Kinetics of microfilaraemia & antigenaemia status by Og(4)C(3) ELISA in bancroftian filariasis.

BACKGROUND & OBJECTIVE: Bancroftian filariasis caused by Wuchereria bancrofti is endemic in many parts of India. In recent years diagnosis of W. bancrofti infection has been revolutionized with the availability of filarial antigen tests, which is important in monitoring success of chemotherapy. We carried out this study to measure microfilariaemia and antigenemia levels in bancroftian microfilariae (mf) carriers at 1 yr follow up after chemotherapy, in lymphoedema patients and in endemic controls from a filariasis endemic area in Tamil Nadu State using Og(4)C(3) ELISA to identify the best mark

A campaign of "communication for behavioural impact" to improve mass drug administrations against lymphatic filariasis: structure, implementation and impact on people's knowledge and treatment coverage.

In the mass drug administrations (MDA) that form the principal strategy of the Global Programme to Eliminate Lymphatic Filariasis, treatment coverages of at least 65%-80% will be needed if the programme is to be successful. In the Indian state of Tamil Nadu, where treatment coverages were typically <65%, a comprehensive strategy of advocacy and communication, called the "communication for behavioural impact" (COMBI) campaign, has been developed and implemented, in an attempt to improve treatment coverage. This strategy combined advocacy, aimed at state-, district- and village-level administrat

Setaria cervi: immunoprophylactic potential of glutathione-S-transferase against filarial parasite Brugia malayi.

Glutathione-S-transferase (GST) has been detected in the adult female Setaria cervi, a bovine filarial parasite. The role of S. cervi GST antigen in inducing immunity in the host against Brugia malayi microfilariae and infective larvae was studied by in vitro antibody dependent cell mediated reaction as well as in situ inoculation of filarial parasites within a microchamber in Mastomys. The immune sera from glutathione-S-transferase immunized Mastomys promoted the adherence of peritoneal exudate cells to B. malayi microfilariae and infective larvae in vitro inducing 80.7 and 77.6% cytotoxicity

Meta-analysis of age-prevalence patterns in lymphatic filariasis: no decline in microfilaraemia prevalence in older age groups as predicted by models with acquired immunity.

The role of acquired immunity in lymphatic filariasis is uncertain. Assuming that immunity against new infections develops gradually with accumulated experience of infection, models predict a decline in prevalence after teenage or early adulthood. A strong indication for acquired immunity was found in longitudinal data from Pondicherry, India, where Mf prevalence was highest around the age of 20 and declined thereafter. We reviewed published studies from India and Subsaharan Africa to investigate whether their age-prevalence patterns support the models with acquired immunity. By comparing prev

Modelling the epidemiology, transmission and control of lymphatic filariasis.

Wuchereria bancrofti transmitted by Culex quinquefasciatus accounts for >90% of the global burden of lymphatic filariasis (LF). Recent advances in diagnostic and control tools and a better epidemiological understanding of the disease have led to hope that LF is eradicable. The World Health Organization has helped a number of member countries to launch nation-wide programmes of mass treatment with antifilarial drugs such as diethylcarbamazine, albendazole and ivermectin, for the elimination of this disease. In order to make rational decisions about control strategies, reliable predictions of th

A polymerase chain reaction assay for the survey of bancroftian filariasis.

A polymerase chain reaction (PCR) assay based on a highly repeated DNA sequence found in Wuchereria bancrofti (SspI repeat) has been modified for the survey of bancroftian filariasis in expatriate workers (Myamese, Karen and Mon) from Myanmar where human filariasis is endemic. The PCR was very sensitive with the ability to detect the presence of as little as 10 pg of parasite DNA. The primers used in this PCR also showed highly specific amplification of parasite DNA without the presence of non-specific and non-target PCR products such as Brugia malayi, Plasmodium falciparum and human DNA. Molecular characterisation of the actin gene of the filarial parasite Wuchereria bancrofti.

