Multiple determinants, common vulnerabilities, and creative responses: addressing the AIDS pandemic in diverse populations globally.

The AIDS epidemic has been fueled by global inequities. Ranging from sexual inequality and underdevelopment to homophobia impeding health care access for men who have sex with men, imbalanced resource allocations, and social biases have potentiated the spread of the epidemic. However, recognition of culturally specific aspects of each microepidemic has yielded development of community-based organizations, which have resulted in locally effective responses to AIDS. This effective approach to HIV prevention, care, and treatment is illustrated through examples of community-based responses in Hait

Radiographic features of paediatric pneumocystis pneumonia -- a historical perspective.

AIM: To determine differences between the plain radiographic features of paediatric pneumocystis pneumonia (PCP) recorded before the emergence of human immunodeficiency virus (HIV) in 1982 and those documented in the HIV era. To establish differences in the radiographic features of PCP documented in HIV-infected children in developed and developing countries. METHOD: A Medline search of articles was conducted from 1950 to 2006, using the terms "pneumocystis pneumonia in children" and "chest radiographic features" or "bilateral opacification" or "lobar consolidation" or "asymmetrical opacifica…

The polio eradication effort has been a great success--let's finish it and replace it with something even better.

The polio eradication campaign has greatly reduced the effects of this disease, but many new challenges have emerged. These challenges include the occurrence of polio outbreaks caused by wild-type polioviruses or circulating vaccine-derived polioviruses (cVDPVs) in areas where vaccination coverage is low, the existence of people who excrete poliovirus persistently, and the inability to know definitely that poliovirus has gone. As a result, there is uncertainty about if, when, and how we can end polio immunisation. In this article, we discuss several scenarios for the future of polio control. B

A proposal to declare neurocysticercosis an international reportable disease.

Neurocysticercosis is an infection of the nervous system caused by Taenia solium. It is the most important human parasitic neurological disease and a common cause of epilepsy in Africa, Asia, and Latin America, representing enormous costs for anticonvulsants, medical resources and lost production. Neurocysticercosis is a human-to-human infection, acquired by the faecal-enteric route from carriers of intestinal T. solium, most often in areas with deficient sanitation. Intestinal tapeworms cause few symptoms, but adult taeniae carried by humans release large numbers of infective eggs and are ext

International trends in rates of hypospadias and cryptorchidism.

Researchers from seven European nations and the United States have published reports of increasing rates of hypospadias during the 1960s, 1970s, and 1980s. Reports of increasing rates of cryptorchidism have come primarily from England. In recent years, these reports have become one focus of the debate over endocrine disruption. This study examines more recent data from a larger number of countries participating in the International Clearinghouse for Birth Defects Monitoring Systems (ICBDMS) to address the questions of whether such increases are worldwide and continuing and whether there are ge

How the world manages acute burns: Insights from an international survey of 106 burn units across 50 countries.

BACKGROUND: There is considerable variability in the management of acute burns globally, including differences between low- and middle-income countries (LMICs) and high-income countries (HICs). This study aimed to describe global surgical burn care practices through an international survey, providing insight into diagnostic, debridement, and coverage techniques and their timing. METHODS: An online survey was developed and distributed to burn surgeons worldwide, focusing on the management of four patient cases and standard practices. The survey was developed and tested in collaboration with bu

Teleintervention's effects on breastfeeding in low-income women in high income countries: a systematic review and meta-analysis.

BACKGROUND: Many mothers in high-income countries (HIC) do not breastfeed to the World Health Organisation's recommendation of two years. This is particularly true for low-income women (LIW). They often face additional socio-structural barriers that encourage early discontinuation and are inadequately supported by current healthcare interventions. Teleinterventions are flexible and widely used following the global pandemic and increase maternal autonomy over intervention delivery. They show promise in improving other maternal conditions in LIW, including postpartum depression. Teleintervention

Population assessment of health system performance in 16 countries.

OBJECTIVE: To demonstrate how the new internationally comparable instrument, the People's Voice Survey, can be used to contribute the perspective of the population in assessing health system performance in countries of all levels of income. METHODS: We surveyed representative samples of populations in 16 low-, middle- and high-income countries on health-care utilization, experience and confidence during 2022-2023. We summarized and visualized data corresponding to the key domains of the World Health Organization universal health coverage framework for health system performance assessment. We

Trends in contraceptive use.

