Vestibular evoked myogenic potential induced by bone-conducted stimuli in patients with conductive hearing loss.

CONCLUSIONS: Bone-conducted vestibular evoked myogenic potentials (B-VEMPs) showed high specificity for the presence of vertigo in patients with unilateral chronic otitis media (COM). These results suggest that vestibular function can be evaluated with B-VEMPs, even in patients with conductive hearing loss. OBJECTIVE: The purpose of this study was to investigate the VEMPs induced by bone-conducted stimuli (B-VEMPs) in patients with conductive hearing loss due to perforated COM. SUBJECTS AND METHODS: The subjects were 48 patients with unilateral perforated COM and conductive hearing loss. The

Prevalence of vestibulopathy in benign paroxysmal positional vertigo patients with and without prior otologic history.

The purpose of this study was to determine the prevalence of reduced or absent labyrinthine reactivity (vestibulopathy) in two groups of participants with posterior canal BPPV. One group had prior diagnosis of otologic disease (positive history group). No one in the second group had ever been diagnosed with otologic disease (negative history group). Caloric responses were retrospectively analyzed for the two groups. Patients with a positive history exhibited a greater prevalence of vestibulopathy than patients with a negative history. The positive history group, on average, also exhibited a la

Preservation of tap vestibular evoked myogenic potentials despite resection of the inferior vestibular nerve.

Sound and skull-tap induced vestibular evoked myogenic potentials (VEMP) were studied in a 43-year-old man following inferior vestibular neurectomy. Surgery was performed because of a small acoustic neuroma. Postoperative caloric testing suggested sparing of superior vestibular nerve function on the operated side. In response to sound stimulation there were no VEMP on the operated side, irrespective of whether sounds were presented by air- or bone-conduction. This suggests sound-induced VEMP to be critically dependent on inferior vestibular nerve function and this is in agreement with present

Differing effects on the inner ear of three gentamicin compounds: GM-C1, -C2 and -C1a.

OBJECTIVE: Gentamicin (GM) has been used to ablate the vestibular system function as a form of treatment for Ménière's disease. Generally, clinicians have not recognized that commercial GM comprises three major compounds--GM-C1, -C2 and -C1a--and the effects of the individual compounds on the inner ear have not been elucidated. In this study we investigated differences in cochleo- and vestibulotoxicity between the three compounds MATERIAL AND METHODS: Hartley guinea pigs were treated for 2 weeks with daily subcutaneous injections (100 mg/kg) of the GM complex or the GM-C1, -C2 or -C1a compoun

Effect of white noise on vestibular evoked myogenic potentials.

OBJECTIVES: To clarify if p13-n23 of vestibular evoked myogenic potentials (VEMPs) is independent of cochlear afferents. METHODS: Twenty normal volunteers and 10 patients with hemifacial palsy enrolled into this study. VEMP and auditory brainstem responses (ABRs) were recorded with or without white noise (WN, 75 dBnHL or 95 dBnHL) ipsilaterally or contralaterally to the stimulated ear. Short tone bursts (STBs) of 0.5 kHz (95 dBnHL, rise/fall time=1 ms, plateau time=2 ms) were presented though headphones. For recording VEMPs surface electromyographic activity was recorded in the supine subject

Clinical characteristics of audio-vestibular impairment in Ménière's disease: does vestibular function deteriorate in accordance with cochlear function?

The clinical course of Ménière's disease and the outcomes of functional examinations were found to be correlated, and a difference was found between the progressions of vestibular and cochlear dysfunction. Neither the total number of definitive vertiginous spells nor the duration of illness correlated significantly with the degree of audio-vestibular dysfunction. In contrast, frequent vertiginous spells were clearly associated with preservation of audio-vestibular function. Moreover, the clinical characteristics of those patients who experienced less than one definitive vertiginous spell per y

Vestibular evoked myogenic potentials in response to laterally directed skull taps.

In recent years it has been demonstrated that loud clicks generate short latency vestibular evoked myogenic potentials (VEMP). It has also been demonstrated that midline forehead skull tap stimulation evokes similar VEMP. In the present study, the influence of skull tap direction on VEMP was studied in 13 normal subjects and in five patients with unilateral vestibular loss. Gentle skull taps were delivered manually above each ear on the side of the skull. The muscular responses were recorded over both sternocleidomastoid muscles using skin electrodes. Among the normals, laterally directed skul

Lack of gender difference in motion sickness induced by vestibular Coriolis cross-coupling.

It has been reported that females are more susceptible to motion sickness than males. Supporting evidence is primarily based on retrospective survey questionnaires and self-reporting. We investigated if there is a gender difference in motion sickness susceptibility using objective and subjective measurements under controlled laboratory conditions. Thirty healthy subjects (14 males and 16 females) between the ages of 18-46 years were exposed to Coriolis cross-coupling stimulation, induced by 120 degrees /s yaw rotation and a simultaneous 45 degrees pitch forward head movement in the sagittal pl

Vestibular function in patients with cochlear implantation.

The aim of this work was to determine the influence of cochlear implantation (CI) on vestibular canal and otolithic function. Between 1995 and 1999, 15 patients (6 females, 9 males; 9-77 years old) underwent a vestibular examination before and after CI. Electronystagmography was performed between 5 and 8 days after CI in 9 patients, and with a time delay of 2-24 months in 10 patients. Pre- and postoperative evaluation included electronystagmography with caloric (44 degrees C, 30 degrees C, ice-water) and pendular rotatory testing. Otolithic function was measured postoperatively using off-verti

Glycerol affects vestibular evoked myogenic potentials in Meniere's disease.

