The impact of helicopter emergency medical services and craniocervical traction on the early reduction of cervical spine dislocation in a rural area of Japan.

BACKGROUND: Several studies have shown an association between achieving decompression of the spinal cord within a few hours and neurological recovery, even in patients with complete paralysis due to cervical spine dislocation. This study aimed to clarify the impact of helicopter emergency medical services (HEMSs) and craniocervical traction on the rapid reduction of lower cervical spine dislocation in rural Japan. METHODS: The success rate of and factors inhibiting closed reduction, the time from injury to reduction and the functional prognosis of lower cervical spine dislocations treated bet

Thoracostomy in children with severe trauma: An overview of the paediatric experience in Victoria, Australia.

OBJECTIVE: Thoracic trauma is a leading cause of paediatric trauma deaths. Traumatic cardiac arrest, tension pneumothorax and massive haemothorax are life-threatening conditions requiring emergency and definitive pleural decompression. In adults, thoracostomy is increasingly preferred over needle thoracocentesis for emergency pleural decompression. The present study reports on the early experience of thoracostomy in children, to inform debate regarding the best approach for emergency pleural compression in paediatric trauma. METHODS: Retrospective review of Ambulance Victoria and The Royal Ch

A case of intra-arterial thrombolysis with alteplase in a patient with hypothenar hammer syndrome but without underlying aneurysm.

Hypothenar hammer syndrome is a cause of symptomatic ischemia of the hand secondary to the formation of aneurysm or thrombosis of the ulnar artery in the setting of a complete or incomplete palmar arch. Acute occlusive thrombus or embolus of the hand represents a complex problem that often may require immediate surgical intervention. We report a case of acute unilateral arterial hand ischemia requiring catheter-directed thrombolysis with Alteplase therapy in a patient with acute occlusive arterial thrombosis of the left ulnar artery. A catheter-directed thrombolytic regimen consisted of Altepl

Minimally Invasive Scoliosis Surgery with Oblique Lateral Lumbar Interbody Fusion: Single Surgeon Feasibility Study.

BackgroundDegenerative deformities of the spine have traditionally been treated with extensive open surgeries. However, these open procedures are associated with a high degree of surgical morbidity. In this study, we explore whether clinical improvement in patients with spinal deformities can be achieved using a new minimally invasive surgery (MIS) called oblique lateral lumbar interbody fusion (OLLIF). OLLIF is a MIS single surgeon procedure in which the disc is approached through Kambin's triangle. OLLIF can achieve correction of spinal deformities through careful cage placement.PurposeThe p

[Neurological prognosis of floppy infants after health examinations].

OBJECTIVE: The term benign congenital hypotonia is retrospective and refers to infants who are hypotonic at birth or shortly thereafter but later show a normal tone. It encompasses many different pathological processes that affect the brain, motor unit, or both. The majority of affected children have cerebral hypotonia. An increased incidence of mental retardation, learning disabilities, and other sequelae of cerebral abnormality are evident later in life, despite the recovery of a normal muscle tone. We followed floppy infants who were pointed out as showing motor delay on health examinations

Vertebroplasty for pain relief and spinal stabilization in multiple myeloma.

Mechanical stabilization of oncological vertebral fractures with cement augmentation is the first mechanism of pain relief, with or without restoration of vertebral body height. The aim of this study was to assess the safety and efficacy of vertebroplasty for painful vertebral body fractures in patients with multiple myeloma, in each phase of the disease. The authors reviewed a consecutive group of patients with multiple myeloma who underwent vertebroplasty at our Institute between November 2003 and December 2005. Twenty-eight levels were performed on 11 patients during 14 treatment sessions.

Trauma Patient Survival Times in a Resource-Limited South African Trauma System: Implications for Combat Casualty Evacuation Timelines.

INTRODUCTION: Future United States military conflicts may involve near-peer adversaries and large-scale combat operations resulting in mass casualties with delayed evacuation. Health service support planning for such conflicts will require data informing patterns of survival among injured combatants. We determined the probability of survival to 24 hours post injury for trauma patients with blunt or penetrating non-compressible torso hemorrhage (NCTH) in a relatively resource-constrained environment. We hypothesized that the conditional probability of 24-hour survival would rise with survival t

Automated video-based AVPU assessment within a FHIR-enabled clinical decision support framework.

Accurate and timely assessment of consciousness is critical for triage, escalation of care, and patient safety in emergency and hospital settings. However, documentation using the AVPU scale (Alert, Verbal, Pain, Unresponsive) remains inconsistent owing to high workload, subjectivity, and fragmented workflows. This study developed and evaluated Consc.ia, a video-based clinical decision-support platform that automates AVPU inference while preserving clinician oversight and enabling seamless, interoperable documentation through HL7 FHIR. A simulated AVPU dataset comprising 136 videos from 58 hea

Surgical Management of Unstable Trochanteric Fractures of the Femur Using Cemented Hemi-Prosthesis in Elderly Patients: A Prospective Study.

