Central venous access device management for children undergoing treatment for blood disorders and cancer: a descriptive international cross-sectional survey.

PURPOSE: This study aimed to explore current central vascular access device (CVAD) management practices in pediatric cancer care globally and compare practices across high-income and middle-income (i.e., upper middle-income and lower middle-income) countries. METHODS: A descriptive cross-sectional international survey was conducted between May 2022 and December 2023. Clinicians involved in CVAD insertion/management for pediatric cancer patients were invited to participate through established clinical networks globally. The survey covered CVAD management practices, including dressings, securem

Pandemic preparedness and response: a survey among experts from high- and low-middle-income countries about the "100 Days Mission".

BACKGROUND: The "100 Days Mission" (100DM), designed by the Coalition for Epidemic Preparedness Innovations (CEPI), represents an ambitious new concept in vaccine development for effective pandemic preparedness, rapid response, and the reduction of health inequalities. We aimed to identify potential obstacles to the success of the 100DM by conducting a survey among experts in vaccinology and public health from both high-income countries (HICs) and low- and middle-income countries (LMICs). MATERIALS AND METHODS: A descriptive cross-sectional study was conducted, using a semi-structured online

Global variations in funding and use of hemodialysis accesses: an international report using the ISN Global Kidney Health Atlas.

BACKGROUND: There is a lack of contemporary data describing global variations in vascular access for hemodialysis (HD). We used the third iteration of the International Society of Nephrology Global Kidney Health Atlas (ISN-GKHA) to highlight differences in funding and availability of hemodialysis accesses used for initiating HD across world regions. METHODS: Survey questions were directed at understanding the funding modules for obtaining vascular access and types of accesses used to initiate dialysis. An electronic survey was sent to national and regional key stakeholders affiliated with the

Global variation in postoperative mortality and complications after cancer surgery: a multicentre, prospective cohort study in 82 countries.

BACKGROUND: 80% of individuals with cancer will require a surgical procedure, yet little comparative data exist on early outcomes in low-income and middle-income countries (LMICs). We compared postoperative outcomes in breast, colorectal, and gastric cancer surgery in hospitals worldwide, focusing on the effect of disease stage and complications on postoperative mortality. METHODS: This was a multicentre, international prospective cohort study of consecutive adult patients undergoing surgery for primary breast, colorectal, or gastric cancer requiring a skin incision done under general or neur

Socioeconomic status and risk of cardiovascular disease in 20 low-income, middle-income, and high-income countries: the Prospective Urban Rural Epidemiologic (PURE) study.

BACKGROUND: Socioeconomic status is associated with differences in risk factors for cardiovascular disease incidence and outcomes, including mortality. However, it is unclear whether the associations between cardiovascular disease and common measures of socioeconomic status-wealth and education-differ among high-income, middle-income, and low-income countries, and, if so, why these differences exist. We explored the association between education and household wealth and cardiovascular disease and mortality to assess which marker is the stronger predictor of outcomes, and examined whether any d

The cost and public health burden of invasive meningococcal disease outbreaks: a systematic review.

BACKGROUND: Invasive meningococcal disease (IMD) is a serious disease with a rapid onset, high mortality rate, and risk of long-term complications. Numerous reports in the literature conclude that IMD outbreaks are associated with substantial costs to society and significant burden on communities due to the cost associated with the prevention of secondary cases. OBJECTIVE: To systematically review the literature on the costs and public health burden associated with IMD outbreaks. METHODS: Studies were primarily identified through searching MEDLINE and EMBASE. Reports were included if they pr

Commentary: reducing the world's stillbirths.

One of the major success stories of modern obstetrics in high-income countries in the last 5 decades is the reduction of stillbirths from rates as high as 50 per 1000 births to about 5 per 1000 births today. Fetal mortality associated with obstructed labour, asphyxia, hypertension, diabetes, Rh disease, placental abruption, post-term pregnancies and infections such as syphilis all have declined. Much of this success has occurred in term births in the intrapartum period so that most stillbirths in high-income countries now occur in the antepartum period and are pre-term. Current stillbirth rate

Epidemiological and clinical characteristics of hookworm-related cutaneous larva migrans.

