Patellar complications after knee arthroplasty. A prospective study of 56 cases using the Kinematic prosthesis.

A prospective study on 56 knees was performed to analyze the incidence and cause of patellar complications following total knee arthroplasty with the Kinematic prosthesis inserted with the Universal Instrumentation system. Patellar complications occurred in 23 cases: 15 had anterior knee pain. There was 1 case of patellar fracture. Subluxation of the patella occurred in 13 cases, of which 6 were painful. Although the mechanical alignment was correctly reconstructed to neutral in all but 2 cases, the instrumentation tended to allow malpositioning of the components into excessive femoral valgus

Octogenarians Face Higher 90-day Medical Risk but Lower 1-year Revision Rates After Primary Total Knee Arthroplasty.

BACKGROUND: Prior studies evaluating age-related outcomes after total knee arthroplasty (TKA) have examined medical complications and revision risk separately. We asked whether octogenarians have higher rates of 90-day medical complications, 30- and 90-day readmissions, and all-cause and etiology-specific revision after primary TKA. MATERIALS AND METHODS: PearlDiver all-payer claims (2010 to 2022) identified isolated, first-instance unilateral primary TKA, excluding index revision, fracture/malignancy, concomitant other-joint arthroplasty, staged/simultaneous bilateral TKA within 30 days, and

Total femur replacement, indications for the procedure and its complications: a systematic review.

INTRODUCTION: Total femur replacement (TFR) is a solution that allows orthopedic surgeons to address the most complicated cases in hip, femur, and knee reconstruction. Complete femur prostheses are used in both oncologic patients with femur tumors that require resection and in cases of complicated prosthetic failure with massive bone defects. TFR is an alternative to hip disarticulation that allows the patient to spare the limb and preserve its function. However, the indications for surgery vary and the procedure has been reported to present a high risk of serious complications. MATERIALS AND

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

Timing and Selection of Lower Extremity Arthroplasty Procedures: Which to Perform First and When to Consider Simultaneous Bilateral Procedures.

» For patients with both lumbar spine pathology, hip and knee degenerative joint disease, it is important to consider the implications of surgically addressing each anatomic region first.» Performing total hip arthroplasty before lumbar spine fusion may decrease the risk of dislocation and revision surgery; however, if spinal fusion is performed first, it may be protective to wait 1 to 2 years to lower the risk of complications.» In all patients with concurrent hip and low back symptoms, it is recommended that an evaluation of both areas is performed before proceeding with either surgical inte

Dislocation of the tibialis posterior tendon: diagnosis and treatment.

Traumatic dislocation of the tibialis tendon occurred from minor ankle sprains in a 37-year-old male and a 53-year-old female. Both complained of local pain at the medial malleolus, and both walked with a limp. The diagnosis was suspected by clinical examination, in one case with 2 months' delay, and verified by ultrasound, computed tomography, and magnetic resonance imaging. The male patient was initially treated for an "uncomplicated ankle sprain." For various reasons surgery was delayed 4 months. During this interval the male patient complained of pain and severe dysfunction, requiring anal

Reconstruction of the medial patellofemoral ligament and reinforcement of the medial patellotibial ligament is an effective treatment for patellofemoral instability with patella alta.

PURPOSE: To evaluate the clinical outcome of the combined reconstruction of the medial patellofemoral ligament (MPFL) and medial patellotibial ligament (MPTL) for patellar instability with patella alta. METHODS: A total of 108 patients underwent a combined reconstruction of the MPFL and MPTL, and 58 patients were included in this study. The clinical results were evaluated and compared using the International Knee Documentation Committee (IKDC) scores, Kujala scores, and visual analogue scale (VAS) scores. The tibial tuberosity-trochlear groove (TT-TG) distance, three indices of patellar heigh

Operative Versus Nonoperative Treatment of Medial Patellofemoral Ligament Injuries: A Systematic Review and Meta-analysis of Randomized Controlled Trials With Trial Sequential Analysis.

