Manual aspiration thrombectomy for basilar infarction in the setting of a persistent primitive hypoglossal artery: Case report and review of the literature.

BACKGROUND: Persistent primitive hypoglossal artery (PPHA), a remnant of embryonal circulation, is a rare variant of the posterior cerebral circulation. Seven prior cases of posterior circulation stroke in the setting of PPHA have been described in the literature, with all but one case being attributable to atherosclerotic embolization from the internal carotid artery (ICA) through the PPHA. CASE DESCRIPTION: We report a unique case of a young male with a PPHA presenting with a "top of the basilar" syndrome following the repair of his atrial septal defect who underwent emergent revascularizat

Management of carotid restenosis.

Carotid endarterectomy is the preferred method for cerebral revascularization in patients with symptomatic and asymptomatic high-grade extracranial carotid artery stenosis. Carotid artery stenting has recently emerged as a less invasive alternative to endarterectomy. Carotid stenting has been demonstrated to be technically feasible and safe in high-risk patients with current data indicating clinical equipoise with respect to endarterectomy. It is clear that carotid stenting will continue to be performed at increasing rates after these encouraging outcomes. Therefore, it is anticipated that the…

Functional connectivity analysis and prediction of cognitive change after carotid artery stenting.

OBJECTIVE: The neurocognitive course of patients who have undergone cerebral revascularization has been the subject of many studies, and the reported effects of carotid artery stenting (CAS) on cognitive function have varied from study to study. The authors hypothesized that cognitive amelioration after CAS is associated with alteration of the default mode network (DMN) connectivity, and they investigated the correlation between functional connectivity (FC) of the DMN and post-CAS changes in cognitive function in order to find a clinical marker that can be used to predict the effect of cerebra…

Early Detection of Cerebral Embolism During Transcatheter Aortic Valve Replacement Facilitated by Rapid Awakening Through the Antagonism of Remimazolam With Flumazenil: A Case Report.

The combination of remimazolam and flumazenil facilitates the rapid acquisition of neurological findings immediately after awakening from anesthesia, potentially improving the likelihood of early detection of neurological complications during anesthesia. Cerebral infarction is a serious complication of transcatheter aortic valve replacement (TAVR), which can significantly impact a patient's quality of life and activities of daily living. Although complete prevention of cerebral infarction remains challenging, early detection and prompt treatment can improve the neurological prognosis. We prese

Predictive value of the hemispheric magnetic resonance angiography score on the development of indirect pial synangiosis after combined revascularization surgery for adult moyamoya disease.

PURPOSE: It is difficult to precisely predict indirect bypass development in the context of combined bypass procedures in moyamoya disease (MMD). We aimed to investigate the predictive value of magnetic resonance angiography (MRA) signal intensity in the peripheral portion of the major cerebral arteries for indirect bypass development in adult patients with MMD. METHODS: We studied 93 hemispheres from 62 adult patients who underwent combined direct and indirect revascularization between 2005 and 2019 and genetic analysis for RNF213 p.R4810K. The signal intensity of the peripheral portion of t

Long-Term Results of Autologous Bifurcated Carotid Artery Reconstructions.

BACKGROUND: Carotid bifurcation revascularization using interposition grafts is rare. While internal carotid artery (ICA) revascularization is regarded as mandatory, the external carotid artery (ECA) is severed in most instances. Long-term results of an autologous bifurcated carotid artery reconstruction are discussed. METHODS: Single-center, retrospective analysis of a consecutive series of patients treated at an academic vascular surgery center. RESULTS: From December 2006 to November 2019, fifty-one patients underwent reconstruction of the carotid artery using an autologous bifurcated int

Mapping altered brain connectivity and its clinical associations in adult moyamoya disease: A resting-state functional MRI study.

Detection of subtle ischemic injuries in moyamoya disease may enable optimization of timing of revascularization surgery, and could potentially improve functional outcomes. Resting-state functional magnetic resonance imaging (rs-fMRI) is widely used to study functional organization of the brain, but it remains unclear whether rs-fMRI could elucidate distinct characteristics in moyamoya disease. Here, we aimed to determine changes in a conventional rs-fMRI measure and analyze any associations with clinical symptoms and cerebral hemodynamics. Thirty-one adults with moyamoya disease and 25 adult

Dynamic cerebral autoregulation in carotid stenosis before and after carotid stenting.

