Comparative changes of lipid levels in treatment-naive, HIV-1-infected adults treated with dolutegravir vs. efavirenz, raltegravir, and ritonavir-boosted darunavir-based regimens over 48 weeks.

BACKGROUND AND OBJECTIVES: Long-term use of antiretroviral therapy (ART) to treat HIV infection has been associated with dyslipidemia and metabolic and cardiovascular complications. Available options for patients at risk of cardiovascular disease include antiretroviral drugs with improved lipid profiles. Dolutegravir is one of a new generation of HIV integrase inhibitors recently incorporated into the US Department of Health and Human Services, German, Spanish, and Italian HIV treatment guidelines as a preferred first-line third agent in combination with dual nucleoside reverse transcriptase i

Solid lipid nanoparticles (SLN) of Efavirenz as lymph targeting drug delivery system: Elucidation of mechanism of uptake using chylomicron flow blocking approach.

The aim of the present work was to develop a lymph targeted SLN formulation of antiretroviral (ARV) drug and to have an understanding of its underlying mechanism of uptake by the lymphatics. The lymphatics are the inaccessible reservoirs of HIV in human body. Efavirenz (EFV) is a BCS class II, ARV drug that undergoes extensive first pass metabolism. The EFV SLN formulation was prepared using Gelucire 44/14, Compritol 888 ATO, Lipoid S 75 and Poloxamer 188 by hot homogenization technique followed by ultrasonication method, with mean particle size of 168 nm, polydispersity index (PDI) <0.220, an

A meta-analysis of sublingual allergen immunotherapy and pharmacotherapy in pollen-induced seasonal allergic rhinoconjunctivitis.

BACKGROUND: The capacity of sublingual allergen immunotherapy (SLIT) to provide effective symptom relief in pollen-induced seasonal allergic rhinitis is often questioned, despite evidence of clinical efficacy from meta-analyses and well-powered, double-blind, placebo-controlled randomized clinical trials. In the absence of direct, head-to-head, comparative trials of SLIT and symptomatic medication, only indirect comparisons are possible. METHODS: We performed a meta-analysis of classes of products (second-generation H1-antihistamines, nasal corticosteroids and grass pollen SLIT tablet formula

Biomarkers of CD4+ T-cell activation as risk factors for tuberculosis-associated immune reconstitution inflammatory syndrome.

OBJECTIVE: Patients coinfected with HIV and Mycobacterium tuberculosis frequently experience a paradoxical worsening of tuberculosis (TB) symptoms early after the initiation of combination antiretroviral therapy (cART). This immune reconstitution inflammatory syndrome (TB-IRIS) can lead to significant morbidity and needs to be distinguished from TB recurrence due to ineffective treatment. We investigated whether plasma biomarkers could predict the occurrence of TB-IRIS. DESIGN: ANRS 129 BKVIR is a single-arm multicentre trial that enrolled 69 cART-naïve HIV-1-infected patients treated for TB.

Protective effect of high-dose montelukast on salbutamol-induced homologous desensitisation in airway smooth muscle.

Montelukast (MK) is a potent cysteinyl-leukotriene receptor antagonist that causes dose-related improvements in chronic asthma. We sought to determine whether MK was able to prevent salbutamol-induced tolerance in airway smooth muscle. Homologous β2-adrenoceptor desensitisation models were established in guinea-pigs and in human bronchial smooth muscle cells (BSMC) by chronic salbutamol administration. Characterisation tools included measurement of the response of tracheal smooth muscle tissues to salbutamol, analysis of gene expression and receptor trafficking, evaluation of intracellular cAM

Bilateral oblique facial clefts and extremity anomaly in an infant after intrauterine efavirenz exposure and review of its teratogenic risk.

