[Tumor-related radiographic changes of the vertebral column in childhood (author's transl)].

Benign tumors and malignant primary or metastatic neoplasms of the vertebral column are rare in childhood. Radiographic changes are sometimes detected by routine investigations since no clinical symptoms are present. The type of tumor can only rarely be specified from the radiographic changes. Usually a specific diagnosis requires histologic studies. If a tumor the vertebral column is suspected detailed radiographic studies are mandatory to secure an early diagnosis. The impending danger of spinal cord compression makes an early diagnosis and rapid surgical intervention even more important. In

Ureteral reconstruction for retroperitoneal tumors in children.

PURPOSE: Removal of solid tumors of the pelvis and abdominal cavity may require resection of an involved ureteral segment. Ureteral stricture can also be a result of intense therapy. We present our experience with urinary reconstruction in this situation. METHODS: A retrospective review of pediatric oncology patients with solid abdominal/pelvic tumors who underwent a ureteral reconstructive procedure was done. Institutional review board wavier was obtained for the review. Patient data were collected on diagnosis, procedures performed, renal function, and follow-up. RESULTS: Thirteen patients

Neuropsychological outcome in long-term survivors of a childhood extracranial solid tumor who have undergone autologous bone marrow transplantation.

The purpose of this study was to evaluate neuropsychological and adaptive functioning of children who have undergone bone marrow transplantation (BMT) without previous cranial irradiation. In total, 76 children treated for an extracranial tumor with BMT without total body irradiation (TBI) were evaluated at least 5 years after the end of the treatment.Overall, their performance and skills were in the normal range and their professional and academic outcomes were satisfactory. Nevertheless, we observed a deleterious effect of deafness on verbal IQ associated with the previous administration of

An exploratory study of environmental and medical factors potentially related to childhood cancer.

To determine whether a general-purpose epidemiologic questionnaire can be used in childhood cancer hospitals to identify associations between environmental factors and the major types of childhood cancer, we report the results of the analysis of the data obtained from such a questionnaire. On admission to St. Jude Children's Research Hospital in Memphis, Tennessee, between 1979 and 1986 a questionnaire was administered to 1,270 mothers of patients diagnosed with childhood cancer. Approximately one-half of the children had acute leukemias (n = 629); the remainder had lymphomas (n = 237) or soli

Computed tomography of sacral and perisacral lesions.

The sacrum and perisacral soft tissues are not readily amenable to investigation by conventional radiological techniques. Computed tomography (CT) has provided an excellent imaging modality for the evaluation of these areas. This article describes the normal CT anatomy of the sacrum, the CT technique for evaluating the sacrum and perisacral soft tissues and the contribution of CT to the assessment of the many disorders that may involve the sacrum. Conditions discussed include osseous tumors of the sacrum, spinal dysraphism, cauda equina tumors and cysts, lesions arising primarily in the presac

Systemic venous blood return via the portal vein.

The patterns of collateral circulation which develop following obstruction of the inferior vena cava have been classically divided into three groups depending upon the level of obstruction, i.e., infrarenal, middle caval, and upper caval. The portal vein can play an active role at all three levels. Multiple potential routes to the portal vein are described and 6 cases are presented.

Pediatric skull base tumors: Clinical features and surgical outcomes; a single center retrospective study with a review of literature.

OBJECTIVES: Pediatric intracranial tumors, particularly at the skull base, are rare and present unique challenges to pediatric neurosurgeons and oncologists, owing to their complex anatomy and diverse histopathology. Robust evidence is still marginal concerning their clinical and surgical courses. Our aim is to describe our experience regarding surgical approaches, with special focus on surgical features, postoperative outcomes, adverse events as well as adjuvant therapeutic concepts. METHODS: Patients aged <18 years undergoing skull base surgery between 2017 and 2023 at our institution were

The impact of lag time to cancer diagnosis and treatment on clinical outcomes prior to the COVID-19 pandemic: a scoping review of systematic reviews and meta-analyses

ABSTRACT Background The COVID-19 pandemic has disrupted cancer care, raising concerns regarding the impact of wait time, or ‘lag time’, on clinical outcomes. We aimed to contextualize pandemic-related lag times by mapping pre-pandemic evidence from systematic reviews and/or meta-analyses on the association between lag time to cancer diagnosis and treatment with mortality- and morbidity-related outcomes. Methods We systematically searched MEDLINE, EMBASE, Web of Science, and Cochrane Library of Systematic Reviews for reviews published prior to the pandemic (1 January 2010-31 December 2019). We

Spheno-orbital Lesions-A Major Review of Nonmeningioma Causes.

