Effects of mitomycin-C on tear film, corneal biomechanics, and surface irregularity in mild to moderate myopic surface ablation: preliminary results.

PURPOSE: To assess the effect of mitomycin-C (MMC) on the tear film, corneal biomechanics, and surface irregularity in surface ablation (photorefractive keratectomy [PRK]) for low to moderate myopia. SETTING: Refractive Surgery Unit, Farabi Eye Hospital, Tehran, Iran. DESIGN: Double-masked randomized clinical trial. METHODS: In patients with spherical equivalent myopia of -0.75 to -3.87 diopters (D) and astigmatism up to -1.75 D, the first eye was randomly assigned to the application of MMC 0.02% or a balanced salt solution for 15 seconds. The fellow eye received the alternate in a masked f

Beveled femtosecond laser astigmatic keratotomy for the treatment of high astigmatism post-penetrating keratoplasty.

PURPOSE: To use beveled femtosecond laser astigmatic keratotomy (FLAK) incisions to treat high astigmatism after penetrating keratoplasty. METHODS: Paired FLAK incisions at a bevel angle of 135 degrees, 65% to 75% depth, and arc lengths of 60 to 90 degrees were performed using a femtosecond laser. One case of perpendicular FLAK was presented for comparison. Vector analysis was used to calculate the changes in astigmatism. Fourier domain optical coherence tomography was used to examine incision morphology. RESULTS: Wound gaping requiring suturing was observed in the case of perpendicular FLAK

Fourth-generation fluoroquinolone-resistant bacterial keratitis after refractive surgery.

We report the first 2 cases of bacterial keratitis resistant to fourth-generation fluoroquinolones after laser in situ keratomileusis (LASIK) and photorefractive keratectomy (PRK). The first patient had Pseudomonas aeruginosa keratitis after PRK despite treatment with moxifloxacin. The second patient was on gatifloxacin post-LASIK when she had methicillin-resistant Staphylococcus aureus (MRSA) keratitis diagnosed. In both cases, culture susceptibilities showed isolates resistant to moxifloxacin and gatifloxacin, and treatment with topical aminoglycosides and surgical intervention was necessary

Refractive outcome of keratoconus treated by combined femtosecond laser and big-bubble deep anterior lamellar keratoplasty.

PURPOSE: To report 1-year follow-up in 11 of 13 eyes with keratoconus treated by deep anterior lamellar keratoplasty with a combined femtosecond laser lamellar resection followed by a big-bubble dissection. METHODS: Thirteen eyes with keratoconus were treated. Recipient and donor were prepared with the 60-kHz IntraLase femtosecond laser (Abbott Medical Optics). In the recipient, the femtosecond laser, after performing a lamellar cut 100 μm above the thinnest corneal point (measured by Pentacam [Oculus Optikgeräte GmbH]), was used to make a mushroom-shaped resection (anterior diameter, 9 mm; p

Haze after photorefractive keratectomy caused by iatrogenic lagophthalmos.

A 22-year-old man had shortening of the levator muscle for a congenital blepharoptosis in the right eye in 2000. In September 2004, he was successfully treated by bilateral photorefractive keratectomy (PRK) for myopia and was asymptomatic with an uncorrected visual acuity of 20/20. In October 2005, right eye visual acuity decreased because of an inferiorly localized haze caused by nocturnal lagophthalmos. Fluorometholone eyedrops and lubrication induced full visual recovery after 2 months, but corresponding topographical abnormalities were only partially improved. Corneal exposure can induce h

PURPOSE: To evaluate the accuracy of modern intraocular lens (IOL) power calculation formulas in eyes with previous myopic laser vision correction (LVC), and to assess the impact of axial length (AL) on the formula performance. METHODS: A total of 108 eyes were included, with 52 eyes in the AL less than 28 mm group and 56 eyes in the AL 28 mm or greater group. Loss and absence of melanocytes due to a number of factors is responsible for vitiligo; known to be the commonest disorder of pigmentation. The aim of the current work was to compare the efficacy and safety of excimer light with topical tacrolimus ointment 0.1% versus excimer light with topical bimatoprost gel 0.01% in treatment of facial vitiligo. The study was carried out on 48 patients presented with facial vitiligo. The patients were divided randomly using sealed envelope method into two groups (24 patients each). Group 1 were treated with excimer light plus topical tacrolimus ointment 0.1

Factors Influencing Cyclotorsion During Photorefractive Keratectomy.

PURPOSE: To determine predictive factors for intra-operative cyclotorsion in photorefractive keratectomy (PRK). METHODS: A retrospective statistical analysis of medical records pertaining to 3,996 eyes undergoing PRK was conducted. Outcome measures of this study were the likely existence of statistically significant relations between diverse and potentially influential factors and the occurrence of intraoperative cyclotorsion. RESULTS: A total of 96% of examined medical records indicated some degree of cyclotorsion with the absolute mean intraoperative value of 1.38° ± 1.67° (range: 0° to 13
