Scaphoid Excision and Four-Corner Fusion for Neglected Perilunate Dislocations: Preliminary Results.

BACKGROUND: Perilunate dislocations are severe uncommon carpal injuries. They are sometimes missed with a reported incidence of up to 25%. Neglect for a period of time allows for soft tissue contractures, as well as bony changes that make reduction extremely difficult. For neglected cases, procedures such as staged open reduction, proximal row carpectomy, and wrist arthrodesis have been offered. The objective of this study was to examine whether scaphoid excision and four-corner fusion could be used to treat neglected perilunate dislocations. METHODS: Ten patients with neglected perilunate di

[Scaphoid fractures in childhood].

This article reviews current knowledge about scaphoid fractures in children and adolescents, including the development of the scaphoid, its injuries and age-specific treatment for acute and late presenting fractures. The following recommendations are based on the literature, although scientific evidence to support treatment strategies is scarce. The diagnosis of scaphoid fractures is often missed on initial X-rays. Therefore all suspected fractures should be treated with a thumb spica cast for 2 weeks, followed by clinical examination and repeat X-ray if symptoms persist. In cases of negative

Frequency and characteristics of the scaphoid cortical ring sign.

The scaphoid cortical ring sign (CRS) has been identified as a radiological indicator of ligamentous injury of the wrist. It has been associated with some pathokinematic states. There exists a range of wrist positions where the CRS may be normally present. The purpose of this study was to define the range of motion when the CRS can normally be observed on a standard posteroanterior radiograph and, in turn, to define the range where the CRS is not expected to be present. One hundred and nine posteroanterior radiographs of normal wrists were evaluated for the presence, partial presence and absen

Carpal Kinematics in the Normal, Scapholunate Ligament Deficient, and Surgically Reconstructed Wrist.

The objective of this study was to evaluate scaphoid, lunate and capitate kinematics after disruption to the primary and secondary scapholunate ligamentous stabilizers, and to assess the effectiveness of scapholunate ligament reconstruction in restoring carpal kinematics post-operatively. Seven upper extremities were harvested, and the scapholunate interosseous ligament (SLIL) was divided. Specimens were mounted onto a computer-controlled dynamic wrist simulator, and simulations of flexion-extension, radial-ulnar deviation, and dart-thrower's motion were undertaken by simulated force applicati

Is Joint Levelling Procedure Better than Temporary Scaphocapitate Pinning in Patients Undergoing Vascularized Bone Graft for Kienböck Disease?

Background: Although there have been many studies of the vascularized bone graft (VBG) or unloading procedures alone for the treatment of Kienböck disease, little information has been reported about patients treated with VBG combined with unloading procedures. The purpose of this study is to 1) describe the outcomes in patients treated with VBG combined with unloading procedures, 2) compare the outcomes according to the unloading procedures and 3) find any radiologic parameters affecting revascularization in Kienböck disease. Methods: A retrospective review was performed involving in 20 patien

The role of extrinsic ligaments in maintaining carpal stability - A prospective statistical analysis of 85 arthroscopic cases.

Several biomechanical studies have shown that the scapholunate (SL) and lunotriquetral (LT) ligaments are not the only stabilizers of the proximal carpal row. However, no study has yet analyzed the range of ligament lesions leading to instability in vivo. Arthroscopy has been used to assess the condition of the wrist's extrinsic ligaments by palpating and tensioning the various ligament and capsule structures. In this prospective study, this arthroscopic method was used in 85 cases of wrist sprain without static instability to evaluate the correlation between lesions of the intrinsic and extri

Scaphoid Malunion Clinical and Radiographic Outcomes at a Minimum of 4 Years Follow-Up.

PURPOSE: To evaluate the radiographic and clinical outcomes of patients with scaphoid malunion after acute fracture at a mean of 7 years after injury. METHODS: Patients with scaphoid malunion were identified from a departmental database of acute scaphoid fractures. Patients with a scaphoid height-to-length ratio greater than 0.6 on final follow-up computed tomography (CT) scan were considered malunited. These patients were contacted to return for CT imaging and clinical assessment. A total of 22 patients were included (4 females and 18 males). Average age of the group was 41 years (range, 16-

Experience of an isolated use of low-intensity pulsed ultrasound therapy on fracture healing in established non-unions: a prospective case series.

