Transfer of a Terminal Motor Branch Nerve to the Flexor Carpi Ulnaris for Triceps Reinnervation: Anatomical Study and Clinical Cases.

PURPOSE: To analyze the anatomical feasibility of transferring a motor branch nerve to the flexor carpi ulnaris (FCU) to the triceps upper medial head motor branch (UMHM) and to report the resultant outcome of the restoration of elbow extension in 5 patients with extensive brachial plexus injury. METHODS: The ulnar and radial nerves were dissected in 10 cadavers. We measured the length and diameter of the branches to the FCU and the UMHM branch and counted the axons. Then, 5 male patients, mean age 30 years, underwent FCU nerve branch transfer for reconstruction of elbow extension. Elbow flex

A case report of a reverse sural flap for posterior ankle soft-tissue defect following an injury in a patient from Tanzania.

In the treatment and management of severe wounds, microsurgical repair remains the gold standard. However, it is difficult to transfer free tissue from a Tanzanian perspective due to donor site morbidity, longer operational times, bulky forms, recipient vessel stress, sophisticated surgical expertise, and high costs of the equipment. Meanwhile, the reverse sural flap has been considered as the ultimate tissue restoration technique. This study reviews a case of a 35-year-old man who was admitted at Mbeya Zonal Referral Hospital in the Department of Orthopaedics, Trauma, and Neurosurgery in Mbey

Combined injuries--soft tissue management.

The combination of severe bone and soft tissue injuries challenges all hand surgeons. Immediate restoration of all damaged structures is the goal whenever possible, integrating soft tissue techniques with principles of internal fixation. Debridement must be radical and resulting defects in bone, vessel, nerve, tendon and skin must be reconstituted with the combination of free and vascularized grafts. Rigid internal fixation is mandatory to allow functional restoration of the hand to begin with a stable platform against which motor tendon units and gliding structures to move. The timing of subs

Ultrasound-Targeted Microbubble Destruction Delivery of Insulin-Like Growth Factor 1 cDNA and Transforming Growth Factor Beta Short Hairpin RNA Enhances Tendon Regeneration and Inhibits Scar Formation In Vivo.

Ultrasound-targeted microbubble destruction (UTMD), which has been successfully used for the treatment of many diseases, offers a promising noninvasive approach for target-specific gene delivery. This study investigated the UTMD delivery of insulin-like growth factor 1 (IGF-1) cDNA and transforming growth factor beta (TGF-β) short hairpin RNA for Achilles tendon injury in rats. Briefly, 168 rats with an injured Achilles tendon were randomly divided into seven groups: (1) IGF-1 + UTMD, (2) TGF-β + UTMD, (3) IGF-1 + TGF-β + UTMD, (4) control, (5) IGF-1, (6) TGF-β, and (7) IGF-1 + TGF-β. Repair of Pronator Quadratus With Partial Muscle Split and Distal Transfer for Volar Plating of Distal Radius Fractures.

Flexor tendon injury is a rare, but serious, complication after volar plate fixation for distal radius fractures. The plate position and prominence at the watershed line are contributing factors that cause flexor tendon injury. With the standard volar approach, the pronator quadratus (PQ) is typically elevated off the radial attachment. The distal part of the plate is often visible after repair of the PQ. We describe a "PQ-splitting" technique for covering the distal edge of the plate if primary PQ repair cannot completely cover the distal part of the plate. We also report the outcome of our s

Outcomes of modified 2-incision technique with use of indomethicin in treatment of distal biceps tendon rupture.

Multiple surgical techniques for distal biceps tendon ruptures exist. Heterotopic ossification is an associated omplication of the 2-incision technique. The purpose of this study was to review the results of distal biceps tendon repairs via the modified 2-incision technique using indomethacin chemoprophylaxis. A retrospective review of 34 2-incision repairs of the distal biceps tendon was performed. All patients received 6 weeks of indomethacin treatment postoperatively to prophylax against heterotopic ossification. Outcome measures included disabilities of the arm, shoulder, and hand (DASH) s

Extensor indicis proprius transfer for the abducted small finger.

PURPOSE: Persistent abduction of the small finger has usually been treated by transfer of the extensor digiti minimi muscle. However, anatomic variations of the extensor system may limit the potential for a successful extensor digiti minimi transfer. Therefore, we evaluated the outcomes of an alternative reconstruction method for the abducted small finger using an extensor indicis proprius (EIP) transfer. METHODS: We performed 8 EIP transfers in 8 patients with persistent, flexible abduction posturing of the small finger. The primary etiology of the deformity was incomplete motor reinnervatio

Hamstring muscle function after tendon harvest for anterior cruciate ligament reconstruction: evaluation with T2 relaxation time of magnetic resonance imaging.

BACKGROUND: Regeneration of the semitendinosus and gracilis tendons after harvesting for anterior cruciate ligament reconstruction has been reported; however, muscle belly function after tendon regeneration has not been well documented. HYPOTHESIS: The semitendinosus and gracilis muscles are highly activated during knee flexion if their tendons are well regenerated after anterior cruciate ligament reconstruction. STUDY DESIGN: Descriptive laboratory study. METHODS: Hamstring muscle activation in 11 patients who had undergone anterior cruciate ligament reconstruction with semitendinosus and

Rupture of flexor tendons as a complication of a distal radius fracture.

Two cases are reported of flexor tendon rupture of ulnar side fingers as a complication after distal radius fracture. One was associated with a prominent distal volar lip of fracture site, the other with a relatively volarly displaced ulnar head due to the dorsally tilted distal radius due to malunion. These may be unique cases of flexor tendon rupture with conservatively treated patients.

Ponseti method: does age at the beginning of treatment make a difference?

UNLABELLED: The Ponseti method is reportedly effective for treating clubfoot in children up to 9 years of age. However, whether age at the beginning of treatment influences the rate of successful correction and the rate of relapse is unknown. We therefore retrospectively reviewed 68 consecutive children with 102 idiopathic clubfeet treated by the Ponseti technique in four Portuguese hospitals. We followed patients a minimum of 30 months (mean, 41.4 months; range, 30-61 months). The patients were divided into two groups according to their age at the beginning of treatment; Group I was younger t

Ulnar subluxation of a ruptured EPL tendon at the metacarpophalangeal joint: case report.

We report a case of ulnar subluxation of a ruptured extensor pollicis longus tendon at the metacarpophalangeal joint due to tethering of the distal tendon and an abnormal vector of pull. In this case, thumb flexion with the tethered EPL tendon resulted in attenuation of the radial sagittal band of the extensor hood. After plication of the radial sagittal band and relocation of the EPL tendon, adequate thumb extension was restored.

External fixation use in the management of massive upper extremity trauma.

Immobilization of fractures associated with instability and comminution by an external skeletal device facilitates wound care, skin coverage, and ultimate reconstructive surgery. External skeletal fixation was applied in 20 patients who sustained complex open injuries of the upper extremity. All injuries were associated with extensive soft tissue and skin defects. Associated neurovascular injuries included 14 nerve injuries and five major vascular injuries. The Hoffman fixation device was used in the majority of cases. Skin coverage was obtained in all patients by an appropriate method of skin

Replacement of the trapezium for arthritis of the basal articulations: a new technique with stabilization by tenodesis.

Forty-six patients (fifty hands) had replacement of the trapezium by a silicone implant that incorporated a local tendon strip to stabilize the device. The patients had intractable pain due to degenerative changes of multiple facets of the trapezium. The over-all rate of subluxation and dislocation was 10 per cent. In all hands, pain-free improved movement resulted. The pinch strength approximated the normal, averaging six kilograms for women and eight kilograms for men.
