[Trigeminal neuralgia-how patients experience their condition disease.].

INTRODUCTION: Effective treatment of patients with trigeminal neuralgia is often a long and complicated procedure. The symptoms of trigeminal neuralgia are clearly defined in most cases. Sudden and brief episodes of severe and stabbing pain (tic douloureux) occur, with pain usually starting from a trigger point. Recent reports suggest 80-90% suppression of pain with various treatment regimens, which seems to indicate that the diagnosis and successful treatment of the disorder are no longer a major problem. In fact, however, the intense suffering of patients and isolated reports in the literatu

Long-stay patients in short-stay hospitals.

In 1980 and 1990, only 4-5 percent of patients in short-stay hospitals were hospitalized for more than 3 weeks. The number of discharges and days of care for these long-stay patients were lower in 1990 as compared with 1980, but they continued to use more than a quarter of all hospital days in 1990. Long-stay patients were more likely than all patients to be 65 years of age and over and have Medicare as their expected principal source of payment. For long-stay and all patients, private insurance covered smaller proportions of discharges and days of care, and Medicaid covered larger proportions

Elective laparoscopically assisted sigmoidectomy for the sigmoid volvulus.

BACKGROUND: The laparoscopic approach for the treatment of sigmoid volvulus has been a rare surgical indication. This phase 2 study investigated the feasibility and surgical outcomes of elective laparoscopic surgery for sigmoid volvulus. METHODS: Patients with sigmoid volvulus were first offered colonoscopic decompression for their acute colonic obstruction. If the colonic decompression was successful, complete bowel preparation was performed, followed by elective laparoscopically assisted sigmoidectomy. The details of the laparoscopic procedures are shown in the video. Briefly, the redundant

Should We Worry? Clinical Characteristics and Outcomes of Pediatric Traumatic Arthrotomies.

BACKGROUND: Traumatic arthrotomies are rare, urgent injuries that carry high presumed risks for joint contamination and septic arthritis (SA). Most of these injuries occur in the knee. Like open fractures, treatment typically involves prompt antibiotic administration followed by surgical irrigation, debridement, and closure. Unlike open fractures, there is sparse evidence guiding pediatric arthrotomy management. This study aimed to characterize the demographics, clinical management, and outcomes of pediatric traumatic arthrotomies of the knee and other joints. METHODS: Children with confirmed

Health Disparities in Patients With Musculoskeletal Injuries: Food Insecurity Is a Common and Clinically Challenging Problem.

BACKGROUND: Health disparities have important effects on orthopaedic patient populations. Socioeconomic factors and poor nutrition have been shown to be associated with an increased risk of complications such as infection in patients undergoing orthopaedic surgery. Currently, there are limited published data on how food insecurity is associated with medical and surgical complications. QUESTIONS/PURPOSES: We sought to (1) determine the percentage of patients who experience food insecurity in an orthopaedic trauma clinic at a large Level 1 trauma center, (2) identify demographic and clinical fa

Optimizing outcomes in redo hypospadias repair: The impact of perioperative hyperbaric oxygen therapy on tissue healing.

INTRODUCTION: Redo hypospadias repairs present significant challenges due to tissue scarring and hypovascularity, substantially increasing the risk of complications. Previous literature document complication rates above 40 % after three or more previous urethroplasties, highlighting the need for strategies that enhance tissue quality. Postoperative hyperbaric oxygen therapy (HBOT) can improve healing outcomes. However, the role of perioperative HBOT in enhancing tissue quality through neovascularization remains unclear. This study aims to evaluate whether perioperative HBOT (both pre- and post

Characterizing the Natural History of Pediatric Brain Tumors Presenting with Metastasis.

BACKGROUND: The natural history of pediatric patients with metastasis of primary brain tumors within and outside the central nervous system is poorly understood, as too are possible clinical correlates with outcome. Correspondingly, the aim of this study was to interrogate a national database to characterize this diagnosis and its clinical course in pediatric patients. METHODS: The U.S. National Cancer Database (NCDB) was interrogated between the years 2005-2016 for all patients aged 18 years and younger with a primary brain tumor diagnosis, as well as evidence of disease metastasis at initia

Patient satisfaction and its associated factors with preoperative anesthetic evaluation services at public hospitals in Nekemte town, Nekemte, Ethiopia 2024: - cross-sectional study.

