Volar percutaneous screw fixation for scaphoid nonunion.

BACKGROUND: Percutaneous screw fixation is widely used in acute fractures of the scaphoid. In this study, we aimed to present our results with volar percutaneous screw fixation in patients with scaphoid nonunions. METHODOLOGY: A total of 12 patients with scaphoid nonunion (≥13 weeks) that underwent volar percutaneous screw fixation were evaluated retrospectively. Two of the patients were female, and 10 were male. Mean age was 27 years (range = 19-41). The mean time that elapsed between the fracture and surgical procedure was 7.5 months (range = 4-12). According to the anatomic location, 33.3%

3-Dimensional Printing-Assisted Percutaneous Fixation for Acute Scaphoid Fracture: 1-Shot Procedure.

Scaphoid fractures are the most common carpal fractures. Percutaneous fixation is a popular treatment option for nondisplaced or minimally displaced scaphoid fractures. Placement of the guidewire in the percutaneous fixation procedure often requires several attempts and adjustments with the help of intraoperative fluoroscopy. This article describes a 1-shot procedure of percutaneous fixation for acute scaphoid fracture with patient-specific guiding templates made by computer reconstructions and 3-dimensional printing technique.

The morphology of proximal pole scaphoid fractures: implications for optimal screw placement.

The purpose of this study was to measure the radiographic parameters of proximal pole scaphoid fractures, and calculate the ideal starting points and trajectories for antegrade screw insertion. Computed tomography scans of 19 consecutive patients with proximal pole fractures were studied using open source digital imaging and communications in medicine (DICOM) imaging measurement software. For scaphoid sagittal measurements, fracture inclination was measured with respect to the scaphoid axis. The ideal starting point for a screw in the proximal pole fragment was then identified on the scaphoid

Incomplete scapholunate and trapeziotrapezoid coalitions with an accessory carpal bone.

Coalitions of the carpal bones associated with accessory carpal bones are relatively uncommon. We present a case of scapholunate and trapeziotrapezoid incomplete coalition with a dorsally situated accessory bone between trapezium and trapezoideum. X-ray, computed tomography, and magnetic resonance imaging findings of the patient are discussed in light of the pertinent literature.

Bilateral Ulnar Deviation Supination Stress Test to Assess Dynamic Scapholunate Instability.

PURPOSE: We describe a new radiologic test to assess the integrity of the scapholunate ligament in dynamic scapholunate dysfunction. METHODS: A bilateral forearm-holding device was designed to perform a comparative radiographic assessment of the scapholunate joint gap during resisted isometric contraction of the extensor carpi ulnaris muscle with full supination of the forearm. The concept is based on the known scaphoid pronation effect of this muscle. Clinical data from 12 patients were collected retrospectively and used to analyze the patients' symptomatic and asymptomatic (contralateral) w

Scaphoid fracture geometrics: an assessment of location and orientation.

The location and angle of scaphoid fractures are important attributes which guide management. We used a 3 dimensional scaphoid model, generated from CT scans, to map scaphoid fracture planes. The point at which the fracture plane crossed the central axis of the scaphoid was noted. The angle of the fracture planes with regard to the central axis was also noted. This allowed calculation of the location of the fracture and the angle of the 379 fractures. The mean point of intersection for fractures with the scaphoid axis was 50% along the scaphoid. Sixty percent of all fractures were found around

Pregnancy-Related Ligamentous Laxity Mimicking Dynamic Scapholunate Instability: A Case Report.

CASE: A 29-year-old woman presented with spontaneous, isolated, total palmar scaphoid subluxation in the left hand approximately 6 weeks postpartum. She had a positive Watson scaphoid shift test, with an easily subluxable and reducible scaphoid unilaterally. She was diagnosed with scapholunate ligamentous laxity with dynamic instability. Approximately 4 months after stopping lactation, she had complete resolution of the scapholunate subluxation; there was no recurrence of symptoms over the next 5 years of follow-up. CONCLUSION: Women can have manifestations of pregnancy and lactation-related

Traction radiography for the diagnosis of scapholunate ligament tears: an experimental cadaver study.

The aim of this experimental cadaver study was to verify that thumb traction radiography can be used to diagnose scapholunate interosseous ligament (SLIL) injury. Eight cadaver forearms were positioned vertically so that the thumb could be held in a Chinese finger trap and traction force applied using a 5 kg weight. Fluoroscopy was performed with the thumb unloaded and under traction, and then unloaded and under traction after division of the SLIL. The scapholunate joint gaps were measured electronically. The difference between the unloaded and loaded wrists with sectioned SLIL was not statist

Radiographic evaluation of the modified Brunelli technique versus the Blatt capsulodesis for scapholunate dissociation in a cadaver model.

PURPOSE: A variety of soft tissue surgical procedures have been developed for treatment of scapholunate (SL) dissociation. The purpose of this study was to compare the degree of correction obtained (as measured on preoperative and postoperative radiographs) when performing the modified Brunelli technique (MBT) with that of the more commonly performed Blatt capsulodesis (BC) and to evaluate each technique after simulated wrist motion. METHODS: Five cadaver wrists were used for this study. The SL interval, SL angle, and radiolunate angle were recorded radiographically, with the SL ligament inta

An international survey of hospital practice in the imaging of acute scaphoid trauma.

OBJECTIVE: Scaphoid fractures are relatively common. If not treated promptly there may be risk of long-term disability. However, unnecessary wrist immobilization is inconvenient and may hinder professional activities. Therefore, early accurate diagnosis is essential. Currently, the American College of Radiology deems MRI and radiographs as the most appropriate investigations in imaging acute scaphoid trauma. Our objective was to assess scaphoid imaging protocols. MATERIALS AND METHODS: To assess scaphoid imaging protocols, an international survey of imaging practice was performed. Two hundred
