Dynamic posturography for staging of patients with Ménière's disease.

OBJECTIVE: In recent years, techniques such as computerised dynamic posturography have facilitated objective evaluation of patients with balance disorders. In this study, we used computerised dynamic posturography for clinical staging of Ménière's disease. MATERIAL AND METHODS: We applied a computerised dynamic posturography sensory organisation test to 53 patients with American Academy of Otolaryngology criteria of Ménière's disease. The following parameters were considered in data analysis: computerised dynamic posturography scores for testing the patient's centre of gravity with a mobile s

Adaptation of the angular vestibulo-ocular reflex to head movements in rotating frames of reference.

Head movements in a rotating frame of reference are commonly encountered, but their long term effects on the angular vestibulo-ocular reflex (aVOR) are not well understood. To study this, monkeys were oscillated about a naso-occipital (roll) axis for several hours while rotating about a spatial vertical axis (roll-while-rotating, RWR). This induced oscillations in roll and pitch eye velocity and continuous horizontal (yaw) nystagmus. For several hours thereafter, simple roll in darkness induced horizontal nystagmus and pitch and roll oscillations. The rising and falling time constants of the h

Dorsolateral medullary infarction: a neurogenic cause of a contralateral, large-amplitude vestibular evoked myogenic potential.

The vestibular evoked myogenic potential (VEMP) has become a useful tool to assess the saccule and inferior vestibular nerve function. Vestibulopathies involving the saccule or inferior vestibular nerve typically result in VEMP responses that are diminished or absent on the involved side. Abnormally large VEMPs are rare. Large VEMPs have been associated with superior canal dehiscence, Ménière's disease, and labyrinthine fistula. In all of these cases, the abnormally large VEMP can be explained on the basis of labyrinthine hydromechanical changes that result in excessive saccular displacement i

Vestibular inputs elicit patterned changes in limb blood flow in conscious cats.

Previous experiments have demonstrated that the vestibular system contributes to regulating sympathetic nervous system activity, particularly the discharges of vasoconstrictor fibres. In the present study, we examined the physiological significance of vestibulosympathetic responses by comparing blood flow and vascular resistance in the forelimb and hindlimb during head-up tilt from the prone position before and after the removal of vestibular inputs through a bilateral vestibular neurectomy. Experiments were performed on conscious cats that were trained to remain sedentary on a tilt table duri

Asymmetrical perception of body rotation after unilateral injury to human vestibular cortex.

Vestibular information plays a key role in many perceptual and cognitive functions, but surprisingly little is known about how vestibular signals are processed at the cortical level in humans. To address this issue, we tested the ability of two patients, with damage to key components of the vestibular network in either the left or right hemisphere, to perceive passive whole-body rotations (25-125 degrees) about the yaw axis. In both patients, the posterior insula, hippocampus, putamen, and thalamus were extensively damaged. The patients' responses were compared with those of nine age- and sex-

Can E-selectin be a reliable marker of inflammation in lumbar disc disease?

The cause of sciatica and low back pain associating with lumbar disc herniation has not been clearly identified until now. Inflammation has been shown to occur via immunohistochemical and biochemical methods in herniated disc tissues. The important prognostic role of E-selectin has recently been substantiated by other studies in early rheumatoid arthritis (RA) and juvenile rheumatoid arthritis (JRA). The important role of adhesion molecules in the initiation and progression of the inflammatory response is well known for infectious diseases and autoimmune disorders. In our study, we aimed to sh

Acute migrainous vertigo: clinical and oculographic findings.

Migrainous vertigo (MV) is an increasingly recognized cause of episodic vertigo. However, the pathophysiology of MV is still a matter of speculation and it is not known to what extent the dysfunction is located in the central or peripheral vestibular system. The aim of this prospective study was to describe the clinical spectrum of acute MV and to clarify which structures of the vestibular system are involved. Testing of 20 patients with acute MV included neuro-otological examination, recording of spontaneous and positional nystagmus with 3D video-oculography, and audiometry. Pathological nyst

Vestibular evoked myogenic potentials versus vestibular test battery in patients with Meniere's disease.

OBJECTIVE: The present study was undertaken to assess the sensitivity of vestibular evoked myogenic potentials testing to side-of-disease in Meniere's disease patients and to test the hypothesis that information supplied by vestibular evoked myogenic potentials is complementary to that provided by a conventional vestibular test battery. STUDY DESIGN: Prospective cohort study. SETTING: Large specialty hospital, department of otolaryngology. SUBJECTS: Twenty consenting adults (9 men and 11 women) with unilateral Meniere's disease by American Academy of Otolaryngology-Head and Neck Surgery dia

Surgery of the inner ear with hearing preservation: serial histological changes.

OBJECTIVES/HYPOTHESIS: Surgery of the inner ear can result in hearing preservation under certain conditions, but the mechanisms responsible for hearing preservation or loss are not well understood. The specific aim of the study is to examine histological sections obtained at different time intervals after varying degrees of surgical entry into the inner ear, to understand how the cochlea is protected. The hypothesis is that internal partitioning occurs. STUDY DESIGN: Histologic examination of guinea pig inner ears by light microscopy. METHODS: Guinea pigs underwent lateral semicircular canal

Compensatory changes in static and dynamic subjective visual vertical in patients following vestibular schwanoma surgery.

