Perianeurysmal fibrosis and its response to corticosteroid treatment: a computerized tomography followup in 1 case.

We report a case of bilateral hydronephrosis due to perianeurysmal fibrosis in which surgical ureterolysis could not be established due to inflammatory mass involvement of the ureteral wall. The right ureter was transected and continuity could not be restored. Therefore, the patient was treated with a high dose regimen of corticosteroids. Followup computerized tomography studies demonstrated resolution of obstruction of the left ureter and a decrease in the size of the perianeurysmal mass. The size of the mass stabilized and did not decrease any further.

Computed tomographic diagnosis of ureteral obstruction secondary to aneurysmal disease.

Three cases of ureteral obstruction associated with aortoiliac aneurysms were studied with computed tomography. CT was able to document the presence of aneurysm and display the perianeurysmal, fibrotic, contrast-enhancing mass accounting for ureteral obstruction. The value of computed tomography in the diagnosis and evaluation of these cases is described.

Erdheim-Chester disease presenting as an intracardiac mass and pericardial effusion confirmed by biopsy: a case report.

BACKGROUND: Erdheim-Chester disease (ECD) is a rare non-Langerhans cell histiocytosis that can affect the bones, heart, lungs, brain, and other organs. Cardiovascular involvement is common in ECD and is associated with a poor prognosis. Here, we report a case of ECD presenting as an intracardiac mass and pericardial effusion confirmed by biopsy with sternotomy. CASE SUMMARY: A 54-year-old man was admitted because of dyspnoea. He was previously diagnosed with bilateral hydronephrosis and retroperitoneal fibrosis. Echocardiography revealed a large amount of pericardial effusion and echogenic ma…

Renal denervation regulates the atrial arrhythmogenic substrates through reverse structural remodeling in heart failure rabbit model.

BACKGROUND: Heart failure (HF) causes atrial remodeling and increases the incidence of atrial fibrillation (AF). Renal denervation (RDN) has been shown to decrease the development of AF. This study aimed to identify the effects of RDN on the atrial arrhythmogenic substrates in HF. METHODS: Rabbits were classified into four groups: control (n=9), RDN (n=10), HF (n=6) and HF-RDN (n=9). Surgical and chemical RDN was approached through bilateral retroperitoneal flank incisions in RDN and HF-RDN. Rapid ventricular pacing of 400bpm for 4weeks was applied in HF and HF-RDN. After 4weeks, the rabbits

Recent advances in autoimmune pancreatitis: concept, diagnosis, and pathogenesis.

Recent advances support the concept of autoimmune pancreatitis (AIP) as a unique systemic disease, because it shows occasional extrapancreatic lesions such as sclerosing cholangitis, sclerosing sialoadenitis, and retroperitoneal fibrosis, pathological features similar to those of fibrosis, and abundant infiltration of IgG4-positive plasma cells, and it is steroid responsive. Based on these findings, several diagnostic criteria have been proposed. Although AIP is accepted worldwide as a unique clinical entity, its pathogenetic mechanism remains unclear. To clarify its pathogenesis, its genetic …

Thoracic manifestations of IgG4-related disease.

Immunoglobulin G4-related disease (IgG4-RD) is a recently described rare systemic fibroinflammatory disease with an estimated incidence of less than 1 in 100,000 persons per year. The disease can affect virtually any organ and is characterized by unifying histopathological findings. Recently, four subgroups of patients have been characterized: hepatobiliary, head and neck, Mikulicz syndrome and retroperitoneal fibrosis, who illustrate the mainly abdominal and ENT tropism of the disease. Yet, thoracic involvement is not uncommon. It can be detected in up to 30% of patients with systemic IgG4-RD…

Recent concepts of autoimmune pancreatitis and IgG4-related disease.

Recent studies suggested the existence of two subtypes of autoimmune pancreatitis (AIP): type 1 related with IgG4 (lymphoplasmacytic scleros … AIP often have extrapancreatic lesions such as sclerosing cholangitis, sclerosing sialadenitis, or retroperitoneal fibrosis showing similar pathological features. Based on these findings, many synonyms have been proposed for these conditions, such as "multifocal idiopathic fibrosclerosis", "IgG4-related autoimmune disease", "IgG4-related sclerosing disease", "IgG4-relate …

From Testis to Retroperitoneum: The Role of Radiomics and Artificial Intelligence for Primary Tumors and Nodal Disease in Testicular Cancer: A Systematic Review.

Background and Objectives: Radiomics and artificial intelligence (AI) offer emerging quantitative tools for enhancing the diagnostic evaluation of testicular cancer. Conventional imaging-ultrasound (US), magnetic resonance imaging (MRI), and computed tomography (CT)-remains central to management but has limited ability to characterize tumor subtypes, detect occult nodal disease, or differentiate necrosis, teratoma, and viable tumor in post-chemotherapy residual masses. This systematic review summarizes current advances in radiomics and AI for both primary tumors and retroperitoneal disease. Ma

Computed tomography in the differential diagnosis of the enlarged retrorectal space.

The value of computed tomography (CT) in the differentiation of an enlarged retrorectal space was analyzed in 132 cases. Classification of barium enema findings into those with simultaneous mucosal alterations and those without any visible lesions of the rectal mucosa seems to be useful. Computed tomography helps in those cases without mucosal changes to differentiate between retrorectal fibrosis, tumorous masses, and inflammatory diseases of the colon. It also demonstrates the lack of pathologic lesions in equivocal cases of pelvic lipomatosis and so-called "normal variants." If simultaneous

The disease has been associated with Sjögren's disease, retroperitoneal fibrosis and other fibrosing conditions, and may be a manifestation of a systemic fibro-inflammatory disorder. CONCLUSION: Autoimmune pancreatocholangitis is a distinct inflammatory disorder involving the pancreas and biliary tree. The disease may mimick pa …

Periaortitis Secondary to Evar: Case Report and Literature Review.

Periaortitis post endovascular aortic aneurysm repair (EVAR) represents a very rare complication, described only in 10 cases between 2001 and 2023. It may appear early or late after EVAR and the majority of patients had ureter compression, and all patients were treated with high dose of corticosteroids, with a complete resolution of symptoms except for one patient. We report a literature review and a case of early post-EVAR periaortitis manifested with acute renal failure due to ureter compression, treated with urgent bilateral J stent and high dose of corticosteroid, with complete resolution

Atypical manifestations of IgG4-related sclerosing disease in the abdomen: imaging findings and pathologic correlations.

OBJECTIVE: The purpose of this essay is to illustrate a variety of atypical imaging manifestations of IgG4-related sclerosing disease in the abdomen and to correlate the imaging and pathologic findings. CONCLUSION: In rare instances, IgG4-related sclerosing disease manifests atypical features in various organs in the abdomen. It is important that radiologists be aware of the typical and atypical features of this disease to provide timely effective treatment.
