Determination of urinary lithogenic parameters in murine models orthologous to autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD), a genetic disease caused by mutations in PKD1 or PKD2 genes, is associated with a high prevalence of nephrolithiasis. The underlying mechanisms may encompass structural abnormalities resulting from cyst growth, urinary metabolic abnormalities or both. An increased frequency of hypocitraturia has been described in ADPKD even in the absence of nephrolithiasis, suggesting that metabolic alterations may be associated with ADPKD per se. We aimed to investigate whether non-cystic Pkd1-haploinsufficient (Pkd1(+/-)) and/or nestin-Cre Pkd1-targeted

Is retrograde flexible nephrolithotripsy feasible for calyceal diverticular stone?

The aim of this study is to manage the calyceal diverticular (CD) stone, several treatment options were applied but it is still a challenging condition of endourology. In this retrospective study, we aim to report the results of our patients with CD stones treated with retrograde flexible nephrolithotripsy (RFNL). A total of 47 patients (55 procedures) who underwent RFNL by a single surgeon for CD stones from January 2004 to March 2013 were evaluated retrospectively. The demographic properties and initial complaints of those patients have been recorded along with their comorbidities and previo

Percutaneous nephrolithotomy in solitary kidneys: experience with 412 cases from Southern Iran.

Some patients with nephrolithiasis who become candidates for percutaneous nephrolithotomy (PCNL) have a single kidney. This poses a challenge for the physician regarding the safety and efficacy of this procedure for these patients. This study has aimed to evaluate the safety and efficacy of PCNL in these patients. Between 2002 and 2014, out of 10,800 cases who underwent PCNL in our centres, 412 had a single kidney. We recorded the results and complications in these patients. Out of 412 patients, 279 were men and 133 women. Their mean age was 46.4 years (range 19-71) and mean stone size was 26.

A human strain of Oxalobacter (HC-1) promotes enteric oxalate secretion in the small intestine of mice and reduces urinary oxalate excretion.

Enteric oxalate secretion that correlated with reductions in urinary oxalate excretion was previously reported in a mouse model of primary hyperoxaluria, and in wild type (WT) mice colonized with a wild rat strain (OXWR) of Oxalobacter (Am J Physiol 300:G461–G469, 2010). Since a human strain of the bacterium is more likely to be clinically used as a probiotic therapeutic, we tested the effects of HC-1 in WT. Following artificial colonization of WT mice with HC-1, the bacteria were confirmed to be present in the large intestine and, unexpectedly, detected in the small intestine for varying peri

Laparoscopic pyelolithotomy versus percutaneous nephrolithotomy for a solitary renal pelvis stone larger than 3 cm: a prospective cohort study.

To prospectively compare the outcome of laparoscopic pyelolithotomy (LP) versus percutaneous nephrolithotomy (PCNL) in patients with a solitary renal pelvis stone larger than 30 mm. We analyzed demographic and perioperative parameters and intermediate outcome in 30 adults who underwent transperitoneal LP for solitary renal pelvis stone larger than 30 mm (Group I) and compared the results with 30 patients who underwent PCNL (Group II). The two groups were matched for age, sex and stone size (Group I 35.3 ± 7.33 mm, Group II 36.6 ± 7.0 mm; P = 0.47). Mean operative time was significantly longer

The presence and distribution of alpha adrenergic receptors in human renal pelvis and calyces.

In this study, we aimed to demonstrate the presence of Alpha (α) 1 receptors and subtypes in human pelvis and calyces, because an agent to facilitate kidney stone movement and help decrease pain may be an α 1 adrenergic blocker, as used in ureteral stones. Twenty patients who applied to our clinic for renal cell carcinoma were enrolled to the study. All patients underwent radical nephrectomy. After the specimens were removed, excisional biopsies were performed on healthy pelvises and calyces. Mean α-receptor stain rates in renal pelvis were 2.65 ± 0.74, 1.35 ± 0.81 and 2.9 ± 0.30 for α 1A, 1B

Predictors of excessive renal displacement during access in percutaneous nephrolithotomy: a randomized clinical trial.

To determine the magnitude of renal displacement (a major cause of access failure or loss) during the renal access steps in percutaneous nephrolithotomy (PCNL), investigate predictors of excessive renal displacement, and compare the effect of one-stage versus gradual dilation on renal displacement during access. Sixty-six adult patients undergoing PCNL were randomized into two groups containing 33 patients each: Group 1 underwent gradual tract dilation with Alken metal dilators, and Group 2 received one-stage tract dilation. In each patient, maximum renal displacement was measured in three pla

Transurethral holmium-YAG laser lithotripsy for large symptomatic prostatic calculi: initial experience.

Symptomatic prostatic calculi are a rare clinical entity with wide range of management options, however, there is no agreement about the preferred method for treating these symptomatic calculi. In this study we describe our experience of transurethral management of symptomatic prostatic calculi using holmium-YAG laser lithotripsy. Patients with large, symptomatic prostatic stones managed by transurethral lithotripsy using holmium-YAG laser over 3-year duration were included in this retrospective study. Patients were evaluated for any underlying pathological condition and calculus load was dete

Retrograde intrarenal surgery in patients with horseshoe kidneys.