OBJECTIVE: Congenital anomalies may be caused by genetic or environmental factors or a combination of both. Oblique facial clefts are very rare congenital deformities. The occurrence of facial clefts and an extremity anomaly suggests a common underlying cause. Lateral oro-ocular clefts do not occur along normal developmental planes and may be part of the amnion disruption complex sequence. Our objective was to report a case of this very event, which also followed an unusual intrauterine exposure and review the literature on the teratogenic risk of efavirenz. STUDY DESIGN: We report a case of

In vitro and in vivo characterization of A-796260: a selective cannabinoid CB2 receptor agonist exhibiting analgesic activity in rodent pain models.

BACKGROUND AND PURPOSE: Selective cannabinoid CB2 receptor agonists have demonstrated analgesic activity across multiple preclinical pain models. AM1241 is an indole derivative that exhibits high affinity and selectivity for the CB2 binding site and broad spectrum analgesic activity in rodent models, but is not an antagonist of CB2 in vitro functional assays. Additionally, its analgesic effects are mu-opioid receptor-dependent. Herein, we describe the in vitro and in vivo pharmacological properties of A-796260, a novel CB2 agonist. EXPERIMENTAL APPROACH: A-796260 was characterized in radiolig

State and trait anxiety in patients affected by nasal polyposis before and after medical treatment.

The present study aimed to establish whether anxiety and depression in nasal polyposis play a role in genesis of the disease, or are a consequence of symptoms. Anxiety levels were evaluated in state and trait forms, and depression, in 30 consecutive patients presenting nasal polyposis before and after effective 7 months' medical treatment with nasal mometasone, loratadine and montelukast. Before and at the end of treatment, patients were asked to fill in the State and Trait Anxiety Inventory and the Zung self-rating depression scale. In 63.15% of patients with high levels of state anxiety befo

Effects of sigma ligands on NMDA receptor function in the bulbectomy model of depression: a behavioural study in the rat.

Sigma (sigma) ligands have been shown to modulate NMDA receptor activity. In the present study we used the olfactory bulbectomy (OBX) animal model of depression to assess the effects of the sigma1 ligand igmesine on OBX-induced behaviour. Behavioural experiments demonstrated OBX (saline-treated) rats to have increased dizocilpine-induced behavioural modifications, including locomotor and circling activity as compared to Sham rats (saline-treated). A short-term (7 d) treatment with low doses of igmesine (50-200 microg/kg.d s.c.) had no effect on dizocilpine-induced behaviour while long-term tre

Identification of a Functional CYP2C8 Variant Allele that Alters Splicing, Reduces Protein Expression, and Increases Drug Exposure.

This study investigated genetic determinants of the pharmacokinetics of the CYP2C8 index drugs repaglinide and gemfibrozil, and their interaction in healthy participants. Sequencing data from a study with montelukast revealed a novel functional CYP2C8 allele (rs2071426, CYP2C8*19), predicted to create an intronic splice donor site. In human liver samples, CYP2C8*19 associated with transcript-specific changes in CYP2C8 mRNA expression, reduced CYP2C8 protein expression, and decreased enzyme activity. Consistently, participants with the CYP2C8*19/*19 genotype had 45% greater area under the plasm

Neuropsychiatric outcomes after montelukast versus inhaled corticosteroids in pediatric asthma: a pooled birth cohort study.

BACKGROUND: The evidence regarding montelukast‑associated neuropsychiatric adverse events in children with asthma is controversial and inconclusive. We sought to further examine the association between use of montelukast and the risk of neuropsychiatric events in children with asthma. METHODS: We conducted a pooled analysis of two birth cohorts: the AMerican PREGNANcy-Mother-Child CohorT (AM-PREGNANT) from the US, and the Quebec Pregnancy Cohort (QPC) from Canada. We included children aged 1 to < 18 years with the first incident diagnosis of asthma after birth and whose initial asthma control

Changes in Metabolic Parameters After Switching From Efavirenz to Dolutegravir-Based Regimen: A Cohort Study in Thai People Living With HIV.