Wuchereria bancrofti is the major cause of lymphatic filariasis in humans. Although it is responsible for this immensely morbid and debilitating disease, very little is known of the basic molecular biology of this parasite, and there is a vast lack of knowledge on its gene organisation. In this study, the actin gene of W. bancrofti has been characterised by sequencing a clone isolated from a genomic DNA library of this parasite. The 5' flanking region had a potential TATA box and a putative mRNA initiation site. The gene had five exons encoding 376 amino acids, and four introns ranging in size

Failure of highly immunogenic filarial proteins to provide host-protective immunity.

In areas that are endemic for lymphatic filariasis, there are individuals who are parasite free and who appear not to have experienced symptoms attributable to filarial infection. These "putatively immune" individuals may recognize immunogens that could be important in host protection. We have immunoscreened expression libraries expressing epitopes encoded by filarial open reading frames and have identified three antigens that are differentially recognized by the two polar clinical groups-endemic normals and asymptomatic microfilaremics. Pre-immunization of susceptible hosts (Meriones unguicul

A study on the provocative day test effect of ivermectin and albendazole on nocturnal periodic Wuchereria bancrofti microfilaraemia.

We conducted a randomized double-blind placebo-controlled study, in the Ahanta West District of Ghana, on the provocative day test effect of ivermectin and albendazole alone and in combination on nocturnal periodic Wuchereria bancrofti microfilaraemia. Sixty-three individuals with high night-time microfilaria (mf) intensities were identified in 1997 or 1998 and randomized into 4 groups. Blood samples for mf were then collected from the same individuals in the daytime (between 09:00 and 15:00) immediately before and 30-50 min after treatment. Groups 1-4 were treated with ivermectin alone (150-2

Lymphatic filariasis in a hyperendemic region: a ten-year, follow-up panel survey.

The present study is a long-term panel survey of a population living in a previously identified Wuchereria bancrofti-endemic area of Benin. Unexpectedly, a marked decrease in the prevalence of microfilaremia (from 9.4% to 0.48%; P < 0.001) occurred over a 10-year period in the absence of chemotherapy or vector control measures. The percentage of patients with chronic pathology remained stable during the study period. The decrease in the prevalence of parasitemia could not be explained by environmental or sociologic changes in the region, or by differences between the two study populations. The

Knowledge and beliefs about transmission, prevention and control of lymphatic filariasis in rural areas of south India.

To identify the gaps in people's knowledge of lymphatic filariasis, we collected descriptive and quantitative information in rural areas of Tamil Nadu, South India. People are well aware of filariasis in their communities and recognize its different clinical manifestations. Only 9% of those with and 20% of those without the disease knew that filariasis is caused through mosquito bites; the rest attributed it to many other causes. People's knowledge about transmission and prevention of filariasis is also very poor. The study identifies the rationale for people's misconceptions about the disease

Lymphoscintigraphic assessment of the effect of diethylcarbamazine treatment on lymphatic damage in human bancroftian filariasis.

Despite many millions of doses administered over the past 40 years, basic and crucial issues regarding the use, mode of action, and effectiveness of diethylcarbamazine (DEC) in many clinical situations remain unresolved. To directly investigate whether the well-known microfilaricidal and macrofilaricidal actions of DEC actually result in subsequent improvement in existing damage to lymphatic vessels or lymph nodes, 29 study subjects in Recife, Brazil were stratified into three groups according to the severity of clinical manifestations of lymphatic insufficiency. After baseline radionuclide ly

Regulation of the immune response in lymphatic filariasis and onchocerciasis.

The persistence of microfilariae in the blood or skin accompanied by a prominent eosinophilia and elevated serum IgE levels are common features of human infection with filarial parasites. In this review Christopher King and Thomas Nutman discuss recent findings on the role of interleukin 4 (IL-4), IL-5 and gamma-interferon (IFN-gamma) in the induction of these immediate hypersensitivity responses. They discuss the role of hypersensitivity in immunity, the development of immune tolerance to filarial antigens and suggest that could explain the impaired immune response of some individuals to fila

Frequency distribution of Wuchereria bancrofti microfilariae in human populations and its relationships with age and sex.