BACKGROUND: Patient satisfaction with healthcare services is increasingly recognized as a reliable indicator of treatment quality. The safety and effectiveness of surgical treatments are greatly dependent on the preoperative anesthetic evaluation. It entails evaluating the patient's health, medical background, and other variables that affect the administration of anesthesia and the outcome of surgery. Because it has a direct impact on patient trust, compliance, and the general perception of the quality of care, patient satisfaction with this procedure is crucial. This study aimed to assess pat

Cost-effectiveness of a bone substitute delivering gentamicin in the treatment of chronic osteomyelitis of long bones: Protocol for the CONVICTION randomized multicenter study.

INTRODUCTION: Chronic osteomyelitis is a serious osteoarticular infection that most often occurs in the long bones, responsible for significant morbidity with the risk of fracture and amputation. Despite advances in both antibiotics and surgical treatment, the probability of recurrence of infection remains at around 20%. Cerament-G (BONESUPPORT AB, Sweden) is a synthetic bone substitute that fills the bone void left by surgery, prevents infection and promotes bone regeneration within this space. Cerament-G also provides the local delivery of high doses of gentamicin over several weeks. Two pro

Out-of-Pocket Costs and Provider Payments in Cleft Lip and Palate Repair.

BACKGROUND: As healthcare spending within the United States grows, payers have attempted to curb spending through higher cost sharing for patients. For families attempting to balance financial obligations with their children's surgical needs, high cost sharing could place families in difficult situations, deciding between life-altering surgery and bankruptcy. We aim to investigate trends in patient cost sharing and provider payments for cleft lip and palate repair. METHODS: The IBM® MarketScan® Commercial Database was queried to extract patients younger than 18 years who underwent primary or

[Active epidemiological surveillance and prevention of diseases caused by biomechanical overload of the upper limbs: experience at a territorial service of occupational medicine].

Nowadays in the western countries, the Work Musculoscheletal Disorders (WMSDs) are the most relevant work-related diseases either for the large number of at-risk workers and for the high prevalence of cases among them. Among WMSDs, the Carpal Tunnel Syndrome (CTS) is considered a sentinel event in occupational medicine and epidemiological surveillance systems were set up to study its temporal occurrence and spatial spread. Surprisingly enough, in our country the epidemiology of the WMSDs is still unknown; although observations of important clusters of cases are increasing, these pathologies ar

Association between postoperative length of stay after cesarean delivery and postpartum maternal complications.

INTRODUCTION: There is no standard for how long a patient's postpartum stay should be after cesarean delivery. There are known patient- and system-level benefits of shortening the postoperative length of stay, and data are mixed on the relationship between maternal length of stay after cesarean deliveries and post-discharge complications. Our objective was to examine whether postoperative length of stay after cesarean delivery was associated with postpartum maternal complications. METHODS: We conducted a retrospective cohort study of all cesarean deliveries in 2019 in the National Surgical Qu

Trends in Opioid Prescribing and New Persistent Opioid Use After Surgery in the United States.

OBJECTIVE: To define recent trends in opioid prescribing after surgery and new persistent opioid use in the United States. BACKGROUND: New persistent opioid use after surgery among opioid-naive individuals has emerged as an important postoperative complication. In response, initiatives to promote more appropriate postoperative opioid prescribing have been adopted in recent years. However, current estimates of opioid prescribing and new persistent opioid use following surgery remain unknown. METHODS: A retrospective cohort study of opioid-naive privately insured adult patients undergoing 17 c

Epidemiological Analysis of Changes in Clinical Practice for Full-Thickness Rotator Cuff Tears From 2010 to 2015.

BACKGROUND: Rotator cuff injuries are a leading cause of shoulder disability among adults. Surgical intervention is a common treatment modality; however, conservative management has been described for the treatment of rotator cuff tears. As the cost of health care increases, the industry has shifted to optimizing patient outcomes, reducing readmissions, and reducing expenditure. In 2010, the American Academy of Orthopaedic Surgeons created clinical practice guidelines (CPGs) to guide the management of rotator cuff injuries. Since their publication, there have been several randomized controlled
