Cryoablation of the accessory pathway in Wolff-Parkinson-White syndrome: initial results and long term follow up.

Cryoablation of the accessory pathway was used in the management of 20 patients with pre-excitation syndromes. All patients had presented with paroxysmal atrioventricular reentrant tachycardia; in addition, six had experienced atrial fibrillation. In 16 patients pre-excitation was overt and in four the accessory pathway was concealed. Intraoperative epicardial and endocardial mapping showed 10 left free wall pathways, seven septal pathways, and four right free wall pathways. One patient had two right free wall accessory pathways. There was one postoperative death (from a ruptured cerebral haem

Focal vulvitis: a characteristic syndrome and cause of dyspareunia. Features, natural history, and management.

Based on the study of 67 affected women during a period of 15 years, we report the clinical features and natural history of focal vulvitis, a unique syndrome characterized by severe and persistent superficial dyspareunia and the presence of one to 11 (median three) minute, exquisitely tender areas of focal inflammation or ulceration on the mucosa of the vestibule. Three fourths of all lesions occur around the Bartholin gland ducts or between them posteriorly. Histopathologic study of tissues from seven patients has not shown a characteristic pattern of inflammation and fails to confirm a repor

Ultra-Low Cryoablation for Scar-Related VT Ablation: Results From the US Early Feasibility Study.

BACKGROUND: A limitation of ablation for scar-related ventricular tachycardia (VT) is insufficient lesion depth to address nonendocardial substrate. Ultra-low temperature cryoablation (ULTC) at -196°C has been shown to create transmural lesions in preclinical models. Early human studies in Europe have shown safety and efficacy. METHODS: An EFS (Early Feasibility Study) was designed in collaboration with the Food and Drug Administration as a prospective, nonrandomized evaluation of the acute safety and effectiveness of ULTC ablation for scar-related VT. RESULTS: Twenty patients (age 63±14 yea

A Preoperative Risk Stratification Approach Using the Focal Risk Group (FRG) in Patients Undergoing Focal Therapy for Localized Prostate Cancer.

INTRODUCTION: Focal therapy is increasingly used for localized prostate cancer, but preoperative risk stratification remains poorly defined. We developed a Focal Risk Group (FRG), a simple clinically informed classification based on commonly available variables, to describe baseline risk of biochemical failure after focal therapy. PATIENTS: We analyzed 324 consecutive patients treated with cryoablation or high-intensity focused ultrasound at a high-volume center. METHODS: The FRG included PSA density, ISUP grade group, dominant lesion size, and clinical stage. Patients were classified as low

Autonomic effects and ablation success after pulsed field and cryoballoon ablation: a SINGLE SHOT CHAMPION substudy.

AIMS: Pulmonary vein isolation (PVI) using radiofrequency and cryoballoon ablation (CBA) modulates the cardiac autonomic nervous system (ANS) by affecting epicardial ganglionated plexi. In contrast, pulsed field ablation (PFA) appears to exert minimal autonomic effects. The impact of cardiac ANS modulation on arrhythmia recurrence remains incompletely understood. METHODS AND RESULTS: In this multicentre trial substudy, 210 patients with symptomatic paroxysmal atrial fibrillation (AF) were randomized 1:1 to PVI using PFA or CBA. All patients received an implantable cardiac monitor at the time

Comparison of 1.1-mm and 1.7-mm Probes in Transbronchial Mediastinal Cryobiopsy: A Randomized Trial.

INTRODUCTION: Recent studies have shown that combined use of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and cryobiopsy provides a higher diagnostic yield for mediastinal lesions as compared with EBUS-TBNA alone. Currently, cryoprobes with diameters of 1.1mm and 1.7mm have been reported to be used for mediastinal cryobiopsy. METHODS: We conducted a randomized controlled trial to evaluate and compare the diagnostic safety and efficacy profile the two cryoprobes in mediastinal disease. Consecutive patients with mediastinal lesions (≥1cm in the short axis) were p

Long-Term Outcomes of Cryoballoon Pulmonary Vein Isolation Freeze Using a Modified ICE-T Protocol With a Double 120 s Applications (ICE-T 2 × 2).

BACKGROUND: Cryoballoon (CB) pulmonary vein isolation (PVI) is a well-established treatment for symptomatic atrial fibrillation (AF). Most centers use application durations of 180 or 240 s. Since freezing-related complications tend to occur later during the application, empirically shortening the duration may reduce procedural risks. A protocol using two consecutive 120-s freezes was proposed. Here, we report its long-term outcomes. METHODS: We performed a long-term follow-up comparison of patients with symptomatic AF undergoing PVI with second-generation CB between 2019 and 2020. The convent

One-Year Success Rates of a Stable, Low Pressure Cryoballoon for the Treatment of Paroxysmal Atrial Fibrillation: Results of the Prospective, International, Multicenter POLAR ICE Study.

INTRODUCTION: Pulmonary vein isolation (PVI) using a cryoballoon is well-established for the treatment of paroxysmal atrial fibrillation (PAF). Compared to other available technologies, the usage of a stable, low-pressure cryoballoon (POLARx, Boston Scientific) has demonstrated lower nadir temperatures and longer thawing times. However, 1-year efficacy and safety still needs to be proven. The aim of the POLAR ICE Study was to evaluate 1-year efficacy and safety outcomes of cryoballoon ablation in patients with paroxysmal atrial fibrillation. METHODS: POLAR ICE, a prospective, non-randomized,

Safety and Effectiveness of Track Cauterization for Lung Cryoablation.

PURPOSE: To evaluate the safety and effectiveness of track cauterization for lung cryoablation through comparison of postprocedural adverse event (AE) rates. MATERIALS AND METHODS: Fifty-nine patients who underwent 164 percutaneous lung cryoablation procedures between 2013 and 2018 were included in this retrospective study. The study cohort was subdivided by whether track cauterization was conducted or not at the end of the procedure. The study cohort was also subdivided by the number of probes (1-2 probes vs 3-4 probes). Postablation AE rates were assessed by immediate and delayed (at 1 mont

Comparison of Postprocedural P-Wave Vector Magnitude on 12-Lead Electrocardiogram Between Cryoballoon and Radiofrequency Ablation.

Pulmonary vein isolation (PVI) causes changes in P-wave parameters. However, the difference in changes in P-wave parameters including P-wave vector magnitude (Pvm) between radiofrequency catheter ablation (RFCA) and cryoballoon ablation (CBA) remains unknown. Paroxysmal atrial fibrillation (PAF) patients who underwent only PVI were enrolled. Pvm was calculated by the square root of the sum of the squared P-wave amplitude in leads II and V6 and one-half of the P-wave amplitude in V2. The patients were divided into 2 groups: RFCA and CBA. ΔPvm was calculated as ΔPvm (mV) = (Pvm at pre-PVI)-(Pvm
