Vulnerable road users are at greater risk during Ramadan -- results from road traffic surveillance data.

OBJECTIVE: To assess how the frequency, nature and outcome of road traffic crashes differ during the fasting month of Ramadan. METHODS: The retrospective study was conducted in Karachi and comprised data from the Road Traffic Injury Surveillance Project which entailed information on all road traffic injury victims presenting to Emergency Departments in the city between September 2006 and September 2011. Data was analysed to find the frequency of road traffic crashes according to time of incident, road user group and survival. Ramadan and Non-Ramadan groups were compared with respect to time a

Pseudoparalysis of shoulder caused by glenohumeral interposition of rotator cuff tendon stumps: a rare complication of posterior shoulder dislocation.

Magnetic resonance imaging (MRI) findings of a rare complication of posterior glenohumeral dislocation are described in two patients which led to shoulder dysfunction caused by interposition of rotator cuff tendon(s) into the glenohumeral joint. Both patients underwent successful surgical fixation and now have returned to normal shoulder function.

Calcitonin for acute neuropathic pain associated with spinal cord injury.

Neuropathic pain associated with spinal cord injury is caused by complex neural mechanisms and is often refractory to standard therapy. Salmon calcitonin was an effective treatment for neuropathic symptoms in this case series of three patients with recent spinal cord injury. Salmon calcitonin is already used to help manage pain after limb amputation and also after vertebral fractures and it is perhaps surprising that it has not been trialled previously for spinal cord injury pain. Calcitonin is thought to exert its effect by modulation of the serotonergic system and is generally well tolerated

Physiological, biochemical and functional changes induced by a simulated 30 min off-road competitive motocross heat.

AIM: Up to now, few studies regarding the impact of motocross heats on functional and physiological variables have been addressed so far. The aim of the present study was to analyze physiological and functional changes induced by a simulated off-road motocross heat. METHODS: Fifteen motocross riders (28.3+/-7.9 yrs; 71.1+/-7.0 kg; 169.0+/-4.0 cm; 53.5+/-3.7 mL.kg(-1).min(-1); 14.9+/-3.3% fat) performed one treadmill running to exhaustion to determine maximal heart rate (HRmax) and maximal oxygen consumption. Thereafter, simulated 30 min competitive off-road motocross heats were performed to m

Injury patterns associated with personal mobility devices and electric bicycles: an analysis from an acute general hospital in Singapore.

INTRODUCTION: The increasing popularity of personal mobility devices (PMDs) and electric bicycles (e-bikes) in Singapore has met with growing public concern over safety. Following the passing of the Active Mobility Bill, there remains a gap in the local literature in terms of injury patterns arising from the use of these devices. METHODS: A retrospective study was conducted on the Singapore National Trauma Registry (SNTR) from the emergency department of an acute hospital from 1 January 2016 to 31 December 2016. All patients with injuries related to the use of PMDs and e-bikes were included.

Prehospital airway management using the laryngeal tube. An emergency department point of view.

BACKGROUND: Competence in airway management and maintenance of oxygenation and ventilation represent fundamental skills in emergency medicine. The successful use of laryngeal tubes (LT, LT-D, LTS II) to secure the airway in the prehospital setting has been published in the past. However, some complications can be associated with the use of a laryngeal tube. METHODS: In a nonconsecutive case series, problems and complications associated with the use of the laryngeal tube in prehospital emergency medicine as seen by independent observers in the emergency room are presented. RESULTS: Various pr

Traumatic brain injuries caused by traffic accidents in five European countries: outcome and public health consequences.

BACKGROUND: Road traffic accidents (RTAs) have been identified by public health organizations as being of major global concern. Traumatic brain injuries (TBIs) are among the most severe injuries and are in a large part caused by RTA. The objective of this article is to analyse the severity and outcome of TBI caused by RTA in different types of road users in five European countries. METHODS: The demographic, severity and outcome measures of 683 individuals with RTA-related TBI from Austria, Slovakia, Bosnia, Croatia and Macedonia were analysed. Five types of road users (car drivers, car passen

Aberrant right subclavian artery in blunt aortic injury: implication for treatment and review of the literature.

Blunt aortic injury (BAI) involving an aberrant right subclavian artery (ARSA) is uncommon. The presence of an ARSA entails several treatment issues, in particular regarding the risk of perioperative posterior cerebral stroke. We report the case of a man with an ARSA who suffered from BAI. An initial conservative treatment with delayed open repair was chosen. A review of the published literature with discussion of the case is presented. The presence of an ARSA in patients with BAI can be considered a poor anatomy for endovascular repair, and suitable patients may benefit from delayed open repa

Incarcerated patellar tendon in Hoffa fracture: an unusual cause of irreducible knee dislocation.

Irreducible knee dislocation due to incarcerated soft tissue structures in associated fractures is extremely uncommon. We report a case of irreducible, closed dislocation of the knee secondary to incarcerated patellar tendon in a Hoffa fracture (unicondylar fracture of femur in the coronal plane) with associated communited fracture of patella and disruption of both cruciates. We treated this case initially with open reduction and internal fixation of fracture and repair of the patellar tendon. During the total followup period of eighteen months, the patient had restricted joint range of motion

Facial fractures in a level I trauma centre: the importance of protective devices and alcohol abuse.

Urban trauma centres have recently noted a shift in the causative mechanism of facial fractures away from motor vehicle crashes (MVC) to blunt assaults (BA). This study was conducted to examine the incidence and aetiology of facial fractures at our institution as well as the relationship with alcohol and protective device use. Trauma registry records of all patients admitted to a level I trauma centre from 1 January 1988 to 1 January 1999 were reviewed. There were 13594 trauma admissions during the 11-year period. Facial fractures were sustained by 1429 patients (10.5%) and this group forms th

Surgical Management of Pediatric Acetabular Fractures: A Retrospective Study of 14 Rare Cases.

BACKGROUND: Acetabular fractures in children are extremely rare, accounting for approximately 1%-4.6% of all pediatric fractures. Due to their rarity, literature on these injuries is limited, with only a few reported cases. The primary objective of this study was to present a series of uncommon pediatric injuries, outline our management approach, and demonstrate that even patients undergoing delayed surgical intervention can achieve favorable clinical outcomes. MATERIALS AND METHODS: This retrospective study reviewed records of skeletally immature patients with traumatic acetabular fractures

Traumatic posterior mediastinal hematoma with left atrial compression: A case series of a rare, life-threatening entity.

BACKGROUND: Traumatic posterior mediastinal hematoma (TPMH) is a potentially life-threatening entity that, in rare instances, can lead to obstructive shock and sudden death secondary to left atrial compression (LAC). We describe the clinical course and outcomes of seven adult patients with TPMH and LAC. A brief literature review is presented. METHODS: Seven patients (ages spanning from the 20s to 80s) with TPMH and LAC were identified at a Level 1 trauma center from 2016 to 2023. The first 2 patients were identified following standard quality improvement case reviews for unexplained death. Th
