Intervention and the hearing-impaired child--an evaluation of outcome.

The investigation was performed in order to evaluate the outcome of intervention in preschool age. The material comprises a cohort of 138 children, 76 boys and 62 girls, born during 1970-1980, living in the Copenhagen County at the time of data collection (1987), all suffering from predominantly congenital/early acquired hearing disability (i.e. hearing level greater than or equal to 35 dB HL for the average of 0.5-4 kHz on the better hearing ear). The type of school attended is used as a measure for outcome of preschool intervention. It was demonstrated that 49% attended normal class (speech)

Formant frequency discrimination with a fine structure sound coding strategy for cochlear implants.

Recent sound coding strategies for cochlear implants (CI) have focused on the transmission of temporal fine structure to the CI recipient. To date, knowledge about the effects of fine structure coding in electrical hearing is poorly charactarized. The aim of this study was to examine whether the presence of temporal fine structure coding affects how the CI recipient perceives sound. This was done by comparing two sound coding strategies with different temporal fine structure coverage in a longitudinal cross-over setting. The more recent FS4 coding strategy provides fine structure coding on typ

Influence of timing, radiation, and reconstruction on complications and speech outcomes with tracheoesophageal puncture.

BACKGROUND: We evaluated the impact of radiation, reconstruction, and timing of tracheoesophageal puncture (TEP) on complications and speech outcomes. METHODS: Retrospective review identified 145 patients who underwent TEP between 2003 and 2007. RESULTS: Ninety-nine patients (68%) had primary and 46 (32%) had secondary TEP, with complications occurring in 65% and 61%, respectively (p = .96). Twenty-nine patients (20%) had major complications (18 primary and 11 secondary; p = .42). Ninety-four patients (65%) had pre-TEP radiation, 39 (27%) post-TEP radiation, and 12 (8%) no radiation. Timing of Furlow Palatoplasty for Patients With Submucous Cleft Palate.

BACKGROUND: Submucous cleft palate (SMCP) is the most common form of cleft involving the posterior palate, resulting in variable degrees of velar dysfunction and speech disturbance. Although early surgical intervention is indicated for patients with true cleft palate, the indications for palatoplasty and timing of surgical intervention for patients with SMCP remain controversial. METHODS: Twenty-nine patients with SMCP were retrospectively reviewed. Patients treated with Furlow palatoplasty were dichotomized based on patient age at the time of surgical correction into early speech development

Word learning deficit among Chinese dyslexic children.

The present study examined word learning difficulties in Chinese dyslexic children, readers of a nonalphabetic script. A total of 105 Hong Kong Chinese children were recruited and divided into three groups: Dyslexic (mean age 8;8), CA control (mean age 8;9), and RL control (mean age 6; 11). They were given a word learning task and a familiar word writing task. It was found that the Dyslexic group performed less well than the RL group in learning irregular words over trials but not the regular ones. Error analyses showed that the Dyslexic group made more orthographic and word association errors

Functional hemitongue reconstruction with the microvascular ulnar forearm flap.

Thirteen patients with squamous cell carcinoma of the tongue underwent full-thickness longitudinal resection of the hemitongue and immediate microvascular reconstruction using a large, contoured ulnar forearm flap. Six of the 13 patients had a composite resection for which an additional vascularized iliac crest graft was used to reconstruct the mandible and to provide support to the overlying contoured flap. To increase tongue mobility, the skin flap was designed for independent reconstruction of the hemitongue and the floor of mouth. Twelve patients were evaluated for swallowing and speech, i

Application of a conversational AI system for voice-interactive history taking in neurosurgical patients.

BACKGROUND: ccurate and structured medical history taking is essential in neurosurgical practice, but repetitive inpatient interviews can be time-consuming for both clinicians and patients. Recent advances in conversational artificial intelligence (AI) and speech recognition have created new opportunities to support clinical documentation and patient communication. This pilot study evaluated the feasibility and patient satisfaction of an AI-based voice-interactive history-taking system in neurosurgical inpatients. METHODS: This single-center, non-interventional pilot study enrolled 16 hospita

Total or subtotal glossectomy with laryngeal preservation: a national study of 29 patients.

Total glossectomy remains a controversial procedure as it often leads to notorious sequalae in swallowing and speaking functions. Disease entities indicating total glossectomy tend to have poor prognosis. We evaluated whether this type of surgery can be concidered justified based on our national series. We reviewed all total and subtotal glossectomies with laryngeal preservation performed in Finland between 2005 and 2014 in terms of overall survival (OS), disease-specific survival (DSS), locoregional control (LRC), and functional outcome as assessed by gastric tube or tracheostomy dependence a

The applicability of a speech-in-noise screening test in occupational hearing conservation.

