Impact of molecular analysis in the diagnosis of cutaneous lymphoid infiltrates.

The diagnosis and classification of cutaneous lymphomas is a challenge for the dermatopathologist. This is particularly true for determining … malignancy. There have been several reports that have identified clonal lymphoid proliferations in pityriasis lichenoides et varioliformis acuta, pseudolymphomas, and lichen planus. For this reason one must carefully evaluate the information that is provided by these techniques. The results should always correlate with clinical, histologic, and immunophenot …

Frequency and significance of minor clinical features in various age-related subgroups of atopic dermatitis in children.

The frequency and significance of minor clinical features for the diagnosis of atopic dermatitis (AD) were evaluated in different age-related subgroups of 100 children (up to 12 yrs) with AD and an equal number of controls. Additional features studied were infraauricular fissuring, diffuse scaling of the scalp, and Hertoghe sign. Features found to be nonspecific were ichthyosis, nipple eczema, cheilitis, keratoconus, anterior subcapsular cataract, pityriasis alba, anterior neck folds, and food intolerance. Other minor features were of variable significance in different age groups. Seasonal var…

Primary care physicians' errors in handling cutaneous disorders. A prospective survey.

This study analyzes the errors made by primary care physicians in handling skin disorders in patients seen prospectively over a 20-month period in a dermatologic practice. There were 319 errors in 260 patients. Eighty-eight percent of the errors were in diagnosis. There was a striking tendency to overdiagnose infectious dermatoses such as bacterial pyodermas, superficial mycoses, scabies, and herpes simplex and to underdiagnose inflammatory dermatoses such as contact dermatitis, nummular dermatitis, pityriasis rosea, and psoriasis. In 218 cases (68%) the error probably could have been prevente…

[The role of clothing in the transmission of pathogenic fungi ].

Laboratory studies were made to examine the possibility of transmission of pathogenic fungi through textiles. 1. Dermatophytes were proven to be present in clothing and leather articles worn over areas infected by fungi. In patients known to be infected with Pityriasis versicolor, contaminated scales were obtained from their textiles, the fungi could be demonstrated. 2. Artificially infected textiles were found to harbor dermatophytes and pathogenic yeasts for a number of weeks without limiting the survival capacity of the fungi. 3. At low temperatures, fungi were not affected by various deter…

[Therapeutic effect of vitamin A acid (retinoic acid) in various forms of palmar/plantar hyperkeratoses: experimental studies on 68 patients].

Up to date, the treatment of palmar/plantar hyperkeratoses presents a therapeutic problem. The known therapeutic procedures result in short- … ng as vitamin A acid was used as a maintenance dose once or twice weekly. In hyperkeratotic exzema, pityriasis rubra pilaris, and in verrucae plantaris vitamin A acid topically applied was found to be unsuitable for treatment. Weekly hair washing: The recommended solution for women with afro-textured hair to alleviate dandruff and scalp discomfort.

Dandruff and scalp discomfort are common concerns for women and men in Africa. Moreover, women with afro-textured hair are prone to scalp discomfort owing to irregular hair washing, frequent use of oil-based products on their scalp, and harsh chemical treatments. Current literature does not, however, provide data on the pathophysiological mechanisms of these conditions in this population. In this study, we investigated the mechanisms behind scalp discomfort and dandruff in women of African descent before and after hair washing. We conducted the in Durban, South Africa, over a 3-week period wit

Mucha-Habermann disease-like eruptions due to Tegafur.

The first case of Mucha-Habermann disease-like drug eruptions due to Tegafur is reported. A 59-year-old man noticed various skin lesions after he had taken 300 mg of Tegafur daily for about 200 days. The patient had papulonecrotic eruptions on his trunk and extremities. The histology from a papular lesion revealed epidermal necrosis surrounded by spongiosis, perivascular inflammatory infiltrations composed of lymphocytes and erythrocytes, and endothelial swelling. The etiology of Mucha-Habermann disease is not known, but an immune mechanism may be supported by our case.

Screening for glucose-6-phosphate dehydrogenase deficiency prior to dapsone therapy.

Dapsone is widely used in dermatological practice. The case of a Greek female patient is reported who had a severe, acute, haemolytic episode shortly after commencing dapsone therapy, despite a normal glucose-6-phosphate dehydrogenase (G6PD) screening test. A subsequent quantitative assay revealed reduced levels of G6PD consistent with the heterozygous state. This illustrates that routine screening tests may fail to detect the decreased levels of (G6PD) which occur in female heterozygotes. Since these patients are at risk of severe dapsone-induced haemolysis, the need for quantitative G6PD ass

Lymphomatoid papulosis.

Lymphomatoid papulosis is an unusual chronic skin disease characterized by the continuous appearance of papulonecrotic, nodular, and occasionally larger tumorous or plaque-like lesions showing histologic features suggestive of malignant lymphoma. The recognition of two histologic types of lymphomatoid papulosis, which are not distinct entities but represent the ends of a spectrum, has provided an explanation for the clinical and histologic variation and many of the current controversies in this disorder. These two types of lymphomatoid papulosis, called type A (or "histiocytic" type) and type

Febrile ulceronecrotic Mucha-Habermann disease: a case with systemic symptoms managed with subcutaneous methotrexate.

Febrile ulceronecrotic Mucha-Habermann disease (FUMHD) is a rare, idiopathic, acquired dermatosis that can affect all ages and ethnic groups. We present a 10-year-old patient with FUMHD associated with arthritis and chronic fatigue, managed with methotrexate. Through our literature review, we also explore treatment protocols for a disease for which internationally standardized management is yet to be formulated.