Background In the case of elderly patients suffering from osteoporosis, the primary objectives of addressing comminuted intertrochanteric fractures are centered upon the recuperation of the patients' pre-fracture levels of activity, the expeditious promotion of full weight-bearing capacity, and the minimization of the likelihood of further surgical interventions. The adoption of hemiarthroplasty as a method for comminuted intertrochanteric fractures is proven as a means of hastening the recovery process, enabling early weight-bearing and mitigating the problems associated with extended bed res

Evaluating the field performance of multiple SARS-Cov-2 antigen rapid tests using nasopharyngeal swab samples.

The SARS-CoV-2 pandemic, which started in December 2019, has been posing significant challenges to the health care system worldwide. As the pandemic spreads with rapidly increasing number of positive cases, early diagnosis of infected patients is crucial to successfully limit the spread of the virus. Although the real-time reverse-transcription polymerase chain reaction (RT-qPCR) is the recommended laboratory method to diagnose COVID-19 infection, many factors such as availability of laboratory equipment, reagents and trained personnel affect the use of time-consuming molecular techniques. Study of percutaneous lumbar decompression and treatment algorithm for patients suffering from neurogenic claudication.

BACKGROUND: Symptomatic lumbar spinal stenosis (LSS) patients often suffer from multiple etiologies, and patient symptoms must be differentiated and identified as either neurogenic claudication, radicular pain, or both. The most common symptom associated with LSS is neurogenic claudication, which has been reported to occur in 91% to 100% of the LSS patient population. Neurogenic claudication symptoms are described as pain radiating to the lower extremities that begins and worsens as the patient ambulates. Neurogenic claudication symptoms worsen over time and can eventually result in significan

Seasonal change in osteoid thickness and mineralization lag time in ambulant patients.

UNLABELLED: Low vitamin D levels are common. Bone biopsies taken from 121 ambulant patients were therefore reviewed. Seasonal changes in mineralization correlated inversely with serum 25-hydroxyvitamin D but not the more active metabolite, 1,25-dihydroxyvitamin D. This implies that the latter is produced in bone. INTRODUCTION: It has been 30 yr since a seasonal variation in osteoid surfaces and calcification fronts was noted in bone biopsies from hip fracture patients in Leeds and attributed to vitamin D status. It was suggested at that time that mild vitamin D deficiency might cause osteopor

Frailty and pre-frailty as independent predictors of mortality in skin cancer survivors: Findings from the US National Health Interview Survey, 1997-2018.

Frailty is a multidimensional geriatric syndrome associated with adverse outcomes in aging populations and cancer patients. Although skin cancer is the most common malignancy in the United States, the prognostic role of frailty among skin cancer survivors remains unclear. This study aimed to examine whether frailty and pre-frailty are independently associated with all-cause mortality among skin cancer survivors. We analyzed data from the National Health Interview Survey 1997-2018, linked with the National Death Index through 2019. Frailty was assessed using a modified Fatigue, Resistance, Ambu

Effects of adding R-BoT + plus verticalization-based robotic locomotor training to conventional rehabilitation in patients with subacute stroke: a randomized controlled trial.

BACKGROUND: Early recovery of gait and trunk control during the subacute phase after stroke is crucial for long-term functional outcomes. Robotic-assisted rehabilitation enabling upright positioning, weight-bearing, and repetitive stepping-like movements has emerged as a promising adjunct to conventional therapy. The primary aim of this study was to evaluate whether adding R-BoT + plus verticalization-based robotic locomotor training to conventional inpatient rehabilitation leads to greater improvement in gait independence than conventional rehabilitation alone. The secondary aims were to asse

Recovery trajectories of spatiotemporal gait after bipolar hemiarthroplasty versus closed reduction and internal fixation for unstable intertrochanteric fracture: a prospective wearable-sensor study.

BACKGROUND: Geriatric hip fractures, particularly unstable intertrochanteric fractures, pose a significant global health challenge with high morbidity and mortality. The optimal surgical management remains debated, with both Closed Reduction and Internal Fixation (CRIF) and Bipolar Hemiarthroplasty (BHA) being widely accepted treatment options. However, there is a conspicuous paucity of objective, quantitative data comparing the longitudinal gait recovery trajectories following these two distinct surgical approaches. This study aimed to leverage validated wearable inertial sensors to systemati
