Surgical and pathogenetic considerations of frontal sinus fungus ball.

Fungus ball (FB) is an non-invasive form of mycosis, that generally affects immunocompetent and non-atopic subjects. Involvement of the frontal sinus is extremely rare. We report two cases with frontal sinus fungus ball that underwent endoscopic endonasal frontal Draf type IIb or III sinusotomy with complete removal of the cheesy clay-like material. There were no intra-operative or postoperative complications, and no recurrence of disease was evident during the follow-up of 51 and 26 months, respectively. The Draf type IIb or type III frontal sinusotomy seems to be highly effective for the tre

Long-term stenting for chronic frontal sinus disease: case series and literature review.

OBJECTIVE: The frontal sinus outflow tract consists anatomically of narrow channels prone to stenosis. Following both endonasal and external approach surgery, up to 30 per cent of patients suffer post-operative re-stenosis of the frontal sinus outflow tract, with recurrent frontal sinus disease. This paper proposes the surgical placement of a long-term frontal sinus stent to maintain fronto-nasal patency, as an alternative to more aggressive surgical procedures such as frontal sinus obliteration and modified Lothrop procedures. DESIGN: We present a series of three patients with frontal sinus

Allergic Aspergillus sinusitis: concepts in diagnosis and treatment of a new clinical entity.

The clinical features in 15 young adult patients with allergic Aspergillus sinusitis includes a history of asthma, recurrent nasal polyps, and radiographic evidence of pansinusitis. Multiple surgical procedures were performed on 12 patients. The histologic diagnosis is made on the mucinous material which contains eosinophils, fungal hyphae (Aspergillus species), and Charcot-Leyden crystals. Mycetomas, direct mucosal and soft tissue invasion by fungi, were not observed. Immunologic findings include an immediate cutaneous reactivity to Aspergillus in 60% of patients, total serum IgE elevation in

Intracranial complications of sinusitis in children. A sequela of periapical abscess.

The cases of two children with periapical abscess in the upper incisors, sinusitis, and intracranial abscess are described. The ethmoid and maxillary sinuses were involved in both patients. Subdural empyema occurred in both, and one of the children had also cerebritis and brain abscess. Anaerobic bacteria were isolated from the infected subdural empyemas. Peptostreptococcus intermedius and microaerophilic streptococci were recovered in one patient and Fusobacterium sp in the other. Surgical drainage and appropriate antimicrobial therapy resulted in complete eradication of the infection in both

[Comparative endoscopic and histological findings in chronic sinusitis (author's transl)].

Comparative studies between endoscopic and histological findings in 93 patients with chronic sinusitis showed that the diagnosis based upon endoscopic mucosal appearances does not compare necessarily with the histological findings. Therefore the endoscopic findings are questionable indications for surgery without histological confirmation.

Bicoronal Incision and Frontal-Basal Approach for Removal of Sinonasal Fibrous Dysplasia Complicated by Orbital Subperiosteal Abscess.

BACKGROUND: Fibrous dysplasia (FD) is a benign, slowly progressive disease resulting from the replacement of normal bone by fibro-osseous tissue. The incidence of craniofacial involvement of FD is as high as 23%. Sinonasal involvement of FD may lead to obstruction of the natural sinus ostium, resulting in acute sinusitis. We present a rare case of sinonasal FD complicated by subperiosteal abscess that was removed by bicoronal incision and frontal-basal approach in the second surgery. CASE DESCRIPTION: A 16-year-old male patient presented with painful swelling on his left eye that had persiste

Fungal rhinitis in dogs.

Fungal rhinitis and sinusitis in dogs are quite common reasons of chronic nasal discharge and rhinoscopy in such cases is commonly suggested. Forty three dogs were examined using rhinoscopy because of the presence of chronic airway symptoms. Clinical examination, routine hematology and serum biochemistry profiles, nasal and frontal sinus radiographs were made in all animals. Additionally, computed tomography in one dog was performed. Samples for histopathology were taken from 9 patients during rhinoscopy, additionally, from 8 of these patients samples for cytopathology were collected by blind

Middle turbinate preservation in endoscopic transsphenoidal surgery of the anterior skull base.

Endoscopic endonasal skull base surgery is a growing field in which the nasal corridors are used to address skull base lesions. Whether the … (MRI) studies were reviewed at a median of 16 months postoperatively and no patients (0%) developed frontal sinusitis. The middle turbinate can be preserved in nearly every endonasal, endoscopic transsphenoidal skull base case while still providing good exposure for successful tumor resection and skull base reconstruction. Postoperative sinonasal function may be be …

Indications for image-guidance in pediatric sinonasal surgery.

OBJECTIVES: To determine the indications and safety of image-guidance for pediatric sinonasal surgery. METHODS: A retrospective review was carried out of all patients undergoing image-guided sinus surgery using a single electromagnetic navigation system. All patients less than 18 years who underwent image-guided endoscopic sinus surgery over a 5-year period at a tertiary children's hospital were included. The means and ranges of age, preoperative setup time, gender distribution, and indications for surgery were determined. Operative time, anatomic regions explored, and intraoperative complica

[Atypical pneumatization of the paranasal sinus system].

200 CT scans of adult patients with paranasal sinus problems seen in the ENT Department either in the University Hospital in Berne, Switzerland, or in the University Hospital in Munich, Germany, were analysed. Among these we found 9 striking abnormalities of paranasal sinus anatomy. We found abnormalities which caused clinical symptoms, such as an infected concha bullosa, a fine-septated infected maxillary sinus, a pneumosinus dilatans of the frontal sinus with aesthetic problems, and a cellula orbito-ethmoidalis with sinusitis. Some of these abnormalities were found by coincidence, such as a

A case of giant fronto-ethmoidal osteoma (45 × 42 mm) with intracranial and orbital extension: a case report.

BACKGROUND: Osteomas are the most common benign tumors of the sinonasal region. Often asymptomatic, they are frequently discovered incidentally during imaging studies conducted for unrelated conditions. Osteomas typically range in size from 2 to 30 mm; those larger than 30 mm or weighing more than 110 g are classified as "big" or "giant" osteomas. Giant osteomas of the paranasal sinuses are extremely rare and can extend into the intraorbital or cerebral space, leading to significant complications. When osteomas become symptomatic, the standard treatment is surgical intervention to alleviate as

Precision Endonasal Endoscopic Surgery of the Frontal Recess Cells and Frontal Sinus Guided by the Natural Sinus Drainage Pathway.

BACKGROUND: The endoscopic endonasal approach to removing lesions in the nasal cavity and sinuses has become the modern first choice. However, if endoscopic surgery is performed without proper knowledge of sinus anatomy, there is a risk of residual lesions, recurrence, and even serious complications. Therefore, this article illustrates the importance of precise sinus opening guided by the natural sinus drainage pathway, using the anatomy of the frontal sinus (FS) and the frontal recess (FR) cells as an example. METHOD: A total of 82 sides cadaveric heads were dissected and analyzed, and the n

Pneumosinus dilatans: is it more than an aesthetic concern?

BACKGROUND: Pneumosinus dilatans (PD) is a pathologic condition involving the hyperaeration of one or several of the paranasal sinuses that can lead to significant deformation of the overlying bone. Although the presenting complaint of patients with PD is most commonly aesthetic in nature, the condition has also been associated with intracranial tumors and several other serious conditions. MATERIALS AND METHODS: A meta-analysis of all available clinical publications on the subject of PD was conducted. Patients were categorized on the basis of their sinus involvement. Associated conditions wer
