An integrative assessment of brain and body function 'at rest' in panic disorder: a combined quantitative EEG/autonomic function study.

We conducted a quantitative electroencephalographic (QEEG) and autonomic assessment of panic disorder (PD). The study samples comprised 52 individuals meeting DSM-IV criteria for PD (with or without agoraphobia) and 104 age-, gender-, and handedness-matched controls. EEG data were acquired from 16 scalp sites during resting eyes-open (REO) and eyes-closed (REC) conditions, and spectral power was assessed within 4 frequency bands: theta, alpha-1, alpha-2, and beta. The main findings were an overall reduction of spectral power in PD, compared to controls (Group main effect, p=.011), which was mo

Sensory contributions to impaired emotion processing in schizophrenia.

Both emotion and visual processing deficits are documented in schizophrenia, and preferential magnocellular visual pathway dysfunction has been reported in several studies. This study examined the contribution to emotion-processing deficits of magnocellular and parvocellular visual pathway function, based on stimulus properties and shape of contrast response functions. Experiment 1 examined the relationship between contrast sensitivity to magnocellular- and parvocellular-biased stimuli and emotion recognition using the Penn Emotion Recognition (ER-40) and Emotion Differentiation (EMODIFF) test

High fetal testosterone and sexually dimorphic cerebral networks in females.

Active masculinization by fetal testosterone is believed to be a major factor behind sex differentiation of the brain. We tested this hypothesis in a 15O-H2O positron emission tomography study of 11 women with congenital adrenal hyperplasia (CAH), a condition with high fetal testosterone, and 26 controls. Two indices of cerebral dimorphism were measured--functional connectivity and cerebral activation by 2 putative pheromones (androstadienone [AND] and estratetraenol [EST]), previously reported to activate the hypothalamic networks in a sex-differentiated manner. Smelling of unscented air was

ADHD and schizophrenia phenomenology: visual scanpaths to emotional faces as a potential psychophysiological marker?

Commonalities in the clinical phenomenology and psychopharmacology of ADHD and schizophrenia are reviewed. The potential of psychostimulants to produce psychotic symptoms emphasizes the need for objective psychophysiological distinctions between these disorders. Impaired emotion perception in both disorders is discussed. It is proposed that visual scanpaths to facial expressions of emotion might prove a potentially useful psychophysiological distinction between ADHD and schizophrenia. There is consistent evidence that both facial affect recognition and scanpaths to facial expressions are impai

Gamma-band activity over early sensory areas predicts detection of changes in audiovisual speech stimuli.

Oscillatory activity in the gamma-band range in human magneto- and electroencephalogram is thought to reflect the oscillatory synchronization of cortical networks. Findings of enhanced gamma-band activity (GBA) during cognitive processes like gestalt perception, attention and memory have led to the notion that GBA may reflect the activation of internal object representations. However, there is little direct evidence suggesting that GBA is related to subjective perceptual experience. In the present study, magnetoencephalogram was recorded during an audiovisual oddball paradigm with infrequent v

An epidemic of optic neuropathy in Tanzania: characterization of the visual disorder and associated peripheral neuropathy.

An epidemic of subacute bilateral visual failure has affected large numbers of teenagers and young adult Africans in coastal Tanzania since 1988. Previous work had indicated that many patients had sensory symptoms, but the nature of the neurological involvement was uncertain. The primary objective of this study was to characterize the accompanying neurological disorder. Furthermore, the nature of the visual loss was uncertain from previous reports as both retinopathy and optic neuropathy had been suggested. Full ophthalmic and neurological examinations were carried out at the Muhimbili Univers

Beyond Structural Pathology: Central Sensitization and Chronic Pain with Reference to Lumbar Disc Herniation-A Narrative Review.

Chronic pain is increasingly understood as a multidimensional condition in which, in a substantial subgroup of patients, a protective symptom can evolve into a persistent maladaptive disorder of the nervous system, while in others it may remain closely tied to ongoing mechanical or structural factors. Central sensitization (CS) represents a key mechanism underlying this transition, characterized by enhanced neural responsiveness and impaired endogenous pain inhibition, leading to a dissociation between pain and tissue pathology. The aim of this narrative review is to critically discuss current

Conditioned Pain Modulation Inter-Site Variability Study: Effect Sizes and Test-Retest Reliability of Two Models.

BACKGROUND: Conditioned pain modulation (CPM) is a set of psychophysical paradigms that is increasingly used clinically to evaluate descending pain modulation pathways. Impairment is common in chronic pain, suggesting CPM may serve as a mechanistic indicator. However, the lack of protocol standardization and reference data prevents clinical use in individual patients. METHODS: We compared two CPM protocols with different conditioning stimulus intensities, test stimulus types, and interaction timing. We assessed CPM effect size, test-retest reliability and sensitivity to detect loss of descend

Short-Term Sleep Disruption Does Not Modify Responses to Experimentally Induced Knee Pain in Healthy Individuals.