BACKGROUND: The medial patellofemoral ligament (MPFL) serves as a major stabilizer in the knee, providing restraint against lateral patellar translation. The optimal approach for treating patients with first-time MPFL injuries remains uncertain, as the choice between operative and nonoperative treatment depends on the risks and variability associated with individual patient anatomy, activity, and presentation. PURPOSE: To evaluate high-level literature on outcomes after operative and nonoperative treatment of MPFL injuries to guide decision-making. STUDY DESIGN: Meta-analysis; Level of evide

Unpacking the Tibial Tubercle-Trochlear Groove Distance: Evaluation of Rotational Factors, Trochlear Groove and Tibial Tubercle Position, and Role of Trochlear Dysplasia.

BACKGROUND: The tibial tubercle to trochlear groove (TT-TG) distance is often utilized for determining the surgical treatment for patients with patellar instability (PI). It is thought to directly represent the position of the TT on the tibia. Recent work has shown that the measurement of the TT-TG distance is multifactorial. PURPOSE: To investigate the relationship between relative tibial external rotation (rTER) and trochlear dysplasia (TD), as well as the location of the TG and TT in patients with and without PI, and to correlate these and other anatomic measurements with the TT-TG distanc

Fibrous tether of the distal femur produced unilateral genu valgus with subsequent patellar dislocation: case report and review of the literature.

Fibrous tether at the distal femur is a rare condition that causes angular deformity of the lower extremity; only 18 cases have been reported previously. We report the case of a 2-year-old boy presenting with unilateral genu valgus. The patient had a subsequent lateral patellar dislocation, 1 year after surgical correction. A second surgery was performed to correct this patellar dislocation. Intraoperative finding indicated contracted lateral patellar retinaculum. To the best of our knowledge, this is the first report of such a finding. The natural history is still not understood. At the 6-mon

The relationship between quadriceps angle and tibial tuberosity-trochlear groove distance in patients with patellar instability.

PURPOSE: The quadriceps angle (Q-angle) represents the angle between the vector of action of the quadriceps and the patellar tendon. An increased Q-angle has been associated with an increased risk of patellar instability, although there is disagreement on its reliability and validity as it is affected by the position of the limb and contraction of the quadriceps. Tibial tuberosity-trochlear groove distance (TT-TG) is ascertained by axial CT scanning, with an increased value associated with patellar instability. This study aimed to determine whether the Q-angle correlates with the TT-TG distanc

Reconstruction of the medial patellofemoral ligament for patellar instability using an autologous gracilis tendon graft.

Patellofemoral dislocations are common. In cases with recurrence or residual instability, surgical intervention is usually considered. Numerous treatment protocols have been used in the past to treat patellofemoral instability secondary to patella dislocation. Reconstruction of the medial patellofemoral ligament is one of the possible options, since it was acknowledged to have a major medial stabilising role on the patella. We present a technique for reconstruction of the medial patellofemoral ligament using an autologous gracilis tendon graft.

Intra-articular dislocation of the patella with incomplete rotation--two case reports and a review of the literature.

Intra-articular dislocation of the patella is a rare clinical condition. Only forty-five cases have been reported. Most of them were encountered in adolescents and required an open reduction. We report two cases in an older age group with an incomplete rotation of the patella, which were successfully treated by closed reduction. We discuss the literature and suggest that the management depends on the degree of patellar rotation.

Recurrent atraumatic superior dislocation of the patella: case report and review of the literature.

Recurrent atraumatic superior dislocation of the patella with spontaneous reduction prevented by interlocking osteophytes has not been previously reported. Eight previous case reports were noted in the literature with single episodes of interlocking by osteophytes in a superior dislocation, none with recurrence, and all treated by closed reduction. The presentation of a 60-year-old woman with recurrent atraumatic superior dislocation of the patella on three separate occasions required closed reduction due to interlocking patella and trochlear osteophytes that were preventing spontaneous reduct

Medial subluxation of the patella as a complication of lateral retinacular release.

We examined 54 patients (60 knees) referred to us because of their failure to improve, or because of a worsening of their preoperative symptoms, following an arthroscopic lateral retinacular release. Thirty knees developed medial subluxation of the patella postoperatively. This disabling condition is new to us. It is previously unreported as a complication of arthroscopic lateral retinacular release. Anterior knee pain was the only reported preoperative symptom in 14 knees. Sixteen knees had a preoperative diagnosis of lateral patellar subluxation on the basis of a positive apprehension sign o
