Lipid peroxidation and antioxidant enzymes activity in Plasmodium vivax malaria patients evolving with cholestatic jaundice.

BACKGROUND: Plasmodium vivax infection has been considered a benign and self-limiting disease, however, recent studies highlight the association between vivax malaria and life-threatening manifestations. Increase in reactive oxygen species has already been described in vivax malaria, as a result of the increased metabolic rate triggered by the multiplying parasite, and large quantities of toxic redox-active byproducts generated. The present study aimed to study the oxidative stress responses in patients infected with P. vivax, who developed jaundice (hyperbilirubinaemia) in the course of the d

Biliary atresia: the animal models.

Biliary atresia (BA) is a progressive fibrosing process of the neonatal biliary tree and liver, of unknown origin, and an as-yet unexplained pathologic mechanism. The crucial point is to elucidate the origin of this rare disease to change palliative surgery to etiology-related procedures. Patient-based research can only begin at the time of the Kasai procedure and does not allow retracing of the pathology back to its origin. Basic research has focused on similar diseases in the veterinary literature and started to simulate BA in animal models. Unfortunately, even after 50 years of research, no

An unusual presentation of precursor T cell lymphoblastic leukemia/lymphoma with cholestatic jaundice: case report.

BACKGROUND: Cholestatic jaundice as a presenting symptom of Precursor T-lymphoblastic leukemia (T-ALL)/lymphoma (T-LBL) has never been reported in literature. Similarly, precursor T-ALL/T-LBL is characteristically negative for synaptophysin. We report the first case of a patient with precursor T-ALL/T-LBL who presented with cholestatic jaundice and aberrant tumor expression of synaptophysin. CASE REPORT: 42 year old male presented with anorexia, nausea, jaundice, pale stools, dark urine and about 35 pound weight loss over the previous 3 weeks. The initial laboratory work was suggestive of cho

Ultrasound-guided percutaneous cholecystostomy for acute neonatal biliary obstruction.

Use of a percutaneously-inserted cholecystostomy drainage tube is an effective therapeutic option for acute hyperbilirubinemia in severely-ill adult patients, but to our knowledge has not been previously reported in infants. We describe an infant who developed acute extrahepatic biliary tract obstruction with marked conjugated (direct) hyperbilirubinemia, and who was determined to be an unsuitable surgical candidate. Ultrasound-guided percutaneous cholecystostomy was performed and resulted in prompt, significant, and sustained decline in serum bilirubin levels. Potential risks and benefits, as

Clinical and pathological aspects of ARC (arthrogryposis, renal dysfunction and cholestasis) syndrome in two siblings.

We describe the first family report of ARC syndrome (arthrogryposis multiplex congenita, renal dysfunction, and cholestasis) diagnosed in Turkey. ARC syndrome is a rare cause of cholestatic jaundice and skeletal abnormalities in the neonatal period. Fanconi-like renal tubular dysfunction completed the clinical picture. Consanguinity and affected membership are the other typical components of this rare disorder, and possibility of autosomal recessive transmission was considered. A broad spectrum of histopathological abnormalities have been described in the liver and kidney. In this report, we d

Complications of chronic pancreatitis.

The most common complication of chronic pancreatitis is pain, which in many cases seems related to pancreatic ductal obstruction with ductal hypertension. Longitudinal pancreaticojejunostomy is indicated in patients with a dilated (larger than 7 mm) duct and pain that requires narcotic analgesics for relief. Chronic pseudocysts may be corrected surgically without the usual 6-week wait, and asymptomatic pseudocysts less than 4 cm in diameter may not require surgery at all. The relative efficacy and risks of percutaneous drainage of pseudocysts versus the standard surgical approaches need to be

Effects of post-hepatic portoenterostomy adjuvant therapy on liver transplantation in children with biliary atresia: A systematic review.

OBJECTIVES: Biliary atresia (BA) is a cholangiopathy characterized by obstruction of the intrahepatic and extrahepatic bile ducts. Hepatic portoenterostomy (HPE) is the primary palliative treatment and there is still an urgent need to improve post-HPE management. This study aims to identify post-HPE adjuvant therapies associated with reducing or delaying the need for liver transplantation in children with BA. METHODS: This systematic review was registered on the PROSPERO platform (ID: CRD42024553671). Studies were extracted from the following databases: PubMed, Web of Science, Lilacs, and Sci

Improving detection and treatment of liver cancer.

Liver cancer is the sixth most common cancer worldwide. The majority (75-90%) of all primary liver tumours are hepatocellular carcinomas (HCC), arising from the liver parenchyma. Other primary liver cancers include cholangiocarcinomas (CC), which make up 10-25% of liver cancers. These tumours arise from cells lining the biliary tree. Chronic viral hepatitis, and in particular hepatitis B virus (HBV), remains the most important risk factor for the development of HCC. In the UK the incidence of CC is 1-2 cases per 100,000 population, but rates are increasing and in the mid-1990s CC overtook HCC

Resolution of intractable cholestasis associated with total parenteral nutrition following biliary irrigation.

Although the protracted use of total parenteral nutrition (TPN) in infants is known to predispose to cholestasis, which in certain instances may not be reversible, failure to understand the pathogenesis of this condition has hindered the development of an effective medical treatment. That surgical treatment may reverse this process in selected patients is the subject of the present report. Two premature infants and one term infant, with short bowel syndrome acquired in infancy, developed conjugated hyperbilirubinemia following institution of balanced TPN. Infectious and metabolic etiologies fo

Abscess of ligamentum teres hepatis post-endoscopic retrograde cholangiopancreatography: A case report and a literature review.