Background Knee osteoarthritis and poor sleep quality often co-occur and have a reciprocal relationship. Long-term exposure to poor sleep can facilitate pain, and short-lasting experimental sleep disturbances increase pain sensitivity. This study combined experimental knee pain and sleep disturbances to investigate whether short-term sleep disruption increases experimental knee pain intensity and if baseline traits affect this vulnerability. Methods Knee pain was induced using hypertonic saline (7%) injected into the infrapatellar fat pad (IFP) before and after three forced awakenings per nigh

Altered Pain Perception and Modulation in Individuals With Post-COVID-Condition: Insights From Quantitative Sensory Testing.

INTRODUCTION: Chronic pain is a significant and debilitating symptom observed in individuals with post-COVID condition (PCC), yet the underlying mechanisms remain poorly understood. This study aimed to investigate whether changes in psychophysical indicators of myofascial pain perception and modulation are present in individuals with PCC compared to symptom-free healthy controls (HC), and whether these changes correlate with the severity of clinical symptoms. METHODS: The study involved 84 individuals with PCC and 50 HC, assessing pain detection and tolerance thresholds (PDT and PTT), spatial

Point-of-care smell testing for evaluation of olfactory function: a narrative review of clinical utility.

IMPORTANCE: Olfactory dysfunction (OD) is increasingly recognized as a clinically relevant biomarker for a wide range of health conditions, including neurodegenerative diseases, respiratory infections such as COVID-19, psychiatric disorders, and age-related sensory decline. Despite its diagnostic value, olfactory testing remains underutilized in routine clinical care due to challenges related to test accessibility, cultural adaptability, cost and practical implementation. OBSERVATIONS: This narrative review provides a clinically focused synthesis of olfactory tests suitable for point-of-care

The association between obstructive sleep apnea and nocturnal enuresis in children: current advances and research trends.

BACKGROUND: Obstructive sleep apnea (OSA) and nocturnal enuresis (NE) represent two clinically prevalent pediatric disorders that frequently present as comorbidities. OSA, characterized by recurrent upper airway collapse during sleep, and NE, defined as involuntary voiding during sleep, collectively contribute to psychosocial distress in children. Emerging evidence suggests that OSA in children can cause NE through multiple mechanisms, though the pathophysiological interplay involving altered arousal thresholds and hormonal dysregulation remains incompletely elucidated. Herein, we summarized t

Modulating nociception networks: the impact of low-intensity focused ultrasound on thalamocortical connectivity.

Pain engages multiple brain networks, with the thalamus serving as a critical subcortical hub. This study aims to explore the effects of low-intensity transcranial focused ultrasound-induced suppression on the organization of thalamocortical nociceptive networks. We employed MR-guided focused ultrasound, a potential non-invasive therapy, with real-time ultrasound beam localization feedback and fMRI monitoring. We first functionally identified the focused ultrasound target at the thalamic ventroposterior lateral nucleus by mapping the whole-brain blood oxygenation level-dependent responses to n

From the latin "re-cordis, passing through the heart": autonomic modulation differentiates migraineurs from controls when recounting a significant life event.

OBJECTIVE: The literature on clinical psychophysiology highlights the possibility of using Heart Rate Variability (HRV) as an index of psychophysical balance and resilience to stress. This study investigates the differences in stress reactivity and subsequent recovery between a group of migraineurs and healthy controls. METHODS: Socio-demographic (i.e., sex, age, profession, marital status, and level of education) and psychophysiological (HR and HRV) measures of a group of thirty subjects with migraine (26 migraineurs without aura (86.7%), 2 migraineurs with aura (6.7%), and 2 migraineurs wit

Visual Performance of People With Albinism Assessed With Generalizable and Adaptive AIM and FInD Methods.

PURPOSE: People with albinism (PwA) are known to have visual impairments; however, little is known about whether these functions are disrupted across earlier and later stages of the visual pathway. We investigated distinct perceptual functions and fixation stability within each observer and compared the data with age- (±5 years) and sex-matched controls. METHODS: Twenty-one self-reported PwA and twenty-one controls were recruited. Angular-indication measurement (AIM) and foraging-interactive-D-prime (FInD) psychophysical methods were deployed to measure OS, OD, and OU near visual acuity, spat

Clinical application of the Swiss Liquid Jet Aesthesiometer for corneal sensitivity measurement.

CLINICAL RELEVANCE: Corneal sensitivity represents an important indicator for corneal health, its innervation and hence also for ocular disease. It is therefore of great interest from a clinical and research perspective to quantify ocular surface sensation. BACKGROUND: The aim of this prospective cross-sectional cohort study was to clinically test the within-day and day-to-day repeatability of the new Swiss Liquid Jet Aesthesiometer, employing small droplets of isotonic saline solution for repeatability, and correlate with the Cochet-Bonnet aesthesiometer in a cohort of participants of two di

Rehabilitative strategies after filtering procedure in glaucoma.

Glaucoma is one of the leading causes of non-reversible blindness worldwide, and almost 6 million people are estimated to be impaired visually in advanced stage of glaucoma. Recently, several studies on glaucoma has been focused towards new therapeutic approaches based on mechanisms independent from IOP control. Effects of new therapeutic agents, visual psychophysical training, or complementary medications targeting optic pathways today seem to be a relevant and effervescent field of research. The goal of the study is to evaluate in glaucoma patients if a rehabilitative strategy with a biofeed
