BACKGROUND: Traumatic brain injury (TBI) is a leading cause of mortality and long-term disability worldwide, with limited disease-modifying therapeutic options. Preclinical studies suggest a neurotrophic role for vitamin B12 in neuronal repair and functional recovery. In this setting, the metabolic, neurotrophic and nutritional properties of vitamin B12 may play a crucial role.
CASE-PRESENTATION: We report the case of a 47-year-old male presenting with severe TBI following a road traffic accident, presenting with a severe TBI at admission and leading to diffuse axonal injury. After initial st

Mild traumatic brain injury (mTBI) is one of the most common neurological injuries, yet its diagnosis remains challenging because of heterogeneous case definitions, the limited sensitivity of conventional imaging, and the nonspecific nature of post-traumatic symptoms. Computed tomography is widely used in acute trauma care to exclude life-threatening intracranial lesions, but it is often normal in patients with mTBI, including those with persistent symptoms. Conventional magnetic resonance imaging (MRI) may improve lesion detection, although many patients still show no clear abnormalities desp

Traumatic brain injury (TBI) results in diffuse white matter (WM) damage, which is a major determinant of death and long-term disability. Irisin, a myokine with promising neuroprotective effects, but whether irisin modulates WM injury after TBI remains unknown. Using a controlled cortical impact (CCI) mouse model, we evaluated post-TBI irisin expression and its protective effects on neurological function and WM integrity. Multiple experimental approaches, such as RT-qPCR, Liperfluo staining, Western blot, and transmission electron microscopy, were employed to assess oligodendrocyte precursor c

INTRODUCTION: Neuropsychiatric systemic lupus erythematosus (NPSLE) is a highly heterogeneous and clinically challenging manifestation of systemic lupus erythematosus, encompassing a broad spectrum of diffuse and focal neuropsychiatric symptoms. Conventional models have emphasized vascular injury and blood-brain barrier (BBB) dysfunction; however, these frameworks alone do not fully account for phenotypic diversity, frequent absence of overt structural lesions, or variable associations with systemic disease activity.
AREAS COVERED: This review summarizes recent advances in the understanding o

Concussions, classified as a type of mild traumatic brain injury (mTBI), are frequently underdiagnosed due to the subjective nature of symptoms and limitations in existing diagnostic methodologies. Current clinical evaluations, including tools such as the Sport Concussion Assessment Tool 5 (SCAT5), Balance Error Scoring System (BESS), and Vestibular Ocular Motor Screening (VOMS), demonstrate high sensitivity and specificity but often fail to capture the full complexity of concussive injuries. Emerging diagnostic approaches, such as blood biomarkers (for example, glial fibrillary acidic protein

The use of extracorporeal membrane oxygenation (ECMO) to support patients with cardiac arrest, cardiogenic shock, and acute respiratory distress syndrome is rising worldwide.1 While ECMO may save the lives of some of our sickest patients, the outlook of ECMO survivorship remains uncertain. Defining longer-term functional and neuropsychiatric outcomes in ECMO survivors is important for 3 reasons. First, critically ill patients are at high risk of experiencing postintensive care syndrome (PICS), defined as new physical, cognitive, or psychological impairments that present in survivors of critica

As of April 2020, the coronavirus 2019 (COVID-19) pandemic has resulted in more than 210,000 deaths globally. The most common cause of death from COVID-19 is acute respiratory failure. We report the case of a 78-year-old female with a history of hypertension, cerebrovascular accident (CVA), type 2 diabetes mellitus, and sarcoidosis, who presented to the emergency department with one day of dyspnea. The patient experienced a rapid decline in respiratory function and was intubated in the intensive care unit (ICU), meeting the Berlin criteria for severe acute respiratory distress syndrome (ARDS).

