Arthroscopic determination of patellofemoral malalignment.

Patellofemoral alignment was determinant by arthroscopy according to the degree of knee flexion at which the lateral facet, the patellar ridge, and the medial facet of the patella gained contact with the femur. The intraarticular pressure was 75 mm Hg. In 17 patients with a normal patellofemoral joint, the lateral facet aligned at a mean knee flexion of 20 degrees, the patellar ridge at 35 degrees, and the medial facet at 50 degrees. Twelve patients with patellar subluxation showed no difference in the alignment of the lateral facet, but differed significantly with regard to alignment of both

[Post-traumatic ankylosis (pathomorphologic aspects)].

At first, the symptoms observed in case of a post-traumatic ankylosis are defined. Then the possible reactions of tissue to a trauma to the joint or the joint region which may lead to an ankylosis are discussed with regard to general pathologic points of view. Some special pathologic aspects of lesions and their consequences are shown for the shoulder joint, the elbow joint, and the knee joint. Finally, several examples are given in order to illustrate possible histomorphologic symptoms leading to a post-traumatic ankylosis.

Successful perioperative preparation of a child with autism spectrum disorder in collaboration with his school for special needs education: a case report.

INTRODUCTION: The incidence of autism spectrum disorder (ASD) in children is greater than 1%. Children with ASD show an increased rate of hospital contact for several reasons. Perioperative preparation for children with ASD can be challenging; therefore, obtaining information from patients' families prior to surgery is important. However, no previous reports have described the collection of information from educational facilities. CASE REPORT: A 12 years-old male patient with ASD was referred for surgery for traumatic dislocation of the left knee joint. Before admission, we obtained valuable

Imaging of soccer injuries in adolescent female athletes.

Soccer participation is increasing among female adolescents with a concomitant rise of injuries in this group. Age- and sex-related factors, such as the transition from skeletal immaturity to skeletal maturity as well as anatomic, physiologic, neuromuscular, and behavioral differences between females and males, all play a role in injury patterns for these athletes. Anterior cruciate ligament (ACL) tears in this group have received a great deal of attention in the medical literature and the media in recent years with increasing knowledge about causative factors, surgical management, and injury

Deltoid ligament forces after tibialis posterior tendon rupture: effects of triple arthrodesis and calcaneal displacement osteotomies.

Deltoid ligament forces were studied after observing deltoid ligament insufficiency in several post-triple arthrodesis patients. Six fresh-frozen, below-knee amputation specimens were axially loaded. The results demonstrate that a properly positioned triple arthrodesis produced deltoid ligament forces that were similar to those seen with an intact tibialis posterior tendon. A triple arthrodesis in combination with a lateral displacement calcaneal osteotomy produced deltoid ligament forces that were 76% greater than those seen with the intact tibialis posterior tendon (P < .05). A triple arthro

Rheumatoid Arthritis of the Foot and Ankle.

Rheumatoid arthritis of the foot and ankle can be a debilitating problem, particularly for patients who have undergone successful hip or knee arthroplasty. Optimal medical management, use of orthotic devices, and surgical intervention are essential components of patient care. Forefoot involvement with hallux valgus and lesser metatarsophalangeal joint subluxation and dislocation are the most common findings. Reconstruction usually requires lesser metatarsophalangeal joint excisional arthroplasty and first metatarsophalangeal joint arthrodesis. Midfoot tarsometatarsal and intertarsal involvemen

Double-Bundle Posterior Cruciate Ligament and Concomitant Medial Collateral Ligament and Posterior Oblique Ligament Reconstruction After a Motor Vehicle Accident.

BACKGROUND: Motor vehicle accidents constitute the most prevalent high-energy mechanisms for knee dislocation, resulting in multiligament knee injuries (MLKIs). When concomitant posterior cruciate ligament (PCL) and posteromedial corner injury becomes chronic after failed conservative treatment, surgical intervention is indicated to restore the native knee biomechanics. INDICATIONS: Double-bundle PCL reconstruction is indicated for chronic instability, increased posterior tibial translation, decreased tibial slope, and failure of conservative treatment measures. posteromedial corner reconstru…

Age at Time of Surgery but Not Sex Is Related to Outcomes After Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction has become a well-accepted procedure for recurrent patellofemoral instability. Despite a growing volume of research assessing surgical results, the relationship of age and sex to outcomes after MPFL reconstruction surgery is unclear. PURPOSE: To investigate whether age at the time of surgery or sex influenced patient-reported quality of life and clinical outcome after MPFL reconstruction for recurrent lateral patellofemoral instability. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Between January 2010 and May 201

Short-term surgical complications following fast-track medial unicompartmental knee arthroplasty.

AIMS: Medial unicompartmental knee arthroplasty (mUKA) is an advised treatment for anteromedial knee osteoarthritis. While long-term survival after mUKA is well described, reported incidences of short-term surgical complications vary and the effect of surgical usage on complications is less established. We aimed to describe the overall occurrence and treatment of surgical complications within 90 days of mUKA, as well as occurrence in high-usage centres compared to low-usage centres. METHODS: mUKAs performed in eight fast-track centres from February 2010 to June 2018 were included from the Lun

Treatment of PCL, ACL, and lateral-side knee injuries: acute and chronic.

Combined posterior cruciate ligament, anterior cruciate ligament, and lateral-side disruption is one of the more common patterns of multiligament knee injury. This is a devastating injury with significant long-term functional sequelae, making accurate diagnosis and management extremely important. While surgical intervention is necessary to restore function, the specific management strategies remain controversial. This article will review the current literature and the authors' preferred approach including physical examination, imaging, timing of surgery, surgical technique, and postoperative r

Accuracy of femoral tunnel positioning in medial patellofemoral ligament reconstruction: anatomic insertion leads to better clinical outcome.

PURPOSE: The medial patello-femoral ligament (MPFL) is considered the primary restraint against patellar dislocation and its reconstruction is indicated in recurrent patellar instability. An anatomical positioning of MPFL femoral insertion is recommended to achieve satisfactory clinical outcomes and prevent osteoarthritis (OA) due to an altered kinematics. The purpose of the study was first to assess the relationship between correct femoral tunnel position and better clinical outcomes and lower patellofemoral osteoarthritis rate. Second, correlation of outcomes with factors potentially affecti
