A novel technique for managing ureteroenteric strictures is robotic-assisted retroperitoneal laparoscopic reimplantation. A 63-year-old morbidly obese male underwent a left nephroureterectomy and cystoprostatectomy after neoadjuvant chemotherapy for transitional cell carcinoma of both the bladder and left kidney. His single right ureter was anastomosed to the ileal conduit. Postoperatively, he developed acute renal failure and hydronephrosis. An antegrade pyelogram demonstrated a distal stricture that failed two attempts at endoscopic management. In an effort to avoid the morbidity of an open

Robotic-assisted surgery has emerged as a new strategy for resection of thymomas. It may provide an option for patients who have had prior thoracic procedures, reducing the risks involved with another open procedure. We present a patient with a thymoma occurring after two prior sternotomies for cardiac procedures. A robotic-assisted thymectomy was performed successfully, with no complications. The minimally invasive approach of robotic-assisted resection of thymomas provides a safe alternative to redo sternotomy.

Improved treatments and rehabilitation for cerebral palsy (CP) have led to an increased number of children with CP surviving into adulthood. Adults with CP show an increased prevalence of pain, fatigue, and musculoskeletal dysfunction, leading to a decrease in ambulatory function. Recent work has demonstrated the potential benefits of intensive task-specific gait training, including the use of robotic-driven gait orthoses, on motor recovery in children with CP. In contrast, reports of interventions aimed at improving motor function in adults with CP are lacking. This case study reports on the

The transoral approach is an effective way to decompress the craniocervical junction due to basilar invagination. This approach has been described and refined, but significant limitations and technical challenges remain. Specifically, should the transoral route be used for intradural pathology, such as a meningioma, or should an inadvertent durotomy occur during extradural dissection, achieving a watertight closure of the dura in such a deep and narrow working channel is limited with the current microscopic and endoscopic techniques. Even closure of the posterior pharyngeal mucosa can be chall

BACKGROUND: Ischemic injury impacts renal function outcomes following partial nephrectomy. Efforts to minimize, better yet, eliminate renal ischemia are imperative.
OBJECTIVE: Describe a novel technique of "zero ischemia" laparoscopic (LPN) and robotic-assisted (RAPN) partial nephrectomy.
DESIGN, SETTING, AND PARTICIPANTS: Data were prospectively collected into an institutional review board-approved database. Fifteen consecutive patients underwent zero ischemia procedures: LPN (n=12), RAPN (n=3). Included were all candidates for LPN or RAPN, irrespective of tumor complexity, including tumors

BACKGROUND: This paper presents a novel robot designed for reduction of lower limb fractures, with the additional features of automatic controlled flexion of the knee joint, individual traction of thigh and leg, and foot rotation. The aim of this design is to assist the orthopaedic surgeon to perform better fracture reduction through motor control, in contrast to current manual control, and the results of assessments of its functions on normal subjects are presented in this paper.
METHODS: The robot was designed to be mounted onto the operation table, and was controlled through open switch re

BACKGROUND: Osteomyelitis associated with using synthetic mesh for laparoscopic sacral colpopexy is rare.
CASE: We present a patient who developed Staphylococcus sacral osteomyelitis after sacral colpopexy with synthetic mesh and titanium tack fixation to the sacral promontory in the absence of mesh erosion or fistula formation. The patient presented with low back pain 6 weeks postoperatively. Magnetic resonance imaging, bone aspirate, and culture confirmed sacral osteomyelitis and discitis 10 weeks after surgery. The patient underwent 8 weeks of outpatient antibiotic treatment. Six months af

Conventional surgical approaches for hypopharyngeal carcinomas have a great risk for developing treatment-related morbidity. To minimize this morbidity, hypopharyngectomy by transoral robotic surgery (TORS) was performed, and the efficacy and feasibility of this procedure were evaluated. TORS was performed using da Vinci Surgical Robot (Intuitive Surgical Inc., Sunnyvale, CA) in 10 patients with T1 or T2 pyriform sinus cancer and posterior pharyngeal wall cancer. FK retractor (Gyrus Medical Inc., Maple Grove, MN) was used for transoral exposure of the lesion. A face-up 30-degree endoscope was

BACKGROUND: Gangrenous cholecystitis remains a serious, life-threatening, surgical emergency. Surgeons are occasionally forced to perform a subtotal cholecystectomy and forego formal dissection of the cystic duct and artery, in an attempt to avoid iatrogenic injury to the common bile duct. Not infrequently, a patient may develop a persistent biliary fistula or symptomatic gall bladder remnant, which may ultimately require operative intervention.
METHODS: Three patients were referred to the University of North Carolina Hospitals for evaluation and treatment. All patients initially presented wi

OBJECTIVE: To describe our initial clinical experience of robotic single-port (RSP) surgery.
PATIENTS AND METHODS: The da Vinci S robot (Intuitive, Sunnyvale, CA, USA) was used to perform radical prostatectomy (RP), dismembered pyeloplasty, and radical nephrectomy. A robot 12-mm scope and 5-mm robotic grasper were introduced through a multichannel single port (R-port, Advanced Surgical Concepts, Dublin, Ireland). An additional 5-mm or 8-mm robotic port was introduced through the same umbilical incision (2 cm) alongside the multichannel port and used to introduce robotic instruments. Vesico-ur

Complete thymectomy (Thx) is a crucial part of treatment for myasthenia gravis (MG) and thymoma. The discussion about the necessity of radical, complete Thx and reduced invasiveness has led to no less than 14 different surgical approaches for Thx. The latest development is robotic-assisted surgery. Though its impact on minimally invasive surgery is not yet clear, it seems to be most promising for surgery in remote, narrow anatomical regions like the mediastinum. One hundred six consecutive robotic-assisted thymectomies (rThx) with the da Vinci robotic surgical system were performed between Jan

Posterior leaflet prolapse has been repaired traditionally by leaflet resection with or without a sliding annuloplasty. However, substantial annular calcification, thin leaflets, or deficient P1 or P3 scallops can complicate this technique. Annular closure after large posterior leaflet resection introduces substantial radial stress even in the presence of a sliding annuloplasty. We describe a novel technique that corrects posterior leaflet prolapse, minimizes leaflet resection, and preserves posterior leaflet-annulus continuity. This reconstructive technique can be applied in traditional mitra

Surgeons may occasionally encounter difficulty in visualizing the whole larynx with a direct laryngoscope. In such cases, rigid endoscopic laryngosurgery using a direct laryngoscope is an optimal solution. Multidirectional examination of the larynx using rigid endoscopes during direct laryngoscopy, leads to better control and management of the ventricle, inferior surface of the vocal fold and subglottis, and the anterior commissure. Currently, 0 degrees , 30 degrees , 70 degrees and 120 degrees angled rigid telescopes are used worldwide. Our experience in telescopic endolaryngeal surgery provi