OBJECTIVE: Noise-induced hearing loss (NIHL) is the most reported occupational health disease in the Netherlands. The internet-based speech-in-noise test Earcheck (Albrecht et al, 2005; Leensen et al, 2011b) is designed to detect beginning NIHL and can be a valuable tool in occupational hearing health surveillance. The aim of this study is to investigate the validity of Earcheck compared to regular screening audiometry. DESIGN: Subjects performed online Earcheck tests at home. The results are compared to a pure-tone screening audiogram obtained during regular occupational health examination.

Association of PHEX Gene Dosage With Meniere Disease and Related Audiovestibular Phenotypes in X-Linked Hypophosphatemia.

IMPORTANCE: Meniere disease (MD) is a heterogenous disorder whose underlying etiologies remain poorly understood. A subtype of MD characterized by endolymphatic sac (ES) underdevelopment (ES hypoplasia), frequent bilateral disease, and male predominance-termed MD-hp-has emerged as a promising model for genetic investigation. OBJECTIVE: To test whether the PHEX gene underlies the association of XLH with MD-hp. DESIGN, SETTING, AND PARTICIPANTS: This prospective, cross-sectional, study was conducted at tertiary academic centers in the US (Boston, Massachusetts) and Switzerland (Zurich) from Ja

Diotic and Antiphasic Digits-in-noise Testing as a Hearing Screening and Triage Tool to Classify Type of Hearing Loss.

OBJECTIVES: The digits-in-noise test (DIN) is a popular self-test measure that has traditionally been used to screen for hearing loss by providing either a pass or refer result. Standard approaches either tested each ear monaurally or used a binaural diotic version where identical digits and noise were presented simultaneously to both ears. Recently, a dichotic, antiphasic version was developed, increasing sensitivity of the DIN to unilateral or asymmetric sensorineural hearing loss (SNHL) and conductive hearing loss (CHL). The purpose of this study was to determine predictors and normative ra

Speech Recognition in Noise Using Binaural Diotic and Antiphasic Digits-in-Noise in Children: Maturation and Self-Test Validity.

BACKGROUND: Digits-in-noise (DIN) tests have become popular for hearing screening over the past 15 years. Several recent studies have highlighted the potential utility of DIN as a school-aged hearing test. However, age may influence test performance in children due to maturation. In addition, a new antiphasic stimulus paradigm has been introduced, allowing binaural intelligibility level difference (BILD) to be measured by using a combination of conventional diotic and antiphasic DIN. PURPOSE: This study determined age-specific normative data for diotic and antiphasic DIN, and a derived measur

Merging multimodal digital biomarkers into "Digital Neuro Fingerprints" for precision neurology in dementias: the promise of the right treatment for the right patient at the right time in the age of AI.

Digital biomarkers are revolutionizing medicine in ways that were unimaginable a few years ago. Consequently, precision medicine approaches now realistically can promise personalization, i.e., the right treatments for the right patients at the right time, including earlier, targeted interventions which lead to a major paradigm shift in how medicine is practiced from reactive to preventive action. Although the scientific evidence is clear on the power of digital biomarkers, there is an unmet need for translating these findings into actionable insights in clinical practice. In this paper, we foc

Bare-Die Antiferromagnetic Computing.

Semiconductor electronic devices are increasingly constrained by fundamental quantum tunneling effects and charge-based mechanisms, which severely limit further miniaturization, write-speed scaling, and environmental robustness of silicon-based technologies. These limitations are particularly prohibitive for deep-space exploration, where extreme temperatures, ultra-strong magnetic fields, and intense radiation rapidly incapacitate conventional electronics without massive shielding. Here, we present an intrinsically resilient, strain-mediated antiferromagnetic MnIr/PMN-PT edge processor that op

Toward a hybrid assessment framework for adolescent borderline personality disorder: a mini review of personality functioning, digital biomarkers, and AI-supported assessment.

Borderline personality disorder (BPD) in adolescence is increasingly recognized as a valid and clinically meaningful diagnosis, yet it remains frequently underdetected and undertreated. Diagnostic uncertainty often arises from overlap between typical developmental features, such as emotional reactivity, identity exploration, and fluctuating peer relationships, and the pathological instability characteristic of BPD. Stigma and clinician hesitancy to diagnose before age 18 further delay intervention, despite evidence that early identification improves outcomes. Dimensional diagnostic frameworks,

Intact but Protracted Facial and Prosodic Emotion Recognition Among Autistic Adults.

Despite extensive research efforts, it is unclear how autistic and non-autistic individuals compare in their ability to recognize emotions. Differences in demographic and task factors have been proposed as explanations for divergent findings, but limitations in samples and designs have obscured insight into this possibility. This study investigated the extent of emotion recognition differences among autistic adults and the influence of these factors upon them. We recruited a large sample of autistic and non-autistic adults (N = 1,239) spanning across adulthood (18-76 years). In three online ex
