[Correction of cardiac defects through a right minithoracotomy in children].

OBJECTIVE: To review the experience of correction of congenital cardiac defects through a right minithoracotomy. METHOD: 319 patients underwent correction of congenital heart malformations through right lateral thoracotomy under cardiopulmonary bypass. The average age was 3.44 +/- 1.59 years (range, 5 months-8 years). The average body weight was 13.66 - 3.98 kg (range, 6 - 26 kg). Cardiac defects repaired included atrial septal defect in 87 patients (1 patient associated with left superior vena cava (LSVC), 6 pulmonary stenosis, 5 partial anomalous pulmonary venous connection), ventricular se

Nonmuscle myosin II-B is required for normal development of the mouse heart.

We used targeted gene disruption in mice to ablate nonmuscle myosin heavy chain B (NMHC-B), one of the two isoforms of nonmuscle myosin II present in all vertebrate cells. Approximately 65% of the NMHC-B-/- embryos died prior to birth, and those that were born suffered from congestive heart failure and died during the first day. No abnormalities were detected in NMHC-B+/- mice. The absence of NMHC-B resulted in a significant increase in the transverse diameters of the cardiac myocytes from 7.8 +/- 1.8 micron (right ventricle) and 7.8 +/- 1.3 micron (left ventricle) in NMHC-B+/+ and B+/- mice t

Medium-term follow-up of pulmonary autograft replacement of aortic valves in children.

Pulmonary autograft replacement (PAG) of the aortic valve in children has been shown to be safe and effective with a low incidence of late valve dysfunction. Relief of all types of left ventricular outflow tract obstruction using the pulmonary root has been possible. Concern about the durability of the pulmonary root in the aortic position, and the potential for growth of the pulmonary autograft used either as a root replacement or intraaortic implant, has been questioned. Sixty-five consecutive patients, aged 1.8 to 21 years (mean 12 years) operated on between September 1986 and January 1993,

Interrupted aortic arch repair: aortic arch advancement without a patch minimizes arch reinterventions.

BACKGROUND: Surgical repair of interrupted aortic arch (IAA) remains challenging and is associated with significant mortality and incidence of late arch obstruction, as recently reported by the Congenital Heart Surgeons' Society (CHSS). In particular, the CHSS reported that any technique other than direct anastomosis with patch augmentation is a risk factor for arch reintervention. The experience at Texas Children's Hospital with IAA repair using an aortic arch advancement technique without a patch was examined. METHODS: Between July 1995 and December 2005, 60 patients underwent IAA repair us

Real-World Experience of Mavacamten for Patients With Obstructive Hypertrophic Cardiomyopathy in South Korea: A Prospective Multi-Center Observational Study.

BACKGROUND AND OBJECTIVES: Mavacamten has shown promise in obstructive hypertrophic cardiomyopathy (oHCM); however, real-world evidence is limited in Asians. We aimed to provide the first multicenter, real-world analysis of mavacamten use in Korea. METHODS: This prospective observational study included symptomatic oHCM patients treated at 7 tertiary hospitals in Korea. Changes in key parameters, including left ventricular outflow tract (LVOT) gradients, N-terminal pro-B-type natriuretic peptide (NT-proBNP), and New York Heart Association functional class (NYHA Fc), were serially assessed. Systolic anterior motion of the mitral valve in hypertrophic cardiomyopathy: a narrative review.

BACKGROUND AND OBJECTIVE: The prevalence of hypertrophic cardiomyopathy (HCM) is estimated to be 1 in 200 to 500 individuals, with systolic anterior motion (SAM) of the mitral valve (MV) and left ventricular outflow tract (LVOT) obstruction present in 60% to 70%. In this narrative review, we aim to elucidate the pathophysiology of SAM-septal contact and LVOT obstruction in HCM by presenting a detailed review on the anatomy of the MV apparatus in HCM, examining the various existing theories pertaining to the SAM phenomenon as supported by cardiac imaging, and providing a critical assessment of

Outcomes and Sex Differences in Alcohol Septal Ablation for Obstructive Hypertrophic Cardiomyopathy-Insights from a Japanese Nationwide Registry.

Recent advances in pre-procedural imaging have improved risk assessment and procedural planning for alcohol septal ablation (ASA) in patients with symptomatic obstructive hypertrophic cardiomyopathy (HOCM). However, limited data exist regarding the current practice and outcomes of ASA in Japan. Data from 634 patients who underwent ASA at 72 hospitals between January 2019 and December 2022 were extracted from the nationwide prospective J-SHD registry. Baseline characteristics, procedural details, and in-hospital outcomes were evaluated, and multivariate logistic regression analyses were perform

Sudden death in athletes.

HCM, as well as coronary and myocardial structural abnormalities, is the most common pathology leading to SCD in young athletes. Furthermore, SCD from fatal arrhythmia seems to be the most common mechanism of death. In this population, however, data are insufficient to support either invasive or noninvasive approaches to clarify risk stratification for SCD. Because of the large population, variants of normal found within the athletic population, and the rarity of the disease, screening for individuals at risk is neither practical nor cost-effective. Not all athletes with HCM are at the same ri

[Aneurysm of the membranous portion of the interventricular septum].

The membranous portion of the interventricular septum represents the final phase of the ventricular growth. It is situated between the orifice of the coronary sinus and the supraventricular crest; immediately below the right aortic semilunar valves and not the coronary. Four cases of aneurism of the membranous portion of the interventricular septum are presented; in two, the diagnosis was made by angiocardiography study and in the rest it was made with the findings of an autopsy. All were of the female sex. Two patients presented a systolic murmur in the low mesocardia; three had heart failure

Hypertrophic cardiomyopathy: state of the art on diagnosis and current therapeutic approach.