OBJECTIVE: Among patients with vestibular schwanoma (VS), vestibular function is nonhomogeneous, both before and after surgical removal of the VS. This paper reports investigations of neural changes, especially changes in the contribution of visual input to vestibular system integration, after VS surgery. METHODS: We examined 33 patients who underwent VS surgery via a middle fossa approach. Static and dynamic subjective visual vertical (SVV) was measured once after surgery and compared to those measured in control subjects. SVVs were assessed using a paradigm requiring the subject to manually

Long-term changes in hippocampal n-methyl-D-aspartate receptor subunits following unilateral vestibular damage in rat.

Previous studies have indicated that damage to the peripheral vestibular system results in dysfunction of hippocampal place cells and an impairment of spatial learning and memory. The aim of this study was to determine whether lesions of one vestibular labyrinth (unilateral vestibular deafferentation, UVD) result in changes in the expression of the NR1 and NR2A subunits of the N-methyl-D-aspartate (NMDA) receptor, and the GluR2 subunit of the alpha-amino-3-hydroxy-5-methyl-4-isoxazole propionate receptor, in subregions of the rat hippocampus (CA1, CA2/3 and the dentate gyrus) at 10 h or 2 week

The effects of noise on the vestibular system.

PURPOSE: Subjects with noise-induced hearing loss sometimes also complain about balance disorders, but reports of clinical series that give contradictory results are highly controversial. This study was designed to evaluate the effects of intense noise on the vestibular labyrinth, both in subjects with symmetrical hearing loss and in subjects with asymmetrical loss, and to examine the correlation between the subjects' complaints and the results of the vestibular function tests. METHODS: A total of 258 male military personnel, heavily exposed to various intense noises, were included in the stu

Effect of conductive hearing loss on the vestibulo-collic reflex.

The vestibulo-collic reflex represents a promising test for evaluating the integrity of otolith function. We have investigated the threshold of this response in a group of normal subjects, and the effect of a conductive hearing loss. A positive response was recorded in 31 of 32 normal subjects. The threshold of the vestibulo-collic reflex varied from 80 to 97 dBHL in these subjects with a 95% response rate at a threshold at 96 dBHL. A total of 23 ears with a conductive hearing loss in 17 patients were also investigated. The average conductive hearing loss (at 0.5, 1, 2 and 4 kHz) ranged from 8

Surgical anatomy of the singular nerve.

Rare cases of chronic persistent positional vertigo do not respond to physiotherapy. For the treatment of these cases Gacek suggested singular nerve neurectomy as a new surgical procedure. The aim of the present study was to investigate the surgical anatomy of the singular nerve in order to evaluate the exact topography. In 25 cadaver temporal bones the posterior ampullary ("singular") nerve was prepared. The topographical correlation between singular nerve, posterior semicircular canal, and the round window membrane was evaluated. The average length of the nerve was 4.2 mm, its diameter 0.6 m

Initial symptoms and retrospective evaluation of prognosis in Menière's disease.

Clinical studies on an initial symptom and a long-term course of vertigo and hearing impairment and retrospective evaluation of the prognosis were performed in Menière's disease. One hundred and fifty-one patients (67 males and 84 females) with Meniere's disease were treated in the Neuro-otological clinic, Kitasato University Hospital from 1990 to 1995. Ages ranged from 17 to 77 years (mean 47.3 years) at the onset of the disease when the first vertigo attack occurred. There were 106 (70.1%) in their 30s, 40s and 50s, and 28 (18.5%) aged 60 years or over. Seventy-eight patients visited the cli

Infralabyrinthine approach to vestibular neurectomy in Menière's disease.

The infralabyrinthine approach to vestibular neurectomy was performed in 9 patients with unilateral Menière's disease. According to the AAOO (1972) criteria, 7 of 9 cases were graded as class B and the remaining 2 cases as class C. Otherwise, according to the AAO-HNS (1985) criteria, 6 patients who could be followed over 2 years were all graded as 'complete' at the vertigo control. The compensation of the spontaneous vestibular signs was rapid in the first 2 postoperative weeks, though an occasional imbalance on movement persisted even 3 years after the operation. No specific caloric reaction

The visual vertigo syndrome.

Neuro-otological and posturography findings in 15 patients with visually induced vertiginous symptoms (visual vertigo, VV) are reported. Most patients were considered to have peripheral vestibular disorders but one had cerebellar degeneration and another a brainstem stroke. Five patients showed abnormally large body sway induced by full-field visual motion stimulation; including the 2 patients with CNS disease. Four of these 5 patients had additional strabismic symptoms (diplopia, squint surgery and ocular muscle weakness). VV may occur if patients with balance disorders show high visual field

Neuroma of the facial nerve masked by chronic otitis media.

The absence of facial twitching, weakness, or palsy makes the diagnosis of facial neuroma difficult. The authors report a case of neuroma of the horizontal portion of the facial nerve masked by the presence of a chronic ear. A woman with a long history of discharge and hypoacousia in her left ear presented with acute dizziness. Examination revealed grade 3 horizontal right nystagmus, left anacousia, and the appearance of an epitympanic cholesteatoma. Computed tomography (CT) was performed after the vestibular condition improved. The clinical diagnosis of chronic otitis media with cholesteatoma