This paper examines the effects of host age and sex on the frequency distribution of Wuchereria bancrofti infections in the human host. Microfilarial counts from a large data base on the epidemiology of bancroftian filariasis in Pondicherry, South India are analysed. Frequency distributions of microfilarial counts divided by age are successfully described by zero-truncated negative binomial distributions, fitted by maximum likelihood. Parameter estimates from the fits indicate a significant trend of decreasing overdispersion with age in the distributions above age 10; this pattern provides ind

Antibodies to phosphocholine-bearing antigens in lymphatic filariasis and changes following treatment with diethylcarbamazine.

Sera from a total of 78 patients infected with Wuchereria bancrofti or appropriate controls were assayed for anti-phosphocholine antibodies in an enzyme-linked immunosorbent-assay (ELISA), using phosphocholine as an antigen. Anti-PC antibodies (both IgM and IgG) were observed in patients with all clinical forms of filariasis but, unexpectedly, were not significantly different from those of normal controls. Among the filariasis patients, however, individuals with patent microfilaraemia had significantly lower IgM-anti-PC titres (P less than 0.05) than did those of the other clinical groups. Com

Altered immune response (humoral and delayed-type hypersensitivity reactions) to sheep red blood cells in the course of experimental filarial infections (Litomosoides carinii, Brugia malayi, Acanthocheilonema viteae) of Mastomys natalensis.

Litomosoides carinii-, Acanthocheilonema viteae- or Brugia malayi-infected Mastomys natalensis were sensitised against sheep red blood cells (SRBC) on various occasions after infection to determine the effect of filarial infections on the immune response to a non-filarial antigen. The phagocytic activity of the reticuloendothelial system (RES) was controlled in vivo by the elimination of 51Cr-labelled SRBC. Antibody titres against SRBC (agglutinating and lytic antibodies) were similar to those of uninfected controls in L. carinii- or B. malayi-infected Mastomys sensitised during prepatency or

Comparative densities of Wuchereria bancrofti microfilaria in paired samples of capillary and venous blood.

Concurrent finger-prick and venous blood samples were obtained from 43 patients infected with Wuchereria bancrofti. Microfilariae were counted in blood smears and on nuclepore filters. Based on the numbers of microfilariae in 20-microliters finger-prick samples, an expected (theoretical) number of microfilariae in venous samples was calculated. This expected value was compared to the actual number of microfilariae present in venous blood. The actual number of microfilariae present ranged from 0.7 to 30 times (median 3) less than expected. The exponential function y = 16.74e.17x, r2 = .69, wher

Evaluation of specific humoral immune responses in human filariasis.

Using the antigens prepared from adult worms of B. malayi and D. immitis, a total of 37 malayan and 150 bancroftian serum samples were analysed for the presence of filarial specific antibodies by counterimmunoelectrophoresis and indirect enzyme linked immunosorbent assay. It was observed that microfilaraemic cases do show good specific humoral immune responses against homologous as well as heterologous antigen. Of the two antigens used, B. malayi adult antigen was found to be superior to D. immitis antigen in the detection of filarial antibodies.

Acute tropical pulmonary eosinophilia. Characterization of the lower respiratory tract inflammation and its response to therapy.

Although acute tropical pulmonary eosinophilia (TPE) is well recognized as a manifestation of filarial infection, the processes that mediate the abnormalities of the lung in TPE are unknown. To evaluate the hypothesis that the derangements of the lower respiratory tract in this disorder are mediated by inflammatory cells in the local milieu, we utilized bronchoalveolar lavage to evaluate affected individuals before and after therapy. Inflammatory cells recovered from the lower respiratory tract of individuals with acute, untreated TPE (n = 8) revealed a striking eosinophilic alveolitis, with m