PURPOSE: To review the imaging features of the broad range of nonmeningioma lesions of the greater wing of the sphenoid (GWS) bone and adjacent orbit to assist clinicians in differentiating these lesions from each other and from the most common lesion of the GWS and adjacent orbit, meningioma. METHODS: We reviewed 32 cases of spheno-orbital lesions involving the GWS from our own practice, as well as 109 published cases (total 141), with emphasis on available imaging features on computerized tomography (CT) and MRI. Features that might assist in differentiating meningioma from its mimics were

Peripheral lymphocyte count and suppopulations of T and B lymphocytes in benign and malignant diseases.

It is well known that there are many independent and inter-related clinical and pathologic factors which influence the prognosis of patients with benign and malignant conditions. Lymphocyte level is an index of cell-mediated immunity which is important in host defense against cancer. But it is surprising that a simple test such as peripheral lymphocyte count could be correlated with clinical stages and survival results in patients with Hodgkin's disease, non-Hodgkin's lymphoma and non-lymphomatous solid tumors. Regarding the latter, lymphocyte count had prognostic values in patients with cance

Rural-Urban Disparities in Survival Among Patients With Malignant Primary Osseous Neoplasms of the Spine and Sacropelvis: A Population-Based Cohort Study.

Background Malignant primary osseous neoplasms of the spine and sacropelvis are rare tumors whose outcomes rely heavily on specialized, guideline-driven care, which is concentrated in metropolitan referral centers. Investigations of rurality in this population have been limited by single-histology cohorts, small numbers of rural patients, and unadjusted comparisons, and none have weighed geographic disadvantage directly against the socioeconomic conditions with which it coexists. This study aimed to determine whether rural residence is independently associated with overall survival and cause-s

Results of RS-99 protocol for childhood solid tumors.

BACKGROUND: Little was known about the therapeutic result of rhabdomyosarcomas (RMSs) and other malignant tumors until the end of the last century in China. Very few prospective clinical research results have been reported. We designed a RS-99 protocol under close cooperation of a multidisciplinary team including surgeons, radiologists, pathologists, and pediatric oncologists at Shanghai Children's Medical Center. This study aimed to improve the prognosis of childhood solid tumors and analyze the results of different tumors with the same protocol, including RMSs, the Ewing sarcoma family of tu

Chronic osteomyelitis: a continuing orthopaedic challenge in developing countries.

Nine patients with chronic osteomyelitis, three with problems due to diagnosis, three with dilemmas regarding treatment and three with other complications are presented. It is suggested that although the diagnosis of osteomyelitis in most cases is straightforward, presentation might sometimes be similar to other conditions, which can lead to a dilemma in the diagnosis. Because of the formidable complications, which may be difficult to manage and the difficulty in guaranteeing permanent cure, the best approach is prevention by judicious treatment of acute haematogenous osteomyelitis and of open

Resection surgery in the treatment of vertebral tumors.

With the purpose of clarifying the limits of resection surgery (en bloc excision) for the treatment of bone tumors of the spine, the authors report the indications and results of 43 operations. This series was homogeneous in terms of oncological and surgical staging, as well as with regard to surgical method used and anaesthesiology. Three different types of surgery were performed: en bloc resection of tumors of the body (sectors 5-9 or 4-8 depending on the WBB staging system), of the posterior arch (sectors 10-3) or characterized by eccentric growth (sectors 2-5 or 8-11). The surgical margins

Cell type-specific control of human neuronectin secretion by polypeptide mediators and phorbol ester.

Neuronectin (NEC1) is a human extracellular matrix (ECM) protein expressed with a unique rostrocaudal pattern in white matter of the normal adult central nervous system. In addition, NEC1 is expressed in normal fetal and adult smooth muscle, along certain epithelial-mesenchymal junctions, and transiently in developing fetal cartilage. Region-specific induction of NEC1 is found in dermal wounds and in the reactive stroma of actinic keratoses, psoriatic skin lesions, and a range of malignant tumors. One explanation for these diverse tissue patterns is that cells capable of producing NEC1 are wid