OBJECTIVES: to show that abnormal vestibular evoked myogenic potentials on the sternocleidomastoid muscle (SCM) in patients with unilateral Meniere's disease are caused by endolymphatic hydrops. SUBJECTS: six normal volunteers and 17 patients with unilateral Meniere's disease were examined. METHODS: click-evoked myogenic potentials were recorded with surface electrodes over each SCM. Responses evoked by clicks recorded after oral administration of glycerol (1.3 g/kg body weight) were compared with those recorded before administration. RESULTS: the change rate of the p13-n23 amplitude was ca

Cortical and subcortical vestibular response to caloric stimulation detected by functional magnetic resonance imaging.

The posterior insula, central sulcus, and inferior parietal lobule including the intraparietal sulcus have been considered the vestibular cortex based on functional brain mapping in humans as well as experiments in lower primates. The same regions receive optokinetic, visual, and proprioceptive projections. We examined the cortical and subcortical projection of vestibular activity with visual and proprioceptive input eliminated during caloric stimulation (CS), using functional magnetic resonance imaging (fMRI). Single-shot gradient-echo echoplanar image (EPI) volumes were sensitive to BOLD con

Vestibulo-ocular reflex deficits to rapid head turns following intratympanic gentamicin instillation.

The response of the vestibulo-ocular reflex following unilateral vestibular deafferentation by gentamicin ablation was studied using transient stimuli. The response to these rapid passive head turns showed a strong asymmetry with permanent, reduced gains toward the side of lesion. These gain reductions have large variation (gains of 0.26 to 0.83), which may result from preferential sparing of regularly firing afferent fibers following gentamicin ablation. Based on the size and nature of the nonlinearity, an explanation based on Ewald's second law was discounted.

Unilateral vestibular deafferentation causes permanent impairment of the human vertical vestibulo-ocular reflex in the pitch plane.

Rapid, passive, unpredictable, low-amplitude (10-20 degrees), high-acceleration (3000-4000 degrees/s2) head rotations were used to study the vertical vestibulo-ocular reflex in the pitch plane (pitch-vVOR) after unilateral vestibular deafferentation. The results from 23 human subjects who had undergone therapeutic unilateral vestibular deafferentation were compared with those from 19 normals. All subjects were tested while seated in the upright position. Group means and two-tailed 95% confidence intervals are reported for the pitch-vVOR gains in normal and unilateral vestibular deafferented su

Unusual visual stimulation in dynamic balance conditions: proposal for a space motion sickness test.

We previously demonstrated the efficiency of normal vision/unusual vestibular cues conflict to induce motion sickness. In the present study, we investigate whether, inversely, unusual visual information/normal vestibular function conflict also elicited motion sickness. The experiments were again carried out in dynamic balance conditions to increase proprioceptive input. Circular translation of the visual field with diplopia were produced by rotating Fresnel prismatic glasses. The stimulation triggered SMS-like symptoms and dynamic balance disturbance. A positive relationship was found between

Changes in the central nervous system and their clinical correlates during long-term spaceflight.

The purpose of this paper is to review the metabolic, neurotransmitter, and morphological changes that occur in the human central nervous system (CNS) during long-duration spaceflight. If there are to be either permanent space habitats or long-duration interplanetary missions, we must determine if there will be any detrimental reversible or irreversible effects on the brain from this prolonged exposure to space. We must also determine how these effects might manifest themselves in performance, psychological, or cognitive dysfunction. The space environment has many characteristics, including mi

Synergies and strategies underlying normal and vestibulary deficient control of balance: implication for neuroprosthetic control.

Future developments of neuroprosthetic control will probably permit locomotion and posture to be maintained without the aid of crutches and will therefore require some form of balance control. Three fundamental questions will arise. First, the question of the location of imbalance-sensing transducers must be assessed. Secondly, the synergy, which is the relative amplitude and timing of muscle activity, and/or the strategy of joint torques required to re-establish a stable posture for different types of balance disturbances must be addressed. Thirdly, the control laws that map either trunk musc

Rhombomere-specific origin of the contralateral vestibulo-acoustic efferent neurons and their migration across the embryonic midline.

The bilateral efferent supply to the inner ear receptor fields is located in the hindbrain. In ovo injections of Dil into the common facial/vestibulo-acoustic nerve root at 3 days of chick development (stage 16) followed by analysis at 7 days has revealed the origin of the contralateral efferent neurons of the inner ear and their relation to the transient hindbrain rhombomeres. These neurons have a rhombomere 4-specific origin and form their commissure not by axonal outgrowth but, unusually, by transmedian cell migration into the contralateral rhombomere 4 and rhombomere 5. Neurons first proje

Hydrostatic pressure in the inner ear fluid compartments and its effects on inner ear function.

The present study summarizes the experimental findings obtained on the pressure in the inner ear fluids and on the effects of pressure changes on cochlear function in the guinea pig. Two types of pressures have to be distinguished in the inner ear fluid compartments: (i) hydrostatic fluid pressure and (ii) superimposed hydrodynamic high frequency (> 100 Hz) sound pressure oscillations. Hydrostatic pressure in the inner ear fluids in guinea pigs is in the order of 200 Pa (2 cm H2O) and shows slow (< 5 Hz) respiratory and pulsatory oscillations as well as considerable physiological variations in

Central projections from singular parts of the vestibular labyrinth in the guinea pig.

Primary afferent projections from singular parts of the vestibular labyrinth were studied in the guinea pig. The posterior ampullary nerve, the common trunk of the anterior and lateral ampullary nerves, as well as fibers innervating the macula sacculi or the macula utriculi were traced with crystals of horseradish peroxidase (HRP) lyophilisate. Posterior, as well as anterior and lateral ampullary fibers were found to project extensively to the superior vestibular nucleus, but also reached the other main vestibular nuclei. Saccular fibers projected mainly to the lateral parts of the lateral ves
