BACKGROUND/PURPOSE: This study was performed to assess the long-term follow-up of five patients who underwent one-stage ileoendorectal pull- … ne patient 2 months postoperatively. In the second patient a functional obstruction was relieved by sphincterotomy.
CONCLUSIONS: Ileoendorectal pull-through with right colon onlay patch is associated with few early and late postoperative complications; it appears to be superior to other procedures in the early postoperative period because of the more rapid retu …

In a prospective study on 169 patients with a so-called high fistula-in-ano (147 transsphincteric, 22 suprasphincteric) the value of a sphincter-saving operation technique was assessed. This technique consists of one-stage fistulectomy as well as of drainage of the intersphincteric space by internal sphincterotomy. The site of the former primary orifice of the fistula is adapted by multiple peranally performed single stitches. The perianal part of the wound is left to heal by second intention. Post-operatively, 19 cases of suture leakage occurred (9.5% with the transsphincteric and 23% with th…

Between 1979 and 1989, forty-six patients with biliary lithiasis were treated by non-surgical methods. Jaundice was present in 32 (70%) patients. In five patients (11%), the bile stones were found during the routine ERCP for cholelithiasis. Five patients (11%) had stones with diameter of 30 mm and over. Clearing of biliary ducts was achieved in 44 patients (95.7%). Forty-five patients required endoscopic sphincterotomy, the other one, had a surgical sphincteroplasty performed at the time of cholecystectomy. The stones passed spontaneously in eight patients (17.4%), Dormia basket, shock-wave an…

Fifteen patients with mean age 74 (range 34-94) years were treated with extracorporeal shock-wave lithotripsy (ESWL) for bile-duct stones (intrahepatic in 4 cases) following failure of surgical or endoscopic treatment. A Dornier HM3 lithotriptor was used, and in all cases the ESWL session was preceded by establishment of external biliary drainage (endoscopic nasobiliary in 9, percutaneous transhepatic in 2 and a surgical T-tube in 4 cases) for radiologic positioning of stone. Endoscopic (14 cases) or surgical (1 case) sphincterotomy was done to facilitate spontaneous passage or endoscopic remo…

BACKGROUND: Biliary leakages are notable complications that can occur following hepatobiliary surgery, traumatic injury, or iatrogenic cause … specific context of the injury. Endoscopic retrograde cholangiopancreatography in combination with sphincterotomy and stent placement is widely regarded as the first-line treatment when the papilla is conventionally accessible.

Biliary lithiasis is common worldwide, affecting almost 20% of the general population, though few experience symptoms. The frequency of choledocholithiasis in patients with symptomatic cholelithiasis is estimated to be 10-33%, depending on patients' age. Unlike gallbladder lithiasis, the medical and surgical treatment of common bile duct stones is uncertain, having changed over the last few years. The prior gold standard treatment for cholelithiasis and choledocholithiasis was open cholecystectomy with bile duct clearance, choledochotomy, and/or surgical sphincterotomy. In the last 10-15 years…

An 86-year-old woman presented with a history of endoscopic papillary sphincterotomy for bile duct stones and diverticulitis. The patient was admitted as an emergency case of acute cholangitis due to choledocholithiasis, underwent endoscopic bile duct stenting, and was discharged with a plan for endoscopic lithotripsy. One month later, the patient was readmitted owing to abdominal pain. Abdominal computed tomography at admission showed that the bile duct stent had migrated to the sigmoid colon and the presence of a small amount of extraintestinal gas, suggesting a colonic perforation. Lower ga…

Beyond classical anticoagulants (vitamin K antagonists, dicumarolici), the so-called "direct-acting anticoagulants" have been recently introduced, and management of these drugs in before an endoscopic examination is different. Solitary duodenal Peutz-Jeghers (PJ)-type hamartomatous polyps are rare and considered a different disease entity than classic PJ syndrome. We describe the case of an 89-year-old man admitted to our emergency department with symptoms of acute cholangitis, liver dysfunction, and slight jaundice. Magnetic resonance imaging showed multiple signal voids, reflecting choledocholithiasis, and an oval-shaped tumor in the common bile duct (CBD). Following endoscopic retrograde cholangiopancreatography, the patient was diagnosed with a lower CBD tumor 20 mm in diameter. Endoscopic sphincterotomy was per…

The analysis of the biopsy specimen confirmed this mass to be an IPMN of the pancreas …
