Structured Symptom Assessment to Identify Patients With Small Fiber or Autonomic Neuropathy in Fibromyalgia.

BACKGROUND: Fibromyalgia (FM) is a complex chronic pain disorder characterized by widespread musculoskeletal pain and symptoms suggesting autonomic dysfunction. Small fiber neuropathy (SFN) has been described in a subgroup of patients. We aimed to explore the value of structured symptom assessment to identify patients with SFN or autonomic neuropathy. METHODS: Forty-six female FM patients were assessed in a cross-sectional study including clinical examinations, validated questionnaires (modified neuropathic pain symptom inventory (m-NPSI), PainDETECT and the composite autonomic symptom score

The Relationship Between Orthodontic Abnormalities and Spinal Deformities: A Review of Literature.

Spinal deformities comprise abnormal curvatures of the spine which impact posture, musculoskeletal balance, as well as biomechanical function. Malocclusion, the improper contact between occlusal surfaces of opposing teeth, affects the occlusal relationship and functionality of the temporomandibular joint (TMJ). While both conditions have been extensively studied independently, their interconnection remains a subject of debate. The aim of this work is to comprehensively review the current literature concerning spinal curvature abnormalities and stomatognathic alterations, concomitantly investig

Severity of premenstrual symptoms among women with musculoskeletal pain: relation to vitamin D, calcium, and psychological symptoms.

Premenstrual syndrome (PMS) has various symptoms that occur during the luteal phase of the menstrual cycle and subside after menstruation. Anxiety and depression are prevalent in women with PMS and may exacerbate the severity of PMS. Vitamin D and calcium deficiency may have a role in developing anxiety, depression, and musculoskeletal pain (MSP). The aim of this study was to evaluate selected premenstrual symptoms in relation to serum vitamin D levels, daily calcium consumption, and psychological symptoms among women with MSP. The study population consisted of 108 women with MSP and 108 healt

Increased serum 1,25-dihydroxyvitamin D levels in gynecologic cancer patients with Post-Acute-Covid-Sequela (PASC)/Long COVID.

Post-acute sequelae of COVID-19 (PASC), also known as Long-Covid (LC), may affect 10-30 % of COVID-infected patients, and is characterized by a variety of debilitating symptoms lasting over 3 months after the acute infection, including but not limited to dyspnea, fatigue, and musculoskeletal, cognitive, and/or mental health impairments. Vitamin D is an essential nutrient primarily recognized for its role in regulating calcium and bone health but also endowed with potent anti-inflammatory activity affecting a variety of immune cells. We retrospectively evaluated the plasmatic levels of both 1,2

Metformin alleviates osteoarthritis in mice by inhibiting chondrocyte ferroptosis and improving subchondral osteosclerosis and angiogenesis.

BACKGROUND: Osteoarthritis (OA) is the most common musculoskeletal disease, and it has a complex pathology and unknown pathogenesis. Chondrocyte ferroptosis is closely associated with the development of OA. As a common drug administered for the treatment of type 2 diabetes, metformin (Met) is known to inhibit the development of ferroptosis. However, its therapeutic effect in OA remains unknown. The present study aimed to explore the effects of Met on cartilage and subchondral bone in a mouse OA model and to explore the potential underlying mechanisms. METHODS: A mouse OA model was induced usi

Epidermal Neurite Density in Skin Biopsies From Patients With Juvenile Fibromyalgia.

OBJECTIVE: Fibromyalgia (FM) is defined by idiopathic, chronic, widespread musculoskeletal pain. In adults with FM, a metaanalysis of lower-leg skin biopsy demonstrated 45% pooled prevalence of abnormally low epidermal neurite density (END). END < 5th centile of the normal distribution is the consensus diagnostic threshold for small-fiber neuropathy. However, the clinical significance of END findings in FM is unknown. Here, we examine the prevalence of small-fiber pathology in juvenile FM, which has not been studied previously. METHODS: We screened 21 patients aged 13-20 years with FM diagnos

MRI changes of cervical spine in asymptomatic and symptomatic young adults.

Several work-related, psychosocial and individual factors have been verified as being related to neck and shoulder pain, but the role of pathology visualized by magnetic resonance imaging (MRI) remains unclear. In this study, the relationship between neck and shoulder pain and cervical high-field MRI findings was investigated in a sample of persons in a longitudinal survey. The study aimed to determine whether subjects with persistent or recurrent neck and shoulder pain were more likely to have abnormal MRI findings of cervical spine than those without neck and shoulder pain. A random sample o

Bi-allelic Variants in TONSL Cause SPONASTRIME Dysplasia and a Spectrum of Skeletal Dysplasia Phenotypes.

SPONASTRIME dysplasia is an autosomal-recessive spondyloepimetaphyseal dysplasia characterized by spine (spondylar) abnormalities, midface hypoplasia with a depressed nasal bridge, metaphyseal striations, and disproportionate short stature. Scoliosis, coxa vara, childhood cataracts, short dental roots, and hypogammaglobulinemia have also been reported in this disorder. Although an autosomal-recessive inheritance pattern has been hypothesized, pathogenic variants in a specific gene have not been discovered in individuals with SPONASTRIME dysplasia. Here, we identified bi-allelic variants in TON

A novel mutation in the matrix metallopeptidase 2 coding gene associated with intrafamilial variability of multicentric osteolysis, nodulosis, and arthropathy.