Abscess of the ligamentum teres hepatis has been described in the medical literature as an extremely rare clinical entity, which often presents a diagnostic dilemma. A 68-year-old man was hospitalized for upper abdominal pain and obstructive jaundice. The patient presented with low-grade intermittent fever. Laboratory investigations showed a white blood cell count of 32.38 × 109/L, a C-reactive protein level of 247.86 mg/L, abnormal liver enzyme and bilirubin levels, and elevated serum levels of amylase and lipase. He was first diagnosed with acute biliary pancreatitis. A computational tomogra

Case Report: Airway obstruction as a fatal complication of infantile hemangioma in newborns: the eye cannot see what the mind does not know.

BACKGROUND: In newborns, infantile hemangiomas (IHs) in the airway can present with rare life-threatening complications associated with airway obstruction, even without cutaneous hemangioma. CASE REPORT: Our female newborn exhibited stridor 1 day after her birth. Furthermore, 7 days after birth, she was admitted to the local hospital to receive phototherapy for neonatal jaundice. However, her stridor was not noted as being related to IH by the clinician during her hospitalization. She was transferred to our hospital at 14 days old because her stridor and condition deteriorated with severe bre

[Therapeutic strategy for type 4 gastric cancer from the clinical oncologist standpoint].

Type 4 gastric cancer has a poor prognosis compared with other types of advanced gastric cancer because of the high incidence of peritoneal metastasis which causes intestinal obstruction, hydronephrosis, or obstructive jaundice. Surgical treatment is often only palliative, and systematic chemotherapy is considered to be important for long survival. S-1 showed a higher response rate for undifferentiated-type adenocarcinoma, and S-1 alone or its combination regimens demonstrated greater anti-tumor effects and longer survival time for gastric linitis plastica compared with conventional 5-FU regim

Comparison of CT duodeno-cholangiopancreatography to ERCP for assessing biliary obstruction.

The authors have developed a CT protocol, CT duodeno-cholangiopancreatography (CDCP), which is performed during a single contrast-enhanced phase, proceeding cranially, allowing enhancement of the pancreas during its parenchymal phase followed by enhancement of the liver during its portal-venous phase. This retrospective pilot study evaluates CDCP compared with endoscopic retrograde cholangiopancreatography (ERCP) as a diagnostic tool for assessing the cause and level of biliary obstruction. Forty-one patients with jaundice underwent CDCP and ERCP between October 2002 and May 2004. Pathologic c

Comparative Analysis of Infection Rates Between Supra-Ampullary and Trans-Ampullary Biliary Stent Placements in Biliary Sepsis.

INTRODUCTION: Biliary stent placement is a critical palliative treatment for obstructive jaundice due to malignancies. The site, either supra-ampullary (above the ampulla of Vater) or trans-ampullary (below the ampulla of Vater), may influence complication rates which significantly impact patient outcomes. This study evaluates the infection and obstruction rates associated with these two distinct stent placement strategies. METHODS: Data from 66 patients who underwent biliary stent placements between 2017 and 2022 at our institution were analysed. Patients were categorised into two groups bas

Endosonography-guided gallbladder drainage for acute cholecystitis following covered metal stent deployment.

Endosonography-guided biliary drainage (ESBD) is gaining acceptance as an effective treatment for obstructive jaundice. Only a few reports on the application of this technique to the gallbladder (endosonography-guided gallbladder drainage [ESGBD]) have been published in the literature. In order to relieve acute cholecystitis which developed in a patient with unresectable malignant biliary obstruction after deployment of a covered metal stent (CMS), we applied this technique. ESGBD was carried out by using an electronic curved linear array echoendoscope. After visualization of the gallbladder a

[Benign recurrent intrahepatic cholestasis type-II--a rare cause of direct hyperbilirubinemia exacerbations with hepatic fibrosis].

Direct hyperbilirubinemia, may result from a variety of pathologies, including structural obstructions with biliary tract occlusions (as in cholelithiasis), infections (e.g. hepatitis) and genetic disorders (Rotor's and Dubin-Johnson's syndrome). One of the less common and probably more frequently underdiagnosed causes is benign recurrent intrahepatic cholestasis (BRIC). First described in 1959, BRIC was further classified into two subgroups which differ in their pathogenesis and clinical manifestation. Both BRIC types originate from impaired function bile salt excretion from hepatocytes to th

Spontaneous perforation of the extrahepatic bile duct in infancy: report of two cases and literature review.

Источник пометил публикацию как отозванную

BACKGROUND: Spontaneous perforation of the bile duct (SPBD) is a rare disease in infancy. The pathogenesis, diagnostic modalities and treatment options for this condition are reviewed and discussed. METHODS: The authors report 2 new observations of SPBD in 2 male newborns aged respectively 27 and 21 days. RESULTS: The 2 newborns presented with cholestatic jaundice. Abdominal sonography showed an extrahepatic mass. Exploratory laparotomy revealed that the perforation was located in the cystic duct in the first case and in the common bile duct (CBD) in the second. The site of perforation was r

Management of hepatic sinusoidal obstruction syndrome following treatment with gemtuzumab ozogamicin (Mylotarg).

Gemtuzumab ozogamicin (Mylotarg) therapy may cause sinusoidal obstruction syndrome (SOS), the mechanism of which probably involves targeting of CD33+ cells in the sinusoids of the liver, activation of stellate cells, damage to sinusoidal endothelial cells, sinusoidal vasoconstriction, and ischemic hepatocyte necrosis. The clinical manifestations of this liver injury are hepatomegaly, weight gain, ascites, jaundice, and elevation of serum aminotransferase enzymes. An approach to patient management includes being certain that SOS is the correct diagnosis; ensuring that liver blood flow is optimi
