Outcomes of patients with cardiogenic shock and atrial fibrillation receiving pulmonary vein isolation or AV node ablation and cardiac resynchronization.

BACKGROUND: The safety and tolerability of early invasive therapies for rapid atrial fibrillation in patients with cardiogenic shock is yet to be established. METHOD: This retrospective study reviewed patients with diagnosis of atrial fibrillation and cardiogenic shock during the same hospital admission between 2019 and 2024. These patients were dependent on inotropes and/or mechanical circulatory support and received PVI or AVJ ablation with CRT implantation during the index hospitalization. Outcomes measured were procedural complications, post-procedural heart failure or Afib hospitalizatio

Nonpharmacologic rate control of postoperative atrial fibrillation in the canine sterile pericarditis model.

INTRODUCTION: Postoperative atrial fibrillation (POAF) is common following open heart surgery, and is associated with significant morbidity. Medications used for ventricular rate control of POAF may not be effective in controlling rapid ventricular rates during the postoperative period because of increased sympathetic tone. The purpose of this study was to develop nonpharmacologic rate control of POAF by atrioventricular node (AVN) fat pad stimulation using clinically available temporary pacing wires in the canine sterile pericarditis model. METHODS: We studied 10 sterile pericarditis dogs in…

[Pacemaker ECG Quiz No. 23: Pacemaker malfunction or arrhythmia?].

In a 69-year-old patient, a DDDR pacemaker was implanted for sinus node disease with syncope due to sinus arrest with pauses > 4 s. Programming was performed carefully including measures to prevent unnecessary ventricular pacing. Approximately 6 months after implantation, the patient who was asymptomatic in the meantime presented for pacemaker control because the ECG had raised the question of pacemaker malfunction.

The angle of the components of the common atrioventricular valve predicts the outcome of surgical correction in patients with atrioventricular septal defect and common atrioventricular junction.

BACKGROUND: Three-dimensional echocardiography offers new insights into valvar function in atrioventricular septal defects (AVSDs). The aim of this study was to identify a morphological marker to predict the functional outcomes of left atrioventricular valves (AVVs) following the repair of AVSDs. METHODS: Twenty-nine consecutive patients were evaluated preoperatively using 2-dimensional and 3-dimensional echocardiography. The angle of the AVV relative to the crux of the heart was measured in multiplanar review mode. RESULTS: The severity of postoperative left AVV regurgitation was correlated

A perspective on atrioventricular delay optimization in patients with a dual chamber pacemaker.

Atrioventricular delay (AVD) is critical in patients with DDD pacemakers (PM). Echo/Doppler evaluation of AVD providing the longest left ventricular filling time (FT) or the highest cardiac output (CO) is used for AVD optimization. Recently myocardial performance index (MPI) has been shown to improve by optimizing AVD. The aim was to compare the CO, FT, MPI derived optimal AVD, and to analyze systolic and diastolic performance at every optimal AVD. Twenty-five patients, 16 men 68 +/- 11 years, ejection fraction >or= 50%, with a DDD PM for third-degree AV block, without other major cardiomyopat

Autonomic blockade unmasks maturational differences in rate-dependent atrioventricular nodal conduction and facilitation in the mouse.

INTRODUCTION: In large animals, rate-dependent AV nodal (AVN) properties of conduction are modulated by autonomic inputs. In this study, we investigated whether the properties of AVN conduction and facilitation are altered by autonomic blockade in the mouse and whether this effect is age dependent. METHODS AND RESULTS: Young (age 4-6 weeks; n = 11) and adult (age 8-9 months; n = 11) female mice underwent in vivo electrophysiologic testing, before and after autonomic blockade. After autonomic blockade, the adult mice had significantly longer AVN effective refractory period (AVNERP; 67 +/- 14 m

[Infectious mitral endocarditis after radiofrequency catheter ablation of a left lateral accessory pathway].

A 2-years-old child with Wolff-Parkinson-White syndrome associated with life-threatening symptoms underwent radiofrequency ablation of a left lateral accessory pathway. A deflectable 5F bipolar electrode catheter positioned above the atrioventricular groove by transeptal approach was used for ablation. The catheters were repeatedly used after ethylene oxide sterilisation. Although immediate post-ablation echocardiography demonstrated no complications, the patient was readmitted two days later with fever and a new mitral murmur. Penicillin-susceptible Staphylococcus aureus was isolated and intr

Effects of intracardiac radiofrequency current application on coronary artery vessels in young pigs.