Hookworm-related cutaneous larva migrans is caused by the migration of animal hookworm larvae in the human skin. The disease mainly occurs in resource-poor communities in the developing world, but it is also reported sporadically in high-income countries and in tourists who have visited the tropics. Diagnosis is made clinically in the presence of a linear serpiginous track moving forward in the skin, associated with itching and a history of exposure. Itching is typically very intense and can prevent patients from sleeping. Bacterial superinfection occurs as a result of scratching. Treatment is

Disease eradication as a public health strategy: a case study of poliomyelitis eradication.

Disease eradication as a public health strategy was discussed at international meetings in 1997 and 1998. In this article, the ongoing poliomyelitis eradication initiative is examined using the criteria for evaluating candidate diseases for eradication proposed at these meetings, which covered costs and benefits, biological determinants of eradicability (technical feasibility) and societal and political considerations (operational feasibility). The benefits of poliomyelitis eradication are shown to include a substantial investment in health services delivery, the elimination of a major cause o

Global progress in infectious disease control.

There is both good news and bad news concerning infectious disease control globally. The good news is that smallpox has been eradicated, eradication of poliomyelitis and guinea worm disease is on track, and many infectious diseases are under effective control in much of the world. The advances are primarily the result of improved sanitation, effective use of vaccines, and introduction and use of specific therapies (whose impact has primarily been on mortality, rather than incidence). The bad news is that infectious diseases are still the leading cause of death world-wide, new diseases are emer

Beyond the first call: unveiling the gray areas in reactive donor notification and counseling in lower-middle-income countries-a systematic scoping review.

INTRODUCTION: Donor notification and counseling after reactive screening for transfusion-transmitted infections (TTIs) are critical to transfusion safety and public health, yet practices in low- and middle-income countries (LMIC) remain inconsistent and poorly evaluated. METHODS: A Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for scoping reviews-guided scoping review was conducted to map evidence on TTI-reactive donor notification and follow-up in LMIC. Whose crisis is it? Local experiences and health governance in the era of polycrisis.

The concept of polycrisis has gained traction to describe the interconnected shocks-pandemics, climate-related disasters, conflict and economic instability-shaping global risk. Yet for many communities in low-income settings, such entanglements are not new but reflect long-standing conditions of permacrisis. We advance a situated perspective that examines how global shocks intersect with pre-existing local vulnerabilities, drawing on examples from Somalia, northern Kenya, Zimbabwe and Lebanon. These cases show that crises are experienced unevenly and simultaneously across systems of food, wate

Noninvasive Medical Devices in Low-Income and Complex Settings: Promise, Challenge, and the Path Ahead.

While noninvasive medical devices for disease diagnosis and management are used routinely in high-income settings, their penetration in low-income countries, and in complex emergency and humanitarian settings remain limited. This article discusses issues of trust, privacy, context, and financial sustainability that need to be addressed for noninvasive devices to live up to their potential and promise in low-income and humanitarian settings.

Significance, applications, and contributions of Gamma Knife radiosurgery in advancing neurosurgical care in lower-middle-income countries: a systematic review.

OBJECTIVE: Gamma Knife radiosurgery (GKRS) is an important neurosurgical intervention most commonly used for pathologies such as meningiomas, acoustic neuromas, paragangliomas, arteriovenous malformations, and trigeminal neuralgia. While the widespread availability of GKRS in developed countries makes it ubiquitous in neurosurgical research, there is a huge disparity in research in lower-middle-income countries (LMICs). This study aimed to quantify research undertaken by authors in LMICs on GKRS. METHODS: The authors systematically reviewed PubMed for research articles that reported on GKRS f
