The association between diabetes mellitus and the risk of anal cancer: Results from a large nationwide cohort study in Taiwan.

BACKGROUND: Few studies have investigated the association between diabetes mellitus and risk of anal cancer and the results to date have been inconsistent. We analysed the relation between a history of diabetes and diabetic retinopathy and risk of anal cancer in a nationwide cohort in Taiwan. METHODS: Data from a retrospective cohort study of 5.9 million Taiwanese men and women aged 18-90 years were used for the analysis. Multivariable proportional hazards regression models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for the association between diabetes diagno

Impact of bacterial sexually transmitted infections on human papillomavirus and anal dysplasia in people living with HIV.

Anal carcinoma is a relevant malignancy with increased incidence among people living with HIV (PLHIV), particularly in men who have sex with men (MSM) and transgender women (TW). Its main precursor lesion, high-grade anal intraepithelial neoplasia (HGAIN), is primarily associated with persistent infection with human papillomavirus (HPV). However, bacterial sexually transmitted infections (STIs) have been proposed as potential cofactors in anal carcinogenesis. The objectives of this study are to evaluate the relationship between asymptomatic bacterial anal STIs, the presence of HGAIN, and the d

Evaluating the Performance of Anal Cytology and Human Papillomavirus Testing for Detection of Anal Cancer and Pre-Cancerous Lesions.

BACKGROUND: Anal squamous cell carcinoma (SCC) incidence is rising; however, anal cancer screening guidelines require further refinement. This study evaluated the accuracy of anal cytology and human papilloma virus (HPV) testing for anal cancer screening in our pathology service, identifying limitations of these tests when applied to this role. METHODS: Pathology records of all 240 patients who had undergone anal or perianal biopsy during 2023 were evaluated. Reports for 1364 smears, 817 biopsy procedures, and 1441 HPV tests conducted on these patients between 2002 and 2024 were reviewed. RE

Phase II Trial of Atezolizumab and Bevacizumab for Treatment of HPV-Positive Unresectable or Metastatic Squamous Cell Carcinoma of the Anal Canal.

PURPOSE: Anti-PD-L1 antibodies are associated with responses in <25% of patients with metastatic human papillomavirus-associated malignancies. VEGF signaling causes immune evasion and immune suppression within the tumor. We evaluated the anti-PD-L1 antibody atezolizumab and anti-VEGF antibody bevacizumab for patients with unresectable, advanced anal cancer. PATIENTS AND METHODS: For this phase II study, participants with previously treated, immunotherapy-naïve anal cancer received atezolizumab (1,200 mg) and bevacizumab (15 mg/kg) intravenously every 21 days. Responses were evaluated every 9

Access to High-Resolution Anoscopy and Colorectal Surgery Support Identified as Important Facilitators to Successful Veterans Affairs Anal Cancer Screening Programs.

BACKGROUND: Anal cancer disproportionately affects people living with HIV. The Department of Veterans Affairs is the largest single provider of health care to people living with HIV in the United States and recommends all veterans living with HIV be screened for anal cancer annually. There are barriers to developing successful anal cancer screening programs, and screenings within Veterans Affairs have been underused. OBJECTIVE: This study aims to identify facilitators to anal cancer screening programs in Veterans Affairs. DESIGN: This is a qualitative study involving semistructured virtual i

Analysis of >15 000 Solid Organ Transplant Recipients Reveals Nonanal Genitourinary HPV-related Disease as Highest Risk Predictor for Anal Squamous Intraepithelial Lesions/Anal Cancer.

BACKGROUND: Solid organ transplantation is a risk predictor for virally-mediated anal squamous intraepithelial lesions and cancer (anal disease). Precancerous squamous intraepithelial lesions can be detected by screening, and treatment may prevent cancer progression. Screening recommendations are not well defined. We aim to define prevalence and describe risk predictors for anal disease in a large population of solid organ transplant recipients. METHODS: Retrospective single-center cohort analysis included solid organ transplant recipients cared for between 2001 and 2022 (N = 15 362). The coh

Males Have Lower Anal Pap Smear Screening in a Miami Safety-Net HIV Clinic.