BACKGROUND: Hypertrophic cardiomyopathy is the most common genetic cardiomyopathy and is characterized by unexplained myocardial hypertrophy. This condition exhibits marked clinical and prognostic heterogeneity. Advances in cardiovascular imaging techniques and genetic testing have significantly improved the diagnostic accuracy, phenotypic characterization, and prognostic assessment. Hypertrophic cardiomyopathy can present with a broad spectrum of genetic substrates and clinical presentations, ranging from asymptomatic individuals to those with heart failure, arrhythmias, and even sudden cardi

Cardiac amyloidosis in severe aortic stenosis patients: an updated review unraveling their complex interplay.

Aortic stenosis (AS) and cardiac amyloidosis (CA) often coexist in elderly patients, complicating diagnosis and treatment. AS is characterized by the narrowing of the aortic valve, leading to left ventricular outflow obstruction, primarily due to age-related calcification and congenital defects. CA, the most common cause of restrictive cardiomyopathy, results from amyloid protein deposits in the myocardium, causing diastolic dysfunction. Accurate diagnosis of CA in AS patients is challenging due to overlapping symptoms and the need for advanced imaging and biomarkers. Aortic valve replacement

The HighLife transcatheter mitral valve replacement system: A novel two-component platform.

Mitral regurgitation (MR) remains a prevalent and undertreated condition, particularly in patients at high surgical risk or unsuitable for mitral transcatheter edge-to-edge repair (M-TEER). The HighLife (HighLife SAS, Paris, France) transcatheter mitral valve replacement (TMVR) system offers a novel solution with its unique two-component design comprising a subannular ring and a self-expanding bioprosthetic valve. Delivered via a transfemoral arterial and transseptal venous approach, this "valve-in-ring" system enables circumferential anchoring without the need for annular calcification or tra

Sudden Cardiac Arrest During a Sedated Cardiac Magnetic Resonance Study in a Nonsyndromic Child with Evolving Supravalvar Aortic Stenosis Due to Familial ELN Mutation.

Supravalvar aortic stenosis (SVAS) is a less common but clinically important form of left ventricular outflow tract obstruction, and commonly associated with Williams syndrome (WS). SVAS outside of WS may also occur sporadically or in a familial form, often with identifiable mutations in the elastin (ELN) gene. While risk of sudden cardiac death in patients with SVAS has been extensively described in the context of WS, less is known about risk in patients with isolated SVAS. We report a case of a nonsyndromic two-year-old boy with evolving manifestations of SVAS who developed sudden cardiac ar

Post-procedural structural heart CT imaging: TAVR, TMVR, and other interventions.

Transcatheter valve replacement has experienced substantial growth in the past decade and this technique can now be used for any of the four heart valves. Transcatheter aortic valve replacement (TAVR) has overtaken surgical aortic valve replacement. Transcatheter mitral valve replacement (TMVR) is often performed in pre-existing valves or after prior valve repair, although numerous devices are undergoing trials for replacement of native valves. Transcatheter tricuspid valve replacement (TTVR) is similarly under active development. Lastly, transcatheter pulmonic valve replacement (TPVR) is most

Optical coherence tomography and multiphoton microscopy offer new options for the quantification of fibrotic aortic valve disease in ApoE-/- mice.

Aortic valve sclerosis is characterized as the thickening of the aortic valve without obstruction of the left ventricular outflow. It has a prevalence of 30% in people over 65 years old. Aortic valve sclerosis represents a cardiovascular risk marker because it may progress to moderate or severe aortic valve stenosis. Thus, the early recognition and management of aortic valve sclerosis are of cardinal importance. We examined the aortic valve geometry and structure from healthy C57Bl6 wild type and age-matched hyperlipidemic ApoE-/- mice with aortic valve sclerosis using optical coherence tomogr

Pathologist's role in identifying cardiac defects-a fetal autopsy series.

AIMS: Major cardiac abnormalities can lead to miscarriages and is also an important indication of medical termination of pregnancy. The present study aims: (1) To determine the spectrum of cardiac anomalies in fetal autopsies, (2) To correlate the autopsy findings with prenatal imaging studies. METHODS: Fetal autopsies with cardiac anomalies were retrospectively analyzed over a period of five years. Autopsy diagnoses were correlated with prenatal ultrasound and echocardiogram findings. RESULTS: Of the 177 fetal autopsies, 40 (22.5%) cases with cardiac defects were identified. Among these cas

Upregulated Autophagy in Calcific Aortic Valve Stenosis Confers Protection of Valvular Interstitial Cells.

Autophagy serves as a cell survival mechanism which becomes dysregulated under pathological conditions and aging. Aortic valve thickening and calcification causing left ventricular outflow obstruction is known as calcific aortic valve stenosis (CAVS). CAVS is a chronic and progressive disease which increases in incidence and severity with age. Currently, no medical treatment exists for CAVS, and the role of autophagy in the disease remains largely unexplored. To further understand the role of autophagy in the progression of CAVS, we analyzed expression of key autophagy genes in healthy, thicke

Modern management of systolic anterior motion of the mitral valve.

Systolic anterior motion (SAM) of the mitral valve (MV) can be a life-threatening condition. The SAM can result in severe left ventricular outflow tract obstruction and/or mitral regurgitation and is associated with an up to 20% risk of sudden death (which is substantially lower in hypertrophic cardiomyopathy (HCM)). The mechanisms of SAM are complex and depend on the functional status of the ventricle. The SAM can occur in the normal population, but is typically observed in patients with HCM or following MV repair. Echocardiography (2D, 3D and stress) has a central diagnostic role as the appl
