Tendon-bone insertion (TBI) injuries, such as anterior cruciate ligament injury and rotator cuff injury, are the most common soft tissue injuries. In most situations, surgical tendon/ligament reconstruction is necessary for treating such injuries. However, a significant number of cases failed because healing of the enthesis occurs through scar tissue formation rather than the regeneration of transitional tissue. In recent years, the therapeutic potential of mesenchymal stem cells (MSCs) has been well documented in animal and clinical studies, such as chronic paraplegia, non-ischemic heart fail

Function was evaluated in a 12-year-old girl who had traumatic, bilateral proximal humeral amputation at seven years of age. Unilateral successful replantation was performed with subsequent nerve grafts and tendon transfers to improve function. The contralateral side was fitted early after amputation with a body-powered prosthesis. Subjectively, the patient preferred the replanted side for activities of daily living. The range of motion and strength was better overall on the replanted side. Sensory return, although poor when compared with a normal hand, allowed the patient when blindfolded to …

BACKGROUND: The enthesis is a specialized transitional tissue that enables load transfer between tendon and bone through a graded tendon-fibrocartilage-bone interface. Following injury or surgical repair, regeneration of the native enthesis remains challenging, often resulting in mechanically inferior fibrous scar tissue. Additively manufactured three-dimensional (3D) scaffolds have emerged as a promising strategy to recreate the structural and biological complexity of the tendon-bone interface. This systematic review evaluated the current preclinical evidence on 3D scaffold-based approaches f

BACKGROUND: Obstetric brachial plexus palsy is caused by traction during birth. Most patients regain useful function with spontaneous recovery. In some cases, cross reinnervation occurs between the biceps and triceps muscles. In these cases, smooth active motion of the elbow joint is impaired by simultaneous biceps and triceps muscle contraction. The biceps and triceps muscle cocontraction could be treated by botulinum toxin type A injection, tendon transfer of the triceps to biceps, and intercostal nerves transfer to the musculocutaneous nerve (MCN) or to the motor branch of the radial nerve …

In established compartment syndrome discrimination between the different forms of flexion contracture, i.e., manifest Volkmann's contracture and intrinsic contracture is necessary. A combination of both is also possible. Classification is essential for determination of whether reconstruction is indicated and what procedure should be selected. Shortening osteotomies of the ulna and radius are now of historical interest only, as is carpalectomy. Lengthening of the flexor tendons is indicated only in mild and localized limited contracture of only some of the long fingers, but there is a danger of

Background: Wide-Awake Local Anaesthesia Without Tourniquet (WALANT), with the benefit of simplicity and a low complication rate, is a well-recognised procedure in orthopaedic surgery. To date, there is no report using WALANT technique with the Camitz transfer procedure. The aim of this study is to determine the early outcomes of using WALANT for Camitz transfer and compare these with outcomes of similar procedures in literature. Methods: Between January 2012 and January 2018, all patients with severe carpal tunnel syndrome (CTS) with thenar muscle atrophy who underwent the Camitz transfer pro

BACKGROUND: Major amputation for advanced soft tissue loss with bone and tendon exposure, can be prevented in diabetes patients with a combined arterial reconstruction and free-flap transfer. We reviewed our 15-year outcome and evaluated the feasibility to save diabetic feet by means of this aggressive strategy.
METHODS: A total of 55 type II diabetes patients (42-80 years of age), hospitalized between January 1992 and December 2006 for a combined arterial reconstruction and free-flap transfer, were followed until December 2007. All would have otherwise required at least a below-knee amputati

BACKGROUND: A child with recurrent or incompletely corrected clubfoot after previous extensive soft tissue release is treated frequently with revision surgery. This leads to further scarring, pain and limitations in range of motion. We have utilized the Ponseti method of manipulation and casting and when indicated, tibialis anterior tendon transfer, instead of revision surgery for these cases.
MATERIALS AND METHODS: A retrospective review of all children treated since 2002 (n = 11) at our institution for recurrent or incompletely corrected clubfoot after previous extensive soft tissue release…

BACKGROUND: We studied the efficacy of medial patellofemoral ligament (MPFL) reconstruction, with or without tibial tubercle (TT) transfer, for recurrent dislocation of the patella, based on subjective functional assessment using the visual analog scale (VAS).
METHODS: Forty-two consecutive knees of 40 patients who underwent MPFL reconstruction using a hamstring tendon were followed up for an average of 4.3 years. Twenty-nine knees underwent MPFL reconstruction (MPFL group) and 13 knees underwent MPFL reconstruction combined with medial transfer of TT (TT+MPFL group). Clinical results, includ

UNLABELLED: Constraint-induced movement therapy (CIMT) was originally implemented to address limb non-use in stroke recovery, and has more recently become a mainstay, evidence-based treatment approach to decrease developmental disregard in children. In addition to combating developmental disregard in the BPBI patient with residual deficit(s), CIMT can be applied in various ways across diverse cases in brachial plexus practice, including post-surgical use to aid in the activation of nerve transfers and/or tendon transfers. The timing, dosage, and setting of CIMT will vary depending on the patie…

Thirty-three feet in 29 patients with metatarsalgia were reviewed after Jones transfer to the lesser rays to evaluate the long-term results and the indications for these procedures. Transfer of the long extensor tendons to their respective metatarsal necks and fusion of the interphalangeal joints with shortening of the toe were performed. The procedures were performed in patients with symptoms of metatarsalgia, secondary to anterior pes cavus deformity (clawfeet, 16 patients), and in patients with mild or severe spreadfoot deformity with fall of the central metatarsals (12 patients). Fifty-eight children or adolescents with cerebral palsy who presented an equino varus deformity of the foot were operated on between the age of 4 to 21 by tibialis posterior tenotomy (23 cases), tibialis posterior transposition (18 cases), tibialis posterior (10 cases) or anterior (6 cases) transfer on the cuboid, with triceps lengthening (37 cases). 56 adolescents were reviewed, 45 had achieved their growth. The foot was painless and weight bearing improved. 15 failures were caused by overcorrection in valgus (7 cases, in 5 of which the tendon was transferred on the cuboid) or under correctio

Shoulder pain in the context of rotator cuff disorders is a frequent source of medical consultation. A wide range of therapeutic options is reported in the literature. Non- or minimally invasive treatments include physiotherapy, anti-inflammatory medication, and infiltration of corticosteroids or platelet-rich plasma. Surgical treatments include subacromial debridement, long head of the biceps tenotomy/tenodesis, rotator cuff repair, superior capsular reconstruction, and tendon transfers. Reverse shoulder arthroplasty completes the therapeutic arsenal. Guiding and advising the patient in the f…

In patients with brachial plexus birth palsy, persistent muscular imbalance across the developing shoulder results in progressive glenohumeral dysplasia, characterized by increased glenoid retroversion, humeral head flattening, and posterior subluxation of the humeral head. Soft-tissue procedures-such as tendon transfers and musculotendinous lengthenings--will provide limited functional improvements in the setting of advanced glenohumeral deformity. For patients with internal rotation contracture and external rotation weakness associated with severe glenohumeral dysplasia, external rotation os…

A method of surgical correction is described for Wartenberg's sign, or persistent abduction of the little finger, using a slip of the extensor digitorum communis of the ring finger. The transferred component can be either the central slip, or the ulnar slip extended by the connexus intertendineus to the little finger. This technique maintains the integrity of the extensor mechanism of the little finger, avoiding loss of active extension, which is frequently observed when extensor digiti minimi is used. The donor site is dependable since the extensor digitorum communis tendon of the ring finger