BACKGROUND: Although people with HIV have a markedly higher risk of anal squamous cell carcinoma (ASCC), there are few evaluations of anal Pap screening determinants within safety-net HIV clinics. METHOD: We conducted an evaluation of anal Pap screening correlates within a safety-net HIV clinic in Miami. Medical records were reviewed for 298 people ages 45 and older receiving HIV primary care. Demographic information and the prevalence of anal Pap screening over 1 year (i.e., 2018-2019) were extracted. RESULTS: Between 2018 and 2019, approximately half (46%) of patients completed anal Pap sc

Clinical Predictors and Outcomes of Invasive Anal Cancer for People With HIV in an Inception Cohort.

BACKGROUND: Due to the heterogeneity of risk for invasive anal cancer (IAC) among people with human immunodeficiency virus (PWH), we investigated predictors of IAC and described outcomes among those with a cancer diagnosis. METHODS: Using a longitudinal inception cohort of anal cancer screening, we evaluated risk factors and outcome probabilities for incident IAC in Cox models. Screening included anal cytology and digital anorectal examination, and, if results of either were abnormal, high-resolution anoscopy. RESULTS: Between 30 November 2006 and 3 March 2021, a total of 8139 PWH received c

Does use of anal cytology as a triage test improve the performance of high-risk human papillomavirus screening in gay and bisexual men for anal cancer prevention?

Anal high-risk human papillomavirus (HRHPV) testing-based anal cancer screening gay and bisexual men (GBM) is associated with high sensitivity, but low specificity. We report the potential role of triage use of anal cytology with HRHPV testing in detecting 12-month persistent anal high-grade squamous epithelial lesions (HSIL) in a cohort of GBM in Sydney, Australia. Participants were GBM from the Study of the Prevention of Anal Cancer (SPANC) who underwent annual anal HPV testing, cytology, and high-resolution anoscopy (HRA)-guided histology. The sensitivity and specificity of five screening a

Improving the screening of precancerous anal lesions in high-risk subjects with normal cytology: A longitudinal cohort study using simple tests.

UNLABELLED: For patients at high risk of anal cancer, annual screening strategies using invasive evaluation methods are stressful. According to a normal examination at baseline using simple and non invasive tests, the aim of the work was to quantify neoplastic events. PATIENTS AND METHOD: Data from patients with a normal evaluation at the first visit were retrospectively extracted from a prospective database. The individual follow-up period was at least two years and three evaluations. Patients with abnormal cytology were assessed using high-resolution anoscopy and targeted biopsies. RESULTS

5-Year Prospective Evaluation of Cytology, Human Papillomavirus Testing, and Biomarkers for Detection of Anal Precancer in Human Immunodeficiency Virus-Positive Men Who Have Sex With Men.

BACKGROUND: Human papillomavirus (HPV)-related biomarkers have shown good cross-sectional performance for anal precancer detection in human immunodeficiency virus-positive (HIV+) men who have sex with men (MSM). However, the long-term performance and risk stratification of these biomarkers are unknown. Here, we prospectively evaluated high-risk (HR) HPV DNA, HPV16/18 genotyping, HPV E6/E7 messenger RNA (mRNA), and p16/Ki-67 dual stain in a population of HIV+ MSM. METHODS: We enrolled 363 HIV+ MSM between 2009-2010, with passive follow-up through 2015. All had anal cytology and a high-resoluti

Warts and all: Fingolimod and unusual HPV-associated lesions.

BACKGROUND: Fingolimod is used to reduce relapse rates in relapsing-remitting multiple sclerosis (MS). It is a sphingosine 1-phosphate (S1P) analogue having antagonistic effects on S1P receptors. Its immunosuppressive effect is due to reduced circulating lymphocyte numbers, and it may also be associated with impaired intrinsic cancer surveillance. Fingolimod side effects include increased rates and severity of viral infections particularly varicella zoster. METHODS: We present five cases of chronic and treatment refractory warts associated with fingolimod therapy. RESULTS: Each of the five c

Beliefs About Anal Cancer among HIV-Infected Women: Barriers and Motivators to Participation in Research.

BACKGROUND: Infection with the human immunodeficiency virus (HIV) remains associated with a greater risk of anal cancer, despite widespread use of combination antiretroviral therapy. Evidence concerning the acceptability of anal cancer screening gives little attention to women. Because HIV-infected women have a high prevalence of depression and history of sexual trauma, understanding acceptability among this group is critical. PURPOSE: We sought to assess barriers and motivators to participation in anal cancer screening research among a racial/ethnically diverse HIV-infected female population
