A biomechanical analysis of intrascaphoid compression using the 3.00 mm Synthes cannulated screw and threaded washer: an in vitro cadaveric study.

This study evaluated the compressive capabilities of the 3.0 mm Synthes cannulated screw and threaded washer. A transverse osteotomy was performed at the waist of eight cadaveric scaphoids and a custom-designed load cell was inserted before internal fixion with a 3.0 mm cannulated screw and threaded washer. The mean intrascaphoid compression achieved was 108 (SD, 60) N. This compressive force is comparable to that produced by standard cortical screws.

Scaphotrapeziotrapezoid Joint Osteoarthritis: Differentiating SOAC Wrist From SLAC Wrist.

Scaphotrapeziotrapezoid (STT) osteoarthritis (OA) is the third most common osteoarthritic joint in the wrist. Recent research challenges prior assumptions about the STT OA relationship with scapholunate dissociation (SLAC) and proposes a new progression model leading to scaphotrapeziotrapezoid osteoarthritis advanced collapse (SOAC) wrist. Normal wrist mechanics enable the scaphoid and proximal carpal row to move synchronously for optimal motion. STT arthrodesis may have adverse effects if midcarpal instability is unrecognized. Understanding outcomes from early clinical studies of STT arthrode

Residual flexion deformity after scaphoid nonunion surgery: 7-year follow-up study.

The clinical implication of a residual flexion deformity following surgery for scaphoid nonunion is unclear. Sixty-three patients who underwent scaphoid nonunion surgery were assessed after a mean of 7 years (range 5-10) to analyse the outcomes based on the presence of residual scaphoid deformity. Primary outcome was Disabilities of the Arm, Shoulder and Hand score. Secondary outcomes were Patient-Rated Wrist Evaluation score, wrist range of motion and strength. Patients were dichotomized to residual deformity or no deformity. Scaphoid deformity was calculated from CT scans based on the median

Analysis of ulnar variance as a risk factor for developing scaphoid nonunion.

OBJECTIVE: Ulnar variance may be a risk factor of developing scaphoid non-union. METHODS: A review was made of the posteroanterior wrist radiographs of 95 patients who were diagnosed of scaphoid fracture. All fractures with displacement less than 1mm treated conservatively were included. The ulnar variance was measured in all patients. RESULTS: Ulnar variance was measured in standard posteroanterior wrist radiographs of 95 patients. Eighteen patients (19%) developed scaphoid nonunion, with a mean value of ulnar variance of -1.34 (-/+ 0.85) mm (CI -2.25 - 0.41). Seventy seven patients (81%) h

Does correction of carpal malalignment influence the union rate of scaphoid nonunion surgery?

UNLABELLED: The aim of this retrospective study was to assess the relation between carpal malalignment correction and radiological union rates in surgery for scaphoid nonunions. A total of 59 scaphoid waist fracture nonunions treated with open reduction and palmar tricortical autograft were divided according to their pre- and postoperative scapholunate (SL) and radiolunate (RL) angles. We found that carpal malalignment failed to correct in 32 of 59 (54.2%) patients despite meticulous surgical technique and placement of an appropriately sized wedge-shaped graft. In total, 43 (72.9%) fractures u

[Wrist perilunate dislocation].

Carpal perilunate dislocations are rare and underdiagnosed because of an ignorance of the pathology and an inappropriate incidence of radiography. The carpus is composed of two rows of bones and 33 ligaments. These ligaments are fundamental for wrist stability. Typically, the injury occurs in young male adults exposed to high-energy trauma with a wrist in hyperextension. The diagnosis is easy with a physical examen and standard postero-anterior and lateral radiographs. The Witvoët and Allieu classification of the perilunate dislocations is the most used. This classification is composed of thre

Vascularised pedicular bone grafts.

When a massive free bone graft has to be incorporated into a large bone defect in the presence of a poor vascular recipient bed, the risks of absorption and failure of the graft to revascularise are high. Experimental studies have confirmed that a bone graft transferred to its recipient site with an intact pedicle of blood supply remains viable, and unites directly with the recipient bone without having to be revascularised and replaced by creeping substitution. It also provides a live bone bridge for reconstruction of a massive bone defect, and is a ready source of vascular osteogenic tissue

Conformational changes in the carpus during finger trap distraction.

PURPOSE: Wrist distraction is a common treatment maneuver used clinically for the reduction of distal radial fractures and midcarpal dislocations. Wrist distraction is also required during wrist arthroscopy to access the radiocarpal joint and has been used as a test for scapholunate ligament injury. However, the effect of a distraction load on the normal wrist has not been well studied. The purpose of this study was to measure the three-dimensional conformational changes of the carpal bones in the normal wrist as a result of a static distractive load. METHODS: Using computed tomography, the d

The evolving role of wrist arthroscopy.