Fracture non-unions are responsible for approximately 5% of cases, with low expectation of spontaneous healing thereafter. The fractures that heal without complications can take months to heal completely. An ultrasound, typically at low intensities (0.5-50 mW/cm2), serves a diagnostic purpose, whereas at higher intensities (0.2-100 W/cm2), its role becomes more therapeutic by generating heat energy. Low-intensity pulsed ultrasound (LIPUS) is frequently used to stimulate or to accelerate fracture healing. We present a prospective review of fracture non-unions treated with low-intensity pulsed u

[First degree open, proximal and distal fractures of the lower arm with scapholunate ligament rupture. Case report of severe extremity trauma].

The combination of proximal and distal radius and ulna fractures with scapholunate ligament injury is extremely rare and this type of injury may potentially only be possible in osteoporotic bone. In this case report this rare injury is discussed, a possible therapy regime is presented and a typical postoperative complication is shown.

Scaphoid excision and midcarpal arthrodesis: the effect of triquetral excision--a biomechanical study.

PURPOSE: To evaluate the biomechanical alterations that occur after traditional scaphoid excision and midcarpal arthrodesis with and without excision of the triquetrum. The hypothesis of this study was that removal of the triquetrum increases the radiolunate contact pressure. METHODS: We cyclically moved 10 fresh cadaver wrists using a wrist joint motion simulator while measuring the contact pressures between the proximal carpal row and the distal radius and ulna using a dynamic pressure sensor. We acquired data in the intact wrist, after a midcarpal arthrodesis with the scaphoid excised, and

Non-operative treatment of non-displaced scaphoid fractures may be preferred.

AIM: To evaluate the outcome of non-displaced scaphoid fractures treated with 6 weeks of cast immobilisation, and to establish whether the benefits of non-operative treatment might outweigh those of early operation. METHODS: A retrospective study analysed 89 consecutive cases of scaphoid fracture treated at our institution between 2004 and 2007. Diagnosis and treatment methods and complication rates were evaluated. RESULTS: Among 71 non-displaced scaphoid fractures, >80% showed clinical consolidation after 6 weeks of cast immobilisation, and the remaining cases after 8-12 weeks. Two cases ne

The clinical scaphoid fracture: early computed tomography as a practical approach.

BACKGROUND: In low- and middle-income countries, mental health training often includes sending few generalist clinicians to specialist-led programs for several weeks. Our objective is to develop and test a video-assisted training model addressing the shortcomings of traditional programs that affect scalability: failing to train all clinicians, disrupting clinical services, and depending on specialists. METHODS: We implemented the program -video lectures and on-site skills training- for all clinicians at a rural Nepali hospital. We used Wilcoxon signed-rank tests to evaluate pre- and post-test

Scaphotrapeziotrapezoid fusion: long-term follow-up study.

The long-term effectiveness of scaphotrapeziotrapezoid (STT) fusion is still controversial. We evaluated our clinical and radiological results of 30 STT fusions in 30 patients (average age 41 years; 23 males, 7 females). The follow-up period averaged 84 months. The indications were Kienböck's disease ( n= 23), isolated STT arthrosis ( n = 6), and dislocation of the trapezium ( n = 1). A total of 26 simultaneous associated procedures were performed. Excisional arthroplasty of the lunate was done in all 23 patients with Kienböck's disease. Of the 30 total patients, 5 had postoperative pain rangi

Degenerative arthritis of the wrist: proximal row carpectomy versus scaphoid excision and four-corner arthrodesis.

Two cohort populations of 19 patients from separate institutions performing exclusively either a scaphoid excision and 4-corner arthrodesis (lunate, capitate, hamate, and triquetrum) or proximal row carpectomy (PRC) for scapholunate advanced collapse arthritis were compared. There were no preoperative differences with respect to age, gender, dominance, stage of arthritis, or preoperative measures of pain and function. The length of the follow-up period averaged 28 months for the 4-corner arthrodesis group compared with 19 months for the PRC patients. At the follow-up examination wrist motion r

Open complete dislocation of trapezium with a vertically split fracture: a case report.

Open complete dislocation of the trapezium is an extraordinarily rare injury with only a few cases reported so far in literature. The association of a vertically split fracture makes this injury even rare and hence worth reporting. A 14 year old Kashmiri boy presented to us with a history of massive trauma to the non dominant left hand sustained as a result of a blow from a heavy hammer. The thenar area was burst out and the trapezium was vertically split apart into two halves which were dislocated from the articular surfaces of the scaphoid as well as the first metacarpal. The mechanism of in
