PURPOSE OF THE STUDY: The authors deal with surgical treatment of osteoarthritis of carpometacarpal joint of the thumb. They present results of three different techniques of interposition arthroplasty.
MATERIAL: Thirteen operations were carried out in the period of 1997-1999 in 12 patients (9 women and 3 men). In seven cases the dominant right hand was affected, in 4 cases the left hand was involved. The average age was 56 years (range, 48-91 years). In none of the patients any extreme activity of the operated on hand is expected in future.
METHODS: The surgical treatment consists in resecti

The purpose of this study was to evaluate the results of full versus partial subperiosteal fibular bone graft for subtalar joint arthrodesis in patients with pes planovalgus foot deformity associated with residual polio myelitis. A prospective study was carried out on 16 patients with postpoliomyelitis valgus foot deformity secondary to invertor muscle paralysis. All patients were treated by peroneal tendon transfer to the medial metatarsals and subtalar extra-articular arthrodesis. In 12 patients, subperiosteal partial fibular graft was used, while four patients received a full fibular graft.…

The lower ankle joint is highly variable in ligamental guidance. It has to be considered in unity with the upper ankle joint. Biomechanical investigations have shown the posterior lower ankle joint to be relatively unstable primarily in supination position. Radiological techniques are available to procedure radiographic evidence to that clinical instability of the lower ankle joint. A surgical approach to instability of both upper and lower ankle joints is proposed in this paper and is discussed in some detail.

Most of the operative techniques described for the treatment of recurrent anterior dislocations or subluxations of the shoulder have two distinct disadvantages: the need to immobilize the arm for several weeks and loss of external rotation. These disadvantages provided an incentive for conducting a clinical evaluation of an operation originally described by Boytchev and modified by the author. In this procedure, the conjoined tendons of the coracobrachialis and the short head of the biceps with the detached tip of the coracoid process are rerouted posterior to the subscapularis muscle and reat

Fifty-four patients with congenital clubfoot (total: 82 club feet) were observed at the 2nd Department of Orthopaedic Surgery of Rome University from 1970 to 1980. The conclusions of this study were the following: The patients with congenital clubfoot who had a uniform treatment from the beginning at the same hospital obtained better results than those who received previous treatments at other hospitals. A relapse was considered as a recurrence of the initial deformity after complete recovery and not as an incompletely corrected congenital clubfoot. In many of the patients with clubfoot who we

In cases of established calcaneus after anterior poliomyelitis the deformity can be greatly reduced by combining an extensive plantar release with an oblique transverse osteotomy of the calcaneus that permits displacement upwards and backwards of the posterior weight-bearing part of the bone. The procedure greatly improves the mechanical advantage of subsequent tendon transplantations to the heel. Between 1956 and 1969 fifteen such osteotomies were carried out and the long-term results have been reviewed.

Rupture of the anterior cruciate ligament constitutes a major disability in young athletes and those patients with persistent disabling instability who have failed to respond to conservative treatment. A new method of reconstruction by means of semitendinsous tenodesis was used in five patients with satisfactory end results after an average follow-up of 21.4 months. Limited clinical experience indicates that with this method less anteroposterior instability persists than with other methods.

Hallux varus deformity most commonly occurs as a complication of bunion surgery. Surgical option depends on the underlying cause, flexibility of the deformity, and presence of osteoarthritis of the first metatarsophalangeal joint. Joint-preserving surgery including medial soft tissue release, metatarsal osteotomy, and tendon transfer can be considered in flexible deformity without degeneration of the first metatarsophalangeal joint. First metatarsophalangeal arthrodesis is indicated in cases of inflammatory arthritis, avascular necrosis, osteoarthritis, neuromuscular disorder, or failed previo…

Supermicrosurgery is sophisticated microsurgical technique, which allows dissection and anastomosis of blood/lymphatic vessels and nerves with external diameter of 0.5 mm or smaller. With increasing attention to quality of life of cancer survivors, less invasive and functionally-better oncological reconstruction using supermicrosurgical techniques is warranted. Unlike conventional free flap reconstruction, supermicrosurgical free flaps can be elevated from anywhere using innominate vessels with diameter of 0.1 mm or larger, allowing patient-oriented least invasive reconstruction. Since lymphat

The causes of metatarsalgia are classified as primary, secondary, and iatrogenic. Anatomical and biomechanical considerations separate "static" from "propulsive" forms of metatarsalgia. The physical examination should be combined with an assessment of weight-bearing radiographs and, if needed, of ultrasound or magnetic resonance imaging scans. The first-line treatment is conservative (stretching exercises, footwear modification, insoles, and lesion debridement). Soft-tissue surgical procedures (gastrocnemius muscle recession, tendon transfer, and plantar plate repair) should also be considered…

Patellar instability is a frequent cause of knee pain in childhood and adolescence. Treatment is usually conservative especially after a single episode of dislocation, although in cases of recurrence the surgical option needs to be evaluated. The techniques proposed involve only soft tissues in order to preserve the skeletal maturation. This study evaluates the long-term results on 20 knees in 19 adolescents affected by patellar instability treated with the distal realignment procedure involving medial transfer of the lateral hemi-patellar tendon according to Goldthwait technique, preceded by

Fascial sling surgery for severe congenital ptosis yields superior results when the operative result is dynamic rather than merely suspensory. The sling itself is attached as in a tendon transfer solely to the frontalis muscle, minimizing adherence to the underlying periosteum and the overlying dermis. The sling is passed behind a pulley created by the superior transverse ligament so as to produce a more normal vector of pull. The dermis of the skin is attached to the sling to create a dynamic supratarsal crease. Postoperative retraining allows the patient to achieve more than one centimeter o…

Limited function due to paralysis following brachial plexus lesions can be improved by secondary operations of the bony and soft tissue. Between April 1994 and December 2000, 109 patients suffering from arm-plexus lesions underwent a total of 144 reconstructive operations guided by our concept of integrated therapy. The average age at the time of surgery was 32 years (range: 15-59). The following operations were performed: shoulder arthrodesis (23), trapezius transfer (74), rotation osteotomy of humerus (9), triceps to biceps transposition (9), transposition of forearm flexors or extensors (8)

PURPOSE: Peroneus tertius (PT) or Fibularis tertius, a muscle of the anterior compartment of the leg is very distinctive to the Homo sapiens. This is because of the evolutionary acquisition of bipedal gait along with the eversion of the foot, which are unique to humans. It is considered as the fifth tendon of the extensor digitorum longus. Variations in the attachments of PT can cause stress fractures like the Jones fracture. PT has been extensively used in tendoplasty, tendon transfer and resection of the foot. The study aims to transpose the knowledge in variations of the morphology of PT fr…

The brachioradialis tendon was transferred to the extensor carpi radialis longus and brevis tendons to restore active extension of the wrist in nine patients who had traumatic tetraplegia. The classification of neurological function was the fifth cervical level for all patients. The average time from the injury to the operation was six years (range, one to twenty years), and the average duration of follow-up was ten years (range, two to fifteen years). The evaluation of the patient included a determination of the preoperative and postoperative ranges of motion of the wrist, manual muscle-testi

Wound coverage after a complete degloving injury of the hand and fingers is 1 of the most difficult problems in hand surgery. Important structures such as tendons, nerves, and bones are exposed and will necrose if not covered adequately. The goal of treatment should be coverage with a pliable, sensitive, and cosmetically similar tissue that will allow early mobilization. The authors' experience has led them to adopt the following general guidelines: replantation of the avulsed structures whenever possible; early transfer of a free composite flap from the foot for a complete and irreparable deg
