OBJECTIVE: Describe the MRI findings in capillary telangiectasias.
MATERIALS AND METHODS: Between 1996 and 1999, we observed 9 cases of capillary telangiectasia in 7 patients explored 5 times for posterior fossa symptoms. In two cases capillary telangiectasia was a fortuitous discovery. All patients were explored by MRI with T1 sequences with and without gadolinium injections, turbo spin echo T2 coupled in 5 cases with a double echo gradient echo T2 sequence (TR: 970 ms, TE: 15 and 35 ms). Two patients also underwent vertebral angiography.
RESULTS: The telangiectasia gave a low intensity sig

OBJECTIVE: Skull base osteomyelitis (SBO) is a rare but life-threatening inflammatory disease often misdiagnosed as a malignancy, such as nasopharyngeal carcinoma (NPC) and external auditory canal cancer (EACC), due to extensive bone destruction. This study aimed to identify the radiological features of SBO using high-resolution CT, which could help in the differential diagnosis of malignancies.
METHODS: High-resolution CT findings of 14 patients with SBO, 25 with NPC, and 19 with EACC were retrospectively reviewed. Abnormal findings at seven sites: 1) external auditory canal, 2) mastoid port

BACKGROUND: Outcomes of petroclival meningiomas (PCM) (morbidity, permanent cranial nerves deficit, tumor removal and recurrence) are inconsistent in the literature, making it a challenge to predict surgical morbidity.
METHODS: A multicenter study of patients with PCMs larger than 2.5cm between 1984 and 2017 was conducted. The authors retrospectively reviewed the patients' medical records, imaging studies and pathology reports to analyze presentation, surgical approach, neurological outcomes, complications, recurrence rates and predictive factors.
RESULTS: There were 154 patients. The follow

Surgical approaches to the anterior cranial base have changed considerably with the introduction of endonasal endoscopic surgery. This study aims to define the factors which help in selecting the optimal surgical approach for the treatment of anterior cranial base encephaloceles. Patients who received treatment for anterior cranial base encephaloceles at our department between 1996 and 2011 were included in the study. Patients' charts were reviewed retrospectively to collect the necessary data. Treatment periods were classified as before 2000, between 2000 and 2005, and after 2005. The relatio

INTRODUCTION: Tentorial meningiomas account for 3-7.7% of all meningiomas in surgically treated patients. In this group, the falco-tentorial meningiomas are considered to be the rarest.
CASE REPORT: The paper presents three cases of the tentorial and falco-tentorial junction meningiomas. Two of the meningiomas were of major size and with an attachment at the falco-tentorial junction, while the other was attached to the inferior notch of the tentorium. The lesions manifested as headaches and disequilibrium that persisted for many years. The Parinaud syndrome and diplopia were observed in the p

CMA values have been effectively used to evaluate the amount of BI, the brainstem and medulla compression, and the amount of postoperative decompression. However, the reliability and reproducibility of this measurement have yet to be determined. In addition, the information that is available concerning CMA values in normal individuals has been limited to small series of patients. We recruited 200 patients that underwent MR imaging of the craniovertebral junction (CVJ) for unrelated reasons. None of the patients had evidence of abnormalities at the CVJ. Two senior spine surgeons then measured t

BACKGROUND: Hereditary spastic paraparesis (HPS) linked to mutations in the spastin gene (SPG4) is considered to be a pure form of spastic hereditary paraparesis. However, in this disease also other signs of central nervous system involvement are frequently found.
METHODS: Clinical, genetical and neuroradiological investigations were carried out in a large family with autosomal dominant spastic paraparesis and in a sporadic case with spastic paraparesis.
RESULTS: Additional clinical and molecular data are provided, studying other members of the same pedigree, as already described, with a fiv

The identification of Nocardia transvalensis, an unusual and probably underrecognized cause of nocardial infection, is clinically significant because of this species' resistance to aminoglycosides, a standard antinocardial therapy. Diagnosis requires analytic methods available predominately in reference laboratories. We report a case of disseminated infection with N transvalensis with primary pulmonary involvement and subsequent development of brain abscesses, and review the literature to date. Familiarity with the epidemiology, pathologic findings, and clinical significance of this and other

A 56-year-old man presented with a rare traumatic basilar artery occlusion caused by a fracture of the clivus. He fell from the height of 2 meters and immediately fell into a coma. Head computed tomography (CT) revealed an open depressed fracture, an acute epidural hematoma 1 cm thick in the left middle frontal fossa, and a longitudinal fracture of the clivus. Emergency removal of the hematoma was performed with cranioplasty. Head CT 8 hours 50 minutes after injury showed infarctions in the brain stem, cerebellum, and occipital lobes. Cerebral angiography revealed occlusion of the basilar arte

The results of posterior fossa explorations for trigeminal neuralgia over the period 1980-1990 in 58 patients and in 59 procedures were studied retrospectively. In 51 procedures vascular compression was treated by microvascular decompression (MVD). In the absence of such a compression, partial sensory rhizotomy (PSR) was performed in 5 cases and only adhesiolysis of thickened arachnoidea in 2 cases. In one additional case the procedure was terminated prematurely due to the development of cerebellar edema. Two months postoperatively a good or fair result was obtained in 80% of the procedures. A

A series of 3,500 cases examined by computed tomography (CT) of the head were reviewed to identify patients who had symptoms and signs suggestive of neurologic disease in the posterior fossa. Three hundred forty eight such cases (about 10%) were found, and the scans were reviewed to determine the character of lesions that could be detected and the accuracy of the neuroradiologic diagnosis. Overall, 16 errors were made (4.6%), of which 13 were false negatives and 3 were false positives. False negatives included nine tumors, two infarcts, and two arteriovenous malformations. Four of the tumors a

BACKGROUND: Posterior fossa tumors are rare in adults and pose a challenge to treat due to the bony contour of the posterior fossa, complex anatomical structures including deep venous sinuses, and the proximity of the fourth ventricle and brain stem. We describe our experience with laser interstitial thermal therapy (LITT) for the management of brain metastases and radiation necrosis of the posterior fossa.
METHODS: We retrospectively analyzed 13 patients with metastases and radiation necrosis of the posterior fossa managed with LITT.
RESULTS: Thirteen patients with histopathologically confi