Wrist arthroscopy is a valuable tool in hand surgery. However, as with every tool, its use must be executed by the right hands with the right training and right instrumentation. In this article, four papers on arthroscopy were examined for their contribution to the literature and to reflect on the evolving role of arthroscopy in hand surgery. The dorsal sensory branch of the ulnar nerve remains vulnerable with the placements of ulnar portals. Building from previous work, a 'nerve density' concept was presented, as it relates to wrist pronation and supination. The advent of arthroscopy has allo

Triquetral Fractures Overview.

PURPOSE OF REVIEW: The triquetrum is the second most commonly fractured carpal bone, comprising 15-18% of all carpal bone fractures. This review summarizes the current knowledge of triquetral fractures, including the anatomy and pathophysiology, evaluation and diagnosis, treatment and management, post-treatment outcomes, and complications. RECENT FINDINGS: Triquetral fractures are frequently caused by impaction of the ulnar wrist after a fall on an outstretched hand or by avulsion of attached ligaments. There are three main types of triquetral fractures: dorsal cortical fractures, triquetral

[Carpal dynamic stability mechanisms. Experimental study].

OBJECTIVE: To evaluate, experimentally in cadavers, the effect of the motor muscles in the wrist in the kinetic behaviour of the carpal, under axial load, and the wrist in a neutral position. MATERIAL AND METHOD: The changes in the spatial orientation of the carpal bones were recorded with a movement trajectory gauge that functions with electromagnetic fields. A total of 30 fresh cadaver wrists were used, in which the principal motor tendons were isolated and subjected to loads proportional to the area of the physiological section of each muscle. The experiment was performed under isometric l

Cortical and trabecular bone contribute strength to the osteopenic distal radius.

Fractures of the distal radius are common, especially in postmenopausal women, and their prevalence increases with age. Knowledge of the factors that increase the risk of fracture in this metaphyseal region would have predictive and therapeutic implications. Of particular interest in this study were (a) the relative contributions of cortical and trabecular bone to the strength of the distal radius and (b) the best radiographic features to use as strength indicators. In 21 forearms from fresh cadavera (median age at the time of death, 75 years), single photon absorptiometry and quantitative com

Radiographic Characteristics of Carpal Instability Associated With Scaphotrapeziotrapezoid Osteoarthritis.

PURPOSE: Scaphotrapeziotrapezoid osteoarthritis (STT-OA) may be related to the progression of carpal instability. This study analyzed the radiographic carpal indices in wrists with varying grades of STT-OA to investigate an association between the severity of STT-OA and the occurrence of dorsal intercalated segment instability (DISI), and to identify radiographic characteristics of carpal instability associated with STT-OA. METHODS: We retrospectively measured carpal indices in 135 wrists with STT-OA (Crosby's classification grade 2, 62; grade 3, 73). Carpal indices measured included radiolun

Long-term follow-up of an undiagnosed trans-scaphoid perilunate dislocation demonstrating articular remodeling and functional adaptation.

The most common perilunate dislocation is the trans-scaphoid dorsal perilunate variant. It is estimated that up to 25% of perilunate injuries are diagnosed late. We report 66-year follow-up of an unreduced, previously undiagnosed trans-scaphoid perilunate dislocation of the wrist. At follow-up, unique post-traumatic articular remodeling was seen on wrist radiographs, and the patient had only a mild functional deficit.

Scapholunate ligament injury occurring with scaphoid fracture--a rare coincidence?

In a retrospective study the results of a series 34 wrist arthroscopies in 189 acute scaphoid fractures were analysed. Scapholunate ligament tears were found in 13 cases. In 10 cases there was complete disruption of scapholunate interosseous ligament causing dynamic instability. Partial ligament tears without instability were seen in three cases. Despite the limitations of this series we conclude that occurrence of scapholunate ligament injury with a scaphoid fracture may be more common than generally thought.

[Functional results of medio-carpal partial arthrodesis with excision of the scaphoid].

Midcarpal arthrodesis with excision of the scaphoid for the treatment of painful carpal collapse has been performed in our hospital since 1993. A clinical study was carried out to evaluate the results and determine special factors, which might influence the results. 26 out of 29 patients operated until 1999 were reexamined after an average follow-up of 27 months. The results were evaluated according to the Mayo-modified wrist score. The DASH score and the pain-disability index (PDI) were calculated postoperatively. Standard X-rays of the wrist were analyzed for alteration of the radio-lunate j

Do wrist anatomical differences predispose to scapholunate ligament injury? A case-control radiographic study.

PURPOSE: Scapholunate interosseous ligament (SLIL) injury is the most common cause of carpal instability and may lead to scapholunate advanced collapse if untreated. While several anatomical wrist variations have been implicated in other wrist pathologies, limited studies have explored their role in SLIL injuries. This study aimed to compare specific anatomical parameters on wrist radiographs between patients with arthroscopically confirmed SLIL injuries and healthy individuals to identify potential anatomical predispositions. METHODS: This study analyzed bilateral wrist radiographs of 87 pat