BACKGROUND: In the past 50-60 years, desired and actual family size have decreased and contraceptive use has increased, especially in developing countries. Unmet need for modern contraceptives and unintended pregnancy levels remain significant. STUDY DESIGN: Data compiled by the United Nations and information from national surveys of women of reproductive age are used to examine trends. RESULTS: Oral contraceptives and condoms have been predominant methods in developed countries; sterilization has been the most common in Latin America and the Caribbean and Asia; hormonal methods, in Africa. …

Interventions to reduce harm associated with adolescent substance use.

A major proportion of the disease burden and deaths for young people in developed nations is attributable to misuse of alcohol and illicit drugs. Patterns of substance use established in adolescence are quite stable and predict chronic patterns of use, mortality, and morbidity later in life. We integrated findings of systematic reviews to summarise evidence for interventions aimed at prevention and reduction of harms related to adolescent substance use. Evidence of efficacy was available for developmental prevention interventions that aim to prevent onset of harmful patterns in settings such a

ECCO consensus on management of inflammatory bowel disease in low- and middle-income countries.

BACKGROUND: The incidence and prevalence of inflammatory bowel disease [IBD] have increased significantly in low- and middle-income countries [LMICs] in recent decades. Managing IBD in these settings presents substantial challenges. This consensus aims to describe the epidemiology of IBD in LMICs and to highlight the key challenges in its diagnosis and treatment. METHOD: The consensus-defining strategy followed the previous European Crohn's and Colitis Organisation [ECCO] consensus guidelines [available at www.ecco-ibd.eu]. The authors reviewed the available evidence and formulated statements

A Pragmatic Call to Integrate and Regulate Non-State Actors in Fragile Health Systems.

In fragile states, healthcare delivery relies on non-state actors like NGOs and private providers. Short-term donor funding has created fragmented services and weak state capacity. However, donor-driven fragmentation is not only a technical coordination problem; it is also a political economy problem shaped by earmarking, attribution pressures, fiduciary risk aversion, donor-controlled contracts, and upward accountability to funders. We propose shifting from state-as-provider to state-as-steward, not as a choice between two mutually exclusive roles, but as a move toward pluralistic health-syst

Governing AI for Pharmacovigilance in Low-Income Countries: Systems Perspective.

Gaps in pharmaceutical governance could widen with the adoption of AI, even as AI promises better pharmacovigilance in low-income countries (LICs). While advanced regulatory systems like Australia's are integrating AI into pharmaceutical governance, LICs with underdeveloped regulatory capabilities, such as South Sudan, lag behind. The potential divergence disorients the World Health Organization's "Medicine Without Harm" agenda and effective global pharmacovigilance. Moreover, evolving global governance initiatives, including the newly established United Nations scientific panel on AI, may be

Low-cost Rehabilitation Solutions: Lessons From LMIC Settings.

PURPOSE OF REVIEW: Rehabilitation in LMICs is more important than in HICs due to the lack of access to acute stroke interventions (thrombolysis, endovascular thrombectomy, and stroke unit care) in most treated patients which leads to worse long-term outcomes, and thereby greater disability and functional impairment, therefore requiring specialized rehabilitation. RECENT FINDINGS: The are barriers to rehabilitation in LMICs. These can be considered under healthcare- and patient-related factors. Healthcare factors include limited skilled rehabilitation specialists, negligible stroke unit care,

Prevalence of Hospital-Acquired Infections in Low- and Middle-Income Countries: Systematic Review and Meta-Analysis.

The burden of hospital-acquired infections (HAIs) is particularly substantial in low- and middle-income countries (LMICs). However, a comprehensive understanding of their prevalence in these regions is lacking. This systematic review aimed to evaluate the prevalence of HAIs in LMICs. Major databases, including Scopus, PubMed, and Web of Science, were thoroughly searched for published studies between January 1, 2000, and July 15, 2024. The PRISMA guidelines were followed. The pooled prevalence of HAIs was estimated using a random-effects model and a 95% confidence interval. The literature searc

Water stress evolution and redistribution pathways under global inequality in consumption.

Global water stress is intensifying alongside the expansion of goods and services consumption. Based on consumption-based water footprint (CWF) accounting for 188 countries from 1990 to 2020, this study reveals a persistent strong coupling between water consumption and economic growth. Global income inequality further exacerbates structural imbalances in water consumption and reshapes the spatial distribution of water stress through trade networks: while high-income countries reduce local stress, low-income countries are increasingly trapped in the dual challenges of water depletion and persis
