AIM: To evaluate the use of intraosseous lines for rapid vascular access in primary resuscitation of preterm and full term neonates.
METHODS: Thirty intraosseous lines were placed in 27 newborns, in whom conventional venous access had failed.
RESULTS: All the neonates survived the resuscitation procedure, with no long term side effects.
CONCLUSION: Intraosseous infusion is quick, safe, and effective in compromised neonates.

Perinatal asphyxia (PA) produces changes in nitric oxide synthase (NOS) activity in neuronal and endothelial cells of the striatum and neocortex. Creatine kinase isoenzyme BB was determined in cerebrospinal fluid (CSF) in 79 preterm neonates using an original enzyme-linked immunosorbent assay. The criterion for inclusion was an Apgar score of 7 or less at 5 min of life. Neurological examination was performed on day 2 and day 5 of life. CSF was obtained on the same days. The Jamaican Perinatal Mortality Survey compared all 2069 perinatal deaths occurring during the 12 months between 1 September 1986 and 31 August 1987 with 10,086 survivors born in the 2 months of September and October 1986. Three groups of preterm infants were studied longitudinally at 14, 20, and 26 weeks of age (corrected for gestational age). The perinatal histories of 46 children with idiopathic hypopituitarism were assessed in order to define the relationship between perinatal insult and the development of hypopituitarism. Compared to normal control siblings, the pregnancies resulting in hypopituitary children were complicated by a significantly higher incidence of vaginal bleeding at various times during gestation (13 pregnancies). Twenty-four percent of the hypopituitary children were delivered by breech, three times the incidence of control siblings and seven times the incidence of breech deliverery in the general population.

In 129 children convulsive states were examined with the use of a set of additional examination methods. The age structure of the convulsions and their course in young children are described. Different origin of the convulsive states is shown. The role of pre- and perinatal factors, mineral metabolism disturbances, infectious and other diseases, as well as of aggravated heredity is demonstrated. The importance of the premorbid background for the formation of the character and the course of the convulsive seizures in children, and for the formation of epileptiform syndrome is stressed.