INTRODUCTION: Radiofrequency current is widely used in children to ablate accessory AV pathways. Previous data in a pig model demonstrated coronary artery obstruction adjacent to radiofrequency current lesions 48 hours and 6 months after energy delivery. In the present study, the long-term effects (>6 months) of radiofrequency current application on coronary artery vessels in young pigs are assessed. METHODS AND RESULTS: Radiofrequency current (500 kHz) was delivered over 30 seconds in ten piglets (mean body weight 12.8 kg) using a steerable 6-French catheter with a 4-mm thermistor tip electr

The effect of maximum heart rate on oxygen kinetics and exercise performance at low and high workloads.

The normal heart rate is linearly related to oxygen consumption during exercise. The maximum heart rate of the normal sinus node is approximated by the formula: HRmax = (220-age) with a variance of approximately 15%. However, the nominal upper rate of most permanent pacemakers is 120 beats/min, a value that remains unchanged for many patients. As this nominal setting falls well below the maximum predicted heart rate for most patients, it is possible that the chronotropic response of rate adaptive pacemakers during moderate and maximal exercise workloads may be less than optimal. The purpose of

Effects of various doses of intracoronary verapamil on coronary resistance vessels in humans.

To investigate the vasodilatory effect of various doses of intracoronary verapamil on coronary resistance vessels, we studied 13 patients with normal angiograms. A coronary Doppler guide wire was inserted into the left anterior descending coronary artery, and coronary blood flow velocity (CBFV) was measured. Verapamil was injected into the left coronary artery at doses of 0.1 mg, 0.5 mg, 1.0 mg, and 2.0 mg at 10-min intervals. Nitroglycerin was also injected into the same artery to avoid changes in cross-sectional area. As a measure of coronary vascular resistance, coronary vascular resistance

Microwave ablation of the atrioventricular junction in open-chest dogs.

The use of microwave energy for ablation of the atrioventricular (AV) junction was examined in open-chest dogs. Using a specially designed microwave catheter and a 2450 MHz generator, microwave energy was delivered to the AV junction according to one of two protocols. In protocol 1, increasing amounts of energy were delivered until irreversible AV block occurred. In protocol 2, only two applications of energy were used, one at low energy and the other at an energy found to be high enough to cause irreversible AV block. Each dog received between one and six applications of microwave energy. The

Choosing the anesthetic and sedative drugs for supraventricular tachycardia ablations: A focused review.

This study provides a review of the contemporary literature for the effects of most commonly used anesthetic drugs for sedation and anesthesia during adult electrophysiologic (EP) studies where supraventricular tachycardias (SVT) need to be induced for diagnostic purposes and/or catheter ablation. Some medications may affect cardiac electrophysiology and conduction, altering the ability to induce the arrhythmia, and may have negative impact on mapping and ablation treatment. The objective of the study is to determine the best sedative choice during SVT ablations. The authors searched MEDLINE,

[Analysis of changes in repolarization for the differential diagnosis of narrow QRS supraventricular tachycardia and the site of the accessory pathway].

In view of the growing role of catheter ablation techniques for the treatment of supraventricular tachycardia, noninvasive determination of tachycardia mechanism and preliminary localization of the accessory pathway (AP) can simplify the cardiac catheterization procedure and reduce fluoroscopic exposure. The purpose of this study was to analyze the diagnostic value of repolarization changes during narrow QRS complex tachycardia (< 0.11 s). In 159 12-lead electrocardiograms during narrow QRS complex tachycardia (13 atrial tachycardias, 57 atrioventricular (AV) node reentrant tachycardias and 89

Value of oesophageal pacing in evaluation of atrial arrhythmias.

The study of 'atrial vulnerability' is often clinically indicated but it requires the use of invasive intracardiac stimulation. The purpose of the study was to assess the use of oesophageal pacing in the evaluation of atrial tachyarrhythmias (ATA). Fifty-five patients with documented ATA (group I) and 60 without (group II) were studied. The protocol of oesophageal pacing consisted of atrial pacing up to the second-degree AV block and programmed stimulation in the control state and after isoproterenol infusion. ATA was induced in 47 group I patients (85%) either in the control state (n = 27) or

Late ventricular standstill following an elective TAVI.

Transcatheter aortic valve implantations (TAVIs) may be complicated by a need for permanent pacemaker implantation post procedure, usually due to local trauma or compression on the conduction system. There are some features that might help predict that a patient is high risk for developing conduction disease following TAVI, for example, underlying right bundle branch block or use of certain types of TAVI. It might also become apparent during the procedure, or before temporary wire removal post procedure. Higher risk patients may undergo rhythm monitoring for longer periods post TAVI. We presen

Better ventricular synchronization via an accessory pathway.

We report of a patient in whom the presence of a left ventricular accessory pathway plus enhanced AV nodal conduction led to a more physiological ventricular activation sequence than that observed after ablation of the accessory pathway in a patient with baseline left bundle branch block and concealed intra-QRS pre-excitation.