BACKGROUND: Impaired dynamic cerebral autoregulation (DCA) has been shown in patients with severe (> or =70%) internal carotid artery (ICA) stenosis, but DCA in moderate (50% to 69%) ICA stenosis, especially its response to carotid revascularization, has rarely been reported. Our study aimed to characterize DCA in severe and moderate ICA stenosis before and after carotid stenting. METHODS: This study included 21 patients with ICA stenosis > or =50% who received carotid stenting. Progressive occlusion of the middle cerebral artery as the main feeder in a cerebral arteriovenous malformation: A case report.

BACKGROUND: Cerebral arteriovenous malformation (AVM) is a congenital condition characterized by abnormal direct connections between arteries and veins through a nidus. Occlusion of major feeding arteries in AVMs is rare and typically associated with moyamoya disease. To date, only three cases have been reported where a previously patent major feeding artery became occluded during follow-up in AVMs without moyamoya disease. CASE DESCRIPTION: A 36-year-old man presented with a seizure and was diagnosed with a left temporal lobe AVM. He was treated conservatively with antiepileptic medication.

Microneurosurgical treatment of a small perimesencephalic pure pial arterial malformation: an under-recognized etiology of angiographically occult subarachnoid hemorrhage. Illustrative case.

BACKGROUND: Pial arterial malformations (PAMs) are rare vascular lesions consisting of dilated tortuous arteries without venous drainage. Current PAM understanding is limited by the lesion's rarity, limited anatomopathological studies, and frequent misclassifications. OBSERVATIONS: A 23-year-old male experienced two spontaneous subarachnoid hemorrhages (SAHs) over 6 months with initially unremarkable diagnostic cerebral angiograms. Magnetic resonance imaging (MRI) and angiography after the second SAH revealed a small perimesencephalic ovoid lesion within the left crural cistern, between the l

Arterial Dissection Following the Use of Remote Aspiration Thrombectomy.

Remote aspiration thrombectomy using a balloon guide catheter for acute carotid artery occlusion has been proposed as a safe and effective technique. We present a case of iatrogenic arterial dissection of the distal cervical segment in a patient with proximal vessel occlusion who underwent attempted revascularization using this strategy. A 57-year-old male patient presented with computed tomography (CT) angiogram evidence of a left carotid terminus and M1 segment occlusion. The patient was taken emergently for mechanical thrombectomy. Remote aspiration thrombectomy was attempted twice using ma

Foramen spinosum and middle meningeal artery in moyamoya disease: Preliminary results of a pilot study.

OBJECT: This study investigated whether the size of the middle cranial fossa foramina reflects the severity of moyamoya disease (MMD). METHODS: It compared 20 adult patients managed without surgical revascularization with 2 age- and sex-matched controls. MR angiography scores were assigned by the severity of occlusive changes of the internal carotid artery, the middle cerebral artery, the anterior and the posterior cerebral arteries and the signals of the distal branches of these arteries. These scores were stratified into MR angiography grades (Houkin's grade: 1-4). The relationships between

Mechanism of acute ischemic stroke in patients with severe middle cerebral artery atherosclerotic disease.

BACKGROUND: Intracranial atherosclerotic disease (ICAD) is one of the most common causes of ischemic stroke worldwide. Although the pathogenesis of cerebral infarct in ICAD has been reported from autopsy series, the mechanism of stroke is not well known. This study used baseline perfusion imaging and diffusion-weighted imaging (DWI) or computerized tomography (CT) imaging to help identify the mechanism of stroke in ICAD involving the middle cerebral artery (MCA). METHODS: We retrospectively reviewed baseline CT or magnetic resonance (MR) perfusion studies and diffusion-weighted MR imaging or

Marked improvement in vessel wall enhancement after direct bypass surgery for moyamoya disease.

PURPOSE: Intracranial vessel wall enhancement (VWE) on MR imaging was associated with progression and poor outcome of moyamoya disease (MMD). However, it remains controversial how the signal intensity (SI) of VWE fluctuates in the perioperative period. The purpose of this study was to investigate the change in the SI of VWE between before and after direct bypass surgery for MMD. METHODS: Subjects undergoing direct bypass surgery for MMD were prospectively recruited. All subjects underwent pre/post operative 3T-MR imaging including pre/post contrast vessel wall imaging in the terminal of intern
