Three pathogenic factors are closely involved in the mechanism of acne with a sequence beginning with seborrhea, then sebum retention, and finally inflammation. The sebaceous gland is a target of androgens. Acne may be related to an excessive sensitivity of sebaceous end-organs to androgens. However, in women, an ovarian or adrenal hyperandrogenism may be implicated. The cause of sebum retention is the hyperkeratinisation of the infra infundibulum of the sebaceous duct. Several studies indicate that Pityrosporum ovale plays an important role in seborrhoeic dermatitis. One hundred and ten fertile women with such androgenism signs as seborrhea, acne, and hirsutism were examined to elucidate the contraceptive and therapeutic efficacy of hormonal contraceptive drugs anteovin, marvelon, and diane-35. The duration of pill administration for each women depended on her cycle duration. All the drugs had a 100% contraceptive efficacy and exerted a therapeutic effect on androgenism symptoms.

62% of 200 papillomatous nevus cell nevi compared to 45% of 40 seborrheic keratoses showed the presence of Pityrosporum ovale/orbiculare (Malassezia furfur) on their surface, supposedly representing a saprophytic state of the microorganisms in vivo. The papillomatous nevus cell nevi were of the compound or dermal type. Significant inflammation, dysplasia or major changes in color were absent. Electron microscopy revealed the characteristic, sometimes budding fungal cells. A relationship between the presence of Pityrosporum organisms and papillomatosis is discussed.

Epidermoid cysts with seborrheic keratosis-like linings are a recently described rare variant of epidermoid cysts. We report a case in which such a cyst developed following the excision of a typical seborrheic keratosis.

Thirty-six patients with seborrhoeic dermatitis of the scalp and culture positive for Pityrosporum ovale were treated in a double-blind placebo controlled study with ketoconazole shampoo twice weekly for 4 weeks. In the ketoconazole group, 16 of 18 patients (89%) became free of lesions or improved, compared with only 8 of 18 (p less than 0.01) in the placebo group. The patients found the shampoo effective, easy to use and cosmetically attractive.

The clinical and bacteriological findings are reported in 13 patients with gram-negative folliculitis before, during, and after treatment with isotretinoin (13-cis-retinoic acid). Patients were treated for 14-36 weeks with a daily dose of 0.5-1.5 mg isotretinoin/kg body weight. Inflammatory lesions were counted before and during weeks 2, 4, 8, 12, 16, 20, 24, 28, and 30 of therapy and again up to 172 weeks after cessation of treatment. A 42-year-old man had idiopathic lipemic tears and massive seborrhea. Recent investigations have shown that cholesterol and other lipids occasionally found in tears are produced by meibomian rather than lacrimal glands. The finding in this patient of hypersecretion by both sebaceous glands and meibomian glands, a sebaceous gland variant, suggests that a common mechanism may regulate secretory control of both of these glands.

The incidence and natural history of dermatosis papulosa nigra was assessed in eighty-two black patients. The overall incidence in the study population was 77 percent, with a predominance in women of almost 2:1.