Switching from efavirenz (EFV) to dolutegravir (DTG) is increasingly common due to DTG's superior efficacy and tolerability, yet its metabolic impact in Southeast Asian populations remains unclear. We conducted a single-center retrospective cohort study (January 2019-December 2022) involving adults on EFV-based antiretroviral therapy for at least 6 months before transitioning to DTG. Weight, body mass index, lipid profiles, fasting blood glucose (FBG), renal function, and alanine transaminase (ALT) were measured at baseline, Month 6, and Month 12. Among 493 participants (mean age 47.6 years; 6

Ciprofol Alleviates Depressive-Like Behaviors in CUMS Mice Through PPARα-Associated ERK/CREB Signaling Activation.

BACKGROUND: Depression is a complex neuropsychiatric disorder involving neuroinflammation, synaptic dysfunction, and neurotransmitter dysregulation. Recent studies have highlighted the therapeutic potential of short-acting anesthetics in the treatment of depression. Ciprofol, a novel intravenous anesthetic with rapid onset and recovery, shows promise, although its antidepressant mechanisms remain underexplored. METHODS: We induced a depressive-like phenotype in mice using a 5-week chronic unpredictable mild stress (CUMS) protocol. Following model establishment, the mice received intraperitone

Effects of EP395, a novel macrolide, on acute neutrophilic airway inflammation.

Macrolide antibiotics have been shown to reduce exacerbations of respiratory diseases, including chronic obstructive pulmonary disease (COPD) and asthma. This effect is believed to be due to the immunomodulatory properties of macrolides rather than their antimicrobial activity. However, prolonged use of macrolide antibiotics can result in the development of antimicrobial resistance, which prompted us to develop EP395, a compound with similar pharmacological actions to macrolides, but without antimicrobial activity. We investigated EP395 in several established models of neutrophilic airway infl

Consideration of ledipasvir/sofosbuvir as an alternative to glecaprevir/pibrentasvir based upon on-treatment FIB-4 changes and sustained virologic response at 12 weeks in hepatitis C genotype 1 and 2 infections: A propensity score-matched study.

Limited data are available on the effectiveness of ledipasvir/sofosbuvir (LDV/SOF) compared with glecaprevir/pibrentasvir (GLE/PIB) for the treatment of hepatitis C virus genotype 2 infection. The sustained virologic response at 12 weeks (SVR 12) rate and on-treatment fibrosis index-4 (FIB-4) change in HCV GT-1 and GT-2 patients receiving LDV/SOF and GLE/PIB treatments were compared using propensity score matching (PSM). The proportion of treatment-naïve patients in each group was 93.3% and 95.7% (P = .313), respectively. The overall SVR 12 rates of LDV/SOF for 12 weeks and GLE/PIB for 8 weeks

Effectiveness of dolutegravir based antiretroviral treatment in preventing vertical transmission of HIV in West Bengal, India: A retrospective cohort study.

BackgroundSeveral antiretroviral regimens have been used for prevention of mother to child transmission (MTCT) of HIV, with maternal dolutegravir-based regimens (e.g. tenofovir/lamivudine/dolutegravir - TLD) being the latest, initiated in last 3-4 years. There is a need for assessment of effectiveness of this new regimen as compared to earlier efavirenz-based (tenofovir/lamivudine/efavirenz - TLE) and other regimens in program settings.MethodWe conducted this retrospective cohort study to determine effectiveness of the TLD regimen in preventing antenatal, intranatal and postnatal MTCT compared

Potential of Anti-Leukotriene Drugs as New Therapeutic Agents for Inhibiting Cholangiocarcinoma Progression.

Cholangiocarcinoma (CCA) is a cancer with a poor prognosis due to difficulties in diagnosis and limited treatment options, highlighting the urgent need for new targeted therapies. In a clinical setting, we found that leukotriene levels in bile were higher than in serum. Immunohistochemical analysis of surgically resected samples also revealed that CysLT receptor 1 (CysLTR1) was more highly expressed in CCA than in normal bile duct tissue, prompting us to investigate leukotriene as a potential therapeutic target in CCA. In vitro studies using CCA cell lines expressing CysLTR1 showed that leukot
