Anatomical glenoid reconstruction via a J-bone graft for recurrent posttraumatic anterior shoulder dislocation.

OBJECTIVE: To reconstruct the anatomical glenoid shape in cases of osseous glenoid rim defects after recurrent posttraumatic anterior should … fusion at the iliac crest. SURGICAL TECHNIQUE: The subscapularis tendon and capsule are split. The humeral head is retracted laterally, and the glenoid defect is prepared and abraded with a rasp. A bicortical iliac crest bone block including crest and outer cortex is harvested and molded in a J-shaped manner. To incorporate the graft, a crevice on the glenoid …

Use of locking plates in the treatment of proximal humerus fractures.

BACKGROUND/HYPOTHESIS: Open reduction and internal fixation (ORIF) using locked plating has demonstrated promise in the treatment of displaced proximal humerus fractures. The purpose of this article is to describe the surgical technique and to report early clinical results with this technique. METHODS: Important surgical principles to follow include adequate use of locking screws in the humeral head, bone graft or bone graft substitutes when needed, rotator cuff sutures to assist with reduction and augment fixation, and sufficient use of intraoperative fluoroscopic imaging. A review was perfo…

Arterial compression during overhead throwing: a risk for arterial injury?

Case studies reporting aneurysm formation in the axillary artery have been described in overhead throwing athletes, possibly due to repetitive arterial compression by the humeral head that has been transiently observed during sonographic diagnostic arm manoeuvres. Whether compression negatively alters arterial health has not been investigated and was the focus of this study. The throwing arm of elite overhead athletes was screened for inducible axillary artery compression. Compressors (COMP, n = 11, mean age: 20 (SD: 2) year, 7 male, 4 female) were age and sex matched with noncompressing (NONC…

The role of transforming growth factor beta isoforms in tendon-to-bone healing.

The purpose of this study was to examine the role of two of the three transforming growth factor beta (TGF-β) isoforms at the healing tendon-to-bone insertion. The supraspinatus tendons of 64 rats were transected at their bony insertions and repaired to the humeral head. One shoulder of each rat received an osmotic pump for sustained delivery of the following factors at the repair site: (1) TGF-β1 and neutralizing antibodies to TGF-β2 and 3 (TGF-β1 group), (2) TGF-β3 and neutralizing antibodies to TGF-β1 and 2 (TGF-β3 group), (3) neutralizing antibodies to TGF-β1, 2, and 3 (anti-TGF-β group), …

Arthroscopic rotator interval closure in shoulder instability repair: a retrospective study.

BACKGROUND: Arthroscopic Bankart repair (ABR) is a standard treatment for recurrent anterior shoulder instability. Young age, hyperlaxity, loss of bone stock and multidirectional or voluntary type of instability are all associated with failure of this procedure. Rotator interval laxity is associated with shoulder instability, whereas rotator interval closure increases humeral head stability and reduces shoulder range of motion. METHODS: The records of patients with recurrent anterior shoulder dislocations who underwent ABR with or without arthroscopic rotator interval closure (ARIC) in our de…

Concomitant Variations of the Scapular Region Musculature With an Aberrant Neurovascular Pattern.

The shoulder complex exhibits a distinctive amalgamation of muscular power, integration, and an extensive range of motion. The shoulder joint allows for a wide range of upper limb movements because the four small muscles that make up the musculotendinous rotator cuff work together to keep the joint stable. The four muscles that come from the scapula and form the rotator cuff are the subscapularis, supraspinatus, infraspinatus, and teres minor. Accurate diagnosis and proper management of shoulder-related injuries are essential, particularly for sportspersons and individuals engaged in heavy wei

Use of Gracile and semi-tendinosus tendons (GRAST) for the reconstruction of irreparable rotator cuff tears.

BACKGROUND: Irreparable rotator cuff tears are common and difficult to treat. Techniques for "filling the loss of substance" require fixatio … only fill a tendinous defect. The transplant could have a subacromial "spacer" effect and lower the humeral head. The donor site morbidity and the fate of the transplant in-vivo are two limits to be discussed. This anatomical study paves the way for clinical experimentation.

How to Measure Glenoid Bone Stock and Version and Why It Is Important: A Practical Guide.

Shoulder osteoarthritis (OA) is a common and debilitating condition and a source of high morbidity and medical expenditures across the world among individuals older than 60 years. Shoulder OA results in the gradual destruction of articular cartilage of the humeral head and glenoid component, causing inflammation, pain, and a restricted range of motion. Most patients are diagnosed with shoulder OA after experiencing shoulder pain and stiffness, and the diagnosis is often made after medical and physical histories are obtained and physical and imaging examinations are performed. Use of various su…

Anterior instability in the throwing shoulder.

The disabled throwing shoulder is a multifactorial problem. Laxity of the glenohumeral joint is necessary to achieve a satisfactory velocity … surgical repair without a shift, utilizing mattress sutures to prevent suture chondromalacia on the humeral head or glenoid. The anterior laxity management centers on the posterior superior labrum, although occasionally the anterior labrum or capsule may be involved as well. Overall, symptomatic anterior instability is less common in the throwing shoulder. Job …

Coverage of axillary lymph nodes with high tangential fields in breast radiotherapy.

The aim of this study is to evaluate the coverage of axillary nodal volumes with high tangent fields (HTF) in breast radiotherapy and to determine the utility of customised blocking. The treatment plans of 30 consecutive patients with early breast cancer were evaluated. The prescription dose was 50 Gy to the whole breast. Axillary level I-II lymph node volumes were delineated and the cranial border of the tangential fields was set just below the humeral head to create HTF. Dose-volume histograms (DVH) were used to calculate the doses received by axillary nodal volumes. In a second planning set…
