regular hemodial

Prolonged paralysis after neuromuscular blockade.

Non-depolarizing neuromuscular blocking agents have been used with increasing frequency in critically ill patients. Recently, numerous reports have described patients with prolonged muscle weakness after use of these agents for more than two days. Brief weakness lasting several hours to several days is likely the result of prolonged neuromuscular blockade, while more prolonged weakness lasting several weeks to months is likely caused by a myopathy. Specific features of this myopathic disorder are reviewed. Clinically, patients have flaccid paralysis with intact sensation and cognition. Electro

Acute electrical burns.

Electric current can damage an individual by thermal heating of the tissues; by disregulating autonomously functioning organ systems, such as the circulatory and respiratory systems; or by once-only or continuing stimulation of the nerves and striated muscles. In the first case, the result is coagulation necrosis, and in the second case life-threatening events can occur, such as arrhythmia or apnea. In the third case, vigorous nerve stimulation can lead to paralysis and vasospasm. Massive muscle contractions due to nerve stimulation or the direct triggering of striated muscles can cause ruptur

Radiographic manifestations of congenital anomalies affecting the airway.

Congenital anomalies of the airway are generally uncommon, but a vast array of possibilities exists. Some present life-threatening emergencies at birth, and others go undiagnosed for years. Clinical symptoms are often nonspecific, and radiographic evaluation is frequently requested to localize and characterize the lesion before endoscopy, surgery, or medical management. The most common intrinsic congenital anomalies causing airway compromise in infants include choanal atresia, mandibular hypoplasia, laryngomalacia, vocal cord paralysis, and congenital subglottic stenosis. The radiologist must

Toxicoses in livestock from the hemlocks (Conium and Cicuta spp.).

The hemlocks, Conium maculatum (poison-hemlock) and Cicuta spp. (waterhemlock), are poisonous plants that cause sizeable losss to the livestock industry. Clinical signs of poisonhemlock toxicosis are similar in all species of livestock and include muscular weakness, incordination, trembling, initial central nervous system stimulation, depression and death from respiratory paralysis. Poison-hemlock also causes skeletal defects in the offspring of cattle, pigs and sheep and cleft palate in pigs when ingested during specific periods of gestation. The primary toxicants in poison-hemlock are coniin…

A Case Report of Acute Intermittent Porphyria Accompanied by Severe Peripheral Neuropathy.

Background: Acute intermittent porphyria (AIP) is the most common and severe form of acute hepatic porphyria, caused by heterozygous mutations in the HMBS gene. Due to its non-specific clinical manifestations and low clinical awareness among clinicians, AIP is frequently misdiagnosed, leading to significant diagnostic delays and potentially fatal complications. Case presentation: We report a 20-year-old female patient who presented with a 9-month history of recurrent abdominal pain, paralytic ileus, unexplained liver injury, and hyponatremia, followed by progressive limb weakness. She was init

Rare Abdominal Pain Onset in Guillain-Barré Syndrome: A Case Report of Acute Motor Sensory Axonal Neuropathy.

BACKGROUND Guillain-Barre syndrome (GBS) typically presents with ascending weakness and sensory disturbances. However, initial symptoms like abdominal pain are rare and can make diagnosis more challenging, as this is not a common feature of GBS. This diagnostic delay can be particularly significant in severe GBS subtypes like acute motor and sensory axonal neuropathy (AMSAN), with only a few reported cases presenting in this atypical manner. CASE REPORT A 42-year-old man experienced 3 days of persistent abdominal pain that progressively disrupted his normal activities. Soon after, he developed

The impact of tracheostomy timing on the outcomes of patients with acute traumatic spinal cord injury: A systematic review and meta-analysis.

BACKGROUND: Spinal cord injury (SCI) can cause serious respiratory problems. Cervical high-level injuries may result in diaphragm paralysis, necessitating tracheostomy to assist airway protection and facilitate breathing. METHODS: A comprehensive literature search of PubMed, Google Scholar, and Web of Science was performed for published studies comparing outcomes between early versus late tracheostomy in acute traumatic SCI patients. RESULTS: The initial search returned 1837 articles, after the final review, 17 studies with a total of 3853 patients were included in the meta-analysis. The mor

ICU-acquired weakness: Critical illness myopathy and polyneuropathy.

Critical illness myopathy (CIM) and critical illness polyneuropathy (CIP) are significant complications in intensive care unit (ICU) patients, first identified in the late 20th century. These conditions often present as flaccid paralysis and respiratory muscle weakness, making it challenging for patients to wean off ventilatory support. The incidence of ICU-acquired weakness, which includes CIM and CIP, ranges from 25 % to 84 % among long-term ventilated patients, with higher rates observed in those with sepsis or systemic inflammatory response syndrome. CIM is characterized by muscle wasting

Neutralizing antibody landscape of the non-polio Enteroviruses and future strategy.

The non-polio Enteroviruses (NPEVs), consist of enteroviruses, coxsackieviruses, echoviruses, and rhinoviruses, are causative agents for a wide variety of diseases, ranging from common cold to encephalitis and acute flaccid paralysis (AFP). In recent years, several NPEVs have become serious public health threats, include EV-A71, which has caused epidemics of hand-foot-and-mouth disease (HMFD) in Southeast Asia, and EV-D68, which caused outbreaks of severe respiratory disease in children worldwide. Infections with these viruses are associated with neurological diseases like aseptic meningitis a

Current status of Guillain-Barré syndrome (GBS) in China: a 10-year comprehensive overview.

Guillain-Barré syndrome (GBS) is an acute inflammatory polyradiculoneuropathy; a disease involving the peripheral nervous system which is the most common cause of acute flaccid paralysis worldwide. So far, it is still lack of a comprehensive overview and understanding of the national epidemiological, clinical characteristics, and the risk factors of GBS in China, as well as differences between China and other countries and regions in these respects. With the global outbreak of the coronavirus disease 2019 (COVID-19), an epidemiological or phenotypic association between severe acute respiratory

Guillain-Barré Syndrome Presenting as Painful Weakness and Edema of the Legs: A Case Report.

Guillain-Barré syndrome (GBS) is an autoimmune inflammatory polyneuropathy, which can be challenging to diagnose due to variability in the initial presenting features. Pain, flaccid paresis, motor sensory disturbance, hyporeflexia, and autonomic dysfunction are the typical manifestations, although atypical features, such as ataxia, neck stiffness, dysphagia, ophthalmoplegia, bulbar palsy, and isolated upper limb weakness, may be seen. It may also progress to fatal respiratory depression. As such, timely diagnosis and treatment are essential. We present the case of a 41-year-old man who present

Emerging trends and insights in acute flaccid myelitis: a comprehensive review of neurologic manifestations.

Acute Flaccid Myelitis (AFM) is a neurological condition in the anterior portion of the spinal cord and can be characterised as paraplegia (paralysis of the lower limbs), and cranial nerve dysfunction. These lesions are caused by the infection due to Enterovirus 68 (EV-D68); a member of the Enterovirus (EV) family belongs to the Enterovirus species within the Picornavirus family and a Polio-like virus. In many cases, the facial, axial, bulbar, respiratory, and extraocular muscles were affected, hence reducing the overall quality of the patient's life. Moreover, severe pathological conditions d