Retrograde intrarenal surgery (RIRS) in patients with horseshoe kidneys (HSKs) remains poorly studied. The present study aimed to assess clinical success and stone-free rates in HSK patients with renal stones treated with flexible ureteroscopy. RIRS was attempted in 20 patients with 25 renal stones in HSK from December 2008 to January 2012. The patients were evaluated with imaging studies including plain abdominal radiography, intravenous urogram, abdominal ultrasonography or non-contrast tomography scan. Success rate was defined as stone-free or residual fragment less than 4 mm. Pre-operative

Hereditary causes of kidney stones and chronic kidney disease.

Adenine phosphoribosyltransferase (APRT) deficiency, cystinuria, Dent disease, familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC), and primary hyperoxaluria (PH) are rare but important causes of severe kidney stone disease and/or chronic kidney disease in children. Recurrent kidney stone disease and nephrocalcinosis, particularly in pre-pubertal children, should alert the physician to the possibility of an inborn error of metabolism as the underlying cause. Unfortunately, the lack of recognition and knowledge of the five disorders has frequently resulted in an unacceptabl

Thirty-day outcomes of Direct In-Scope Suction (DISS) in real-world practice using magneto, MOSES, and Thulium fiber laser technologies for renal and ureteral stones: an EAU endourology study.

Direct in-scope suction (DISS) ureteroscopy integrates aspiration through the ureteroscope working channel, but the interaction between suction and contemporary laser platforms is incompletely defined. We prospectively compared MOSES high-power holmium: YAG, thulium fiber laser (TFL), and Magneto holmium: YAG during DISS-assisted flexible ureteroscopy across 16 centres in 14 countries. Adults treated between May and December 2025 underwent baseline and 30-day non-contrast computed tomography (NCCT). The primary efficacy endpoint was zero-residual-fragment stone-free status (ZRF-SFS; Grade A) o

Bridging evidence gaps in opioid-sparing postoperative pain control for percutaneous nephrolithotomy: a network meta-analysis identifying optimal strategies.

Percutaneous nephrolithotomy (PCNL) is the gold standard for large renal calculi, yet postoperative pain from tract dilation, capsule injury, and inflammation often delays recovery, prolongs hospital stays, and reduces patient satisfaction. Amid the opioid crisis, reliance on opioids for pain control raises concerns over adverse effects like respiratory depression, nausea, and dependence. This has spurred interest in opioid-sparing multimodal strategies, with recent studies suggesting feasibility of opioid-free discharge and comparable relief from NSAIDs with fewer side effects. However, robus

Establishing stone composition from pure dust: feasibility and analytic yield of FTIR spectroscopy during suction-assisted flexible ureteroscopy.

The shift toward dust-only flexible ureteroscopy (FURS) prevents routine metabolic stone analysis due to a lack of retrievable fragments. We evaluated the technical success, analytical yield, and composition profile of Fourier-transform infrared (FTIR) spectroscopy performed on stone dust collected via flexible navigable suction access sheaths (FANS). This prospective single-center study included 109 patients undergoing FURS with FANS for upper tract stone disease. The surgeon aspirated the stone dust slurry into a container. Then, the fluid samples collected were sent to the lab to be centrif

Adherence to medical management of kidney stones: A systematic review.

The effectiveness of dietary and pharmacologic stone prevention strategies may be limited by poor patient adherence. This systematic review aims to assess adherence to the American Urological Association (AUA) guidelines for medical management of kidney stone disease by (1) determining how adherence influences clinical outcomes, (2) identifying which preventive interventions demonstrate the highest adherence, and (3) characterizing factors associated with adherence. A systematic review was conducted according to PRISMA guidelines. PubMed, Scopus, and Embase were searched from database inceptio

Sex-specific associations of traditional and novel anthropometric indices with incident kidney stone disease: a 10-year longitudinal cohort study.

Excess adiposity and altered body fat distribution are increasingly recognized as important contributors to metabolic disorders, including kidney stone disease (KSD). However, how to best assess obesity-related KSD risk and the temporal relationship remain unclear. This 10-year retrospective longitudinal cohort study included participants undergoing annual health examinations. Data on demographics, KSD, and six anthropometric indices were collected. Cox regression stratified by sex was used. Model 1 examined the associations of baseline indices with 10-year incident KSD, while Model 2 examined

Preoperative CT-based markers and inflammatory parameters as predictors of failed ureteroscopic access in unstented patients with ureteral stones: a prospective single-center cohort study.

Failed ureteroscopic access (FA) in unstented patients remains a significant clinical challenge. We aimed to identify preoperative clinical, laboratory, and CT-based predictors of FA, determine optimal cutoff values, and develop a clinically applicable risk-stratification model. This prospective cohort study enrolled 141 consecutive patients with ureteral stones undergoing unstented ureteroscopy (83 semi-rigid, 58 flexible) at a tertiary center (January 2024-January 2025). Preoperative demographics, pain characteristics, inflammatory markers (TLC, ESR, CRP, procalcitonin), and NCCT parameters