OBJECTIVE: To determine whether ascending arousal network (AAn) connectivity is reduced in patients presenting with traumatic coma.
METHODS: We performed high-angular-resolution diffusion imaging in 16 patients with acute severe traumatic brain injury who were comatose on admission and in 16 matched controls. We used probabilistic tractography to measure the connectivity probability (CP) of AAn axonal pathways linking the brainstem tegmentum to the hypothalamus, thalamus, and basal forebrain. To assess the spatial specificity of CP differences between patients and controls, we also measured C

OBJECTIVE: Symptomatic cerebral fat embolism (CFE) is a rare complication that occurs after a traumatic injury or orthopaedic surgery and is diagnostically challenging. No data is currently available concerning long-term follow-up.
METHODS: We identified from medical records 9 patients with CFE and revised the clinical signs and the diagnostic process. We then analysed long-term follow-up data, targeting clinical course after discharge, neurological impairment, and current quality of life, using the Barthel index and the modified Rankin Scale.
RESULTS: All 9 patients initially showed severe

A case of malignant lymphoma in the skull after head injury associated with whole bone metastasis is reported. The patient was a 66-year-old man who was admitted to Almeida Memorial Hospital because of headache and general fatigue 2 months after head injury. After admission tumors appear in the frontal and occipital region and grew rapidly. Plain craniogram revealed large map-like bone destructions and multiple punched out lesions. Bone scintigram with 99mTc-MDP revealed multiple accumulations of RI in the skull, vertebrae, ribs and pelvis. CT scan revealed destructive, markedly enhancing bone

BACKGROUND AND PURPOSE: Intraventricular hemorrhage (IVH) is a common complication of prematurity that results in neurological sequelae, including cerebral palsy, posthemorrhagic hydrocephalus, and cognitive deficits. Despite this, there is no standardized animal model exhibiting neurological consequences of IVH in prematurely delivered animals. We asked whether induction of moderate-to-severe IVH in premature rabbit pups would produce long-term sequelae of cerebral palsy, posthemorrhagic hydrocephalus, reduced myelination, and gliosis.
METHODS: The premature rabbit pups, delivered by cesarea

BACKGROUND: Distinguishing between tumor progression and radiation necrosis during brain tumor follow-up is challenging. Advanced MR imaging techniques, such as perfusion, diffusion-weighted imaging (DWI), and spectroscopy, can help differentiate between these conditions. In our clinic, we established a novel technique, the rapid washout and late-enhancement map. This study investigated the value of this map in distinguishing true progression from radiation necrosis.
METHODS: Patients with a histologically confirmed initial diagnosis of glioblastoma, high-grade astrocytoma, primary central ne

AIM: Replacement of aneurysm clips or temporary parent artery occlusion during aneurysm clipping (AC) carries the risk of inducing postoperative neurologic deficits. When studying the risk of surgical complications associated with cerebral aneurysms, patients with similar conditions should be compared to eliminate the influence of rupture and location of aneurysm.
MATERIAL AND METHODS: We used 3.0-Tesla (3.0T) magnetic resonance diffusion-weighted imaging (DWI) and magnetic resonance angiography (MRA) to analyze surgical complications after AC. A total of 42 AC procedures for 40 unruptured an

Traumatic brain injury can result in persistent cognitive, behavioural, and emotional deficits, with the hippocampus among the most vulnerable circuits after injury. However, how diffuse injury differentially alters hippocampal subregions across time remains incompletely defined. Here, we used a mouse closed-head injury model to characterize early transcriptomics, subacute-to-chronic electrophysiology, dendritic spine morphology, and delayed immunoreactivity for glial fibrillary acidic protein (GFAP), ionized calcium-binding adapter molecule 1 (IBA1), and the pan-leukocyte marker CD45. Immunosuppression of unknown aetiology is a hallmark feature of glioblastoma and is characterized by decreased CD4 T-cell counts and downregulation of major histocompatibility complex class II expression on peripheral blood monocytes in patients. This immunosuppression is a critical barrier to the successful development of immunotherapies for glioblastoma. We recapitulated the immunosuppression observed in glioblastoma patients in the C57BL/6 mouse and investigated the aetiology of low CD4 T-cell counts. We determined that thymic involution was a hallmark feature of immunosuppression in three