BACKGROUND: MONA, which stands for a spectrum of Multicentric Osteolysis, subcutaneous Nodulosis, and Athropathia, is an ultra rare autosomal recessive disorder caused by mutations in the matrix metallopeptidase 2 (MMP2) gene. To date only 44 individuals, carrying 22 different mutations have been reported. Here we report on two brothers with identical homozygous MMP2 gene mutations, but with clearly different phenotypes. METHODS: Genomic DNA was isolated from the affected brothers and the parents. An iliac crest bone biopsy was taken from the younger patient (index case). The level of matrix

Targeted therapy in patients with PIK3CA-related overgrowth syndrome.

CLOVES syndrome (congenital lipomatous overgrowth, vascular malformations, epidermal naevi, scoliosis/skeletal and spinal syndrome) is a genetic disorder that results from somatic, mosaic gain-of-function mutations of the PIK3CA gene, and belongs to the spectrum of PIK3CA-related overgrowth syndromes (PROS). This rare condition has no specific treatment and a poor survival rate. Here, we describe a postnatal mouse model of PROS/CLOVES that partially recapitulates the human disease, and demonstrate the efficacy of BYL719, an inhibitor of PIK3CA, in preventing and improving organ dysfunction. Understanding Chronic Hip Pain Through the ICF Framework: Implications for Assessment, Diagnosis, and Management.

Chronic hip pain is a prevalent musculoskeletal condition that can impair mobility, physical function, participation, and quality of life. Traditional assessment and management approaches often emphasize structural abnormalities and symptom control, but these approaches may not fully capture the broader functional, psychosocial, and contextual impact of chronic hip pain. The International Classification of Functioning, Disability and Health (ICF) provides a multidimensional framework for understanding health conditions through body structures and functions, activity and participation, and cont

Three-dimensional ultrashort echo time (3D UTE) MRI of Achilles tendon at 4.7T MRI with comparison to conventional sequences in an experimental murine model of spondyloarthropathy.

BACKGROUND: Due to the very short T2 of its components, the normal anatomy of Achilles enthesis is impossible to define with "conventional" long echo time (TE) T2 sequences. However, this is a common site affected by rheumatologic disease. Early abnormalities related to inflammatory processes are impossible to detect in this location. PURPOSE: To assess the feasibility of a 3D-UTE (ultrashort echo time) sequence to evaluate normal and pathological Achilles entheses, determining both anterior fibrocartilaginous and posterior collagenic portions at 4.7T, in a rat model of spondyloarthropathy (S

Clinical and laboratory features of canine Anaplasma platys infection in 32 naturally infected dogs in the Mediterranean basin.

Since the first description of Anaplasma platys Infection (ApI), the disease has been sporadically reported worldwide. Whereas it is considered a subclinical disease in the United States or in Australia, severe cases are reported in Europe. Thus far, little information is available regarding the clinical and laboratory findings associated with the disease and the implication of co-infections with other vector-borne pathogens (VBPs) in Southern Europe. The purpose of the study was to describe clinical and laboratory findings in PCR-confirmed naturally infected dogs in the Mediterranean Basin, a

Magnetic resonance imaging of the throwing elbow in the uninjured, high school-aged baseball pitcher.

BACKGROUND: Tissue adaptations in response to pitching are an expected finding during magnetic resonance imaging (MRI) evaluation of the throwing elbow of adult pitchers. These changes are considered normal in the absence of symptom complaints. It is unclear when during the playing career these tissue adaptations are initiated. HYPOTHESIS: Abnormalities in the appearance of the throwing elbow compared with the nonthrowing elbow would be visible during MRI assessment of this asymptomatic population of high school-aged throwers. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHO

Selective percutaneous myofasciotomy in persistent toe-walking: Anatomical rationale and surgical technique.

UNLABELLED: Persistent toe-walking is a gait abnormality characterized by forefoot initial contact and reduced or absent heel strike beyond early childhood. Although often regarded as a transient developmental variant, persistence of this pattern may lead to progressive limitation of ankle dorsiflexion, fixed equinus contracture, and long-term functional impairment. Toe-walking may occur as an isolated idiopathic condition or in association with neurological, neuromuscular, or genetic disorders. Increasing evidence suggests that a subset of patients previously classified as idiopathic harbor u

Congenital Muscular Torticollis as a Potential Neonatal Marker of Autism Spectrum Disorder and Other Neurodevelopmental Conditions.

BACKGROUND: Early intervention consistently improves developmental outcomes in children with autism spectrum disorder (ASD) and related conditions, underscoring the importance for timely recognition. Identifying early markers of atypical neurodevelopment is therefore critical. Congenital muscular torticollis (CMT), unilateral shortening of the sternocleidomastoid muscle causing infant head tilt, shares prenatal and perinatal risk factors with ASD and other developmental abnormalities. This overlap suggests that CMT may signal early disruption in neural development, potentially enabling earlier
