Mandibular distraction osteogenesis in the treatment of pediatric temporomandibular joint ankylosis with micrognathia and obstructive sleep apnea syndrome: A case report with 4-year follow-up.

Temporomandibular joint (TMJ) ankylosis in pediatric patients is rare and may cause severe micrognathia and obstructive sleep apnea syndrome. The present study reports on the treatment and 4-year follow-up of a pediatric patient with early-onset bilateral TMJ ankylosis and severe secondary micrognathia, as well as obstructive sleep apnea syndrome. A typical 'bird face' appearance was noted with severe mandible retrognathism and a significant convex facial profile. The treatment of this patient involved TMJ ankylosis release with condylectomy and simultaneous bilateral mandibular distraction os

Applications of stem cells in orthodontics and dentofacial orthopedics: Current trends and future perspectives.

A simple overview of daily orthodontic practice involves use of brackets, wires and elastomeric modules. However, investigating the underlying effect of orthodontic forces shows various molecular and cellular changes. Also, orthodontics is in close relation with dentofacial orthopedics which involves bone regeneration. In this review current and future applications of stem cells (SCs) in orthodontics and dentofacial orthopedics have been discussed. For craniofacial anomalies, SCs have been applied to regenerate hard tissue (such as treatment of alveolar cleft) and soft tissue (such as treatmen

A current overview of materials and strategies for potential use in maxillofacial tissue regeneration.

Tissue regeneration is rapidly evolving to treat anomalies in the entire human body. The production of biodegradable, customizable scaffolds to achieve this clinical aim is dependent on the interdisciplinary collaboration among clinicians, bioengineers and materials scientists. While bone grafts and varying reconstructive procedures have been traditionally used for maxillofacial defects, the goal of this review is to provide insight on all materials involved in the progressing utilization of the tissue engineering approach to yield successful treatment outcomes for both hard and soft tissues.

Pierre Robin sequence: Subdivision, data, theories, and treatment - Part 4: Recommended management and treatment of Pierre Robin sequence and its application.

CONTEXT: The disorder currently accepted as Pierre Robin syndrome/anomaly/sequence (PRS) has been plagued by controversy ever since initially being described. Controversy exists not only about the appropriate terminology and etiopathogenesis of the disorder but also about its management. Therefore, clinical findings and treatment outcomes of a large database of 266 PRS cases were compared with the current state of knowledge in the scientific literature related to history, clinical description, diagnostic criteria, epidemiology, theories of oligohydramnios, mandibular catch-up growth, midfacial

Double Free Flap Transfer using a Vascularized Free Fibular Flap and a Rectus Abdominalis Musculocutaneous Flap for an Extensive Oromandibular Defect: Prevention of Sinking or Drooping of the Flap With an Anterior Rectus Sheath.

The double free flap procedure is a preferred treatment for extensive composite defects of the oromandibular area. In this procedure, the choice and use of the flaps are both important. Flaps with adequate soft tissue are required to fill the extensive dead space for huge oromandibular defects. Such flaps, however, tend to sink and droop with time because of gravity, resulting in poor functional and aesthetic results. Here, the authors describe a procedure that avoids flap sinking and drooping, using a vascularized fibular osteocutaneous flap, which is well established for mandibular bone defe

Temporomandibular joint osteochondritis dissecans: case report.

BACKGROUND: Osteochondritis dissecans is a process in which segment(s) of cartilage separate from an articular surface. It is most often reported in the larger joints of the body, including the knee, elbow, hip, wrist, and ankle. Involvement of the temporomandibular joint (TMJ) is exceedingly rare. OBJECTIVES: To describe a probable case of osteochondritis dissecans and discuss the differential diagnosis for this condition. STUDY DESIGN: In addition to clinical examination the patient was imaged using panoramic radiography, computed tomography and magnetic resonance imaging. RESULTS: Panora

Suction-assisted lipectomy for lipodystrophy syndromes attributed to HIV-protease inhibitor use.

The addition of HIV-protease inhibitors to the arsenal of therapies for the treatment of HIV infection has resulted in significant suppression of viral load such that HIV-positive individuals experience reduced morbidity and extended life expectancy. Recently, a number of syndromes have been described involving abnormal fat distribution that may be associated with prolonged HIV-protease inhibitor therapy. These syndromes include hypertrophy of the cervicodorsal fat pad ("buffalo hump"); a tendency toward increased central adiposity ("protease paunch"); adiposity in the submental, mandibular, a

Assessment and treatment of facial deformity resulting from radiation to the orbital area in childhood.

Children who receive radiation for malignant tumors in the orbital area frequently develop widespread craniofacial deformities. These affect the skull, orbit, maxilla, and mandible. When these patients seek treatment at a later age, they require careful assessment using cephalometrics and three-dimensional imaging. It is recommended that the four levels of skeletal deformity be corrected in a single procedure, that is frontotemporal expansion with repositioning of the skull base area, orbital expansion and repositioning together with maxillary and mandibular surgery. Bone grafts should be inla

Temporomandibular joint morphology in children after treatment of condylar fractures with functional appliance therapy: a follow-up study us computed tomography.

The consequences of treating fractures of the temporomandibular joint in children by means of functional appliance therapy remain controversial. In this retrospective study we evaluated the temporomandibular joint morphology of 19 children and adolescents who had sustained a total of 21 condylar fractures between three and nine years previously and who had been treated in this way. Restoration of function was assessed clinically by measurement of mandibular movements, and palpation and auscultation of the joint. Spiral CT was used for the radiological examination with 2D and 3D reconstructions

Standardize routine angiography assessment of leg vasculatures before fibular flap harvest: lessons of congenital and acquired vascular anomalies undetected by color Doppler and physical examinations.

BACKGROUND: Fibular flaps have been widely used for mandibular and maxillary reconstructions. On occasion, anatomical variants of fibular arteries (FA) will be encountered. PURPOSE: Although anatomical variants of FA during fibular harvest have been reported, controversy exists regarding whether simple color Doppler ultrasonography (CDU) and physical examinations (PE) are sufficient for early preoperative detection. MATERIAL AND METHODS: A 10-year retrospective analysis in our department was performed to find the patients with various FA anomalies confirmed by computed tomography angiography

Sternoclavicular Joint Graft in Temporomandibular Joint Reconstruction for Ankylosis.

BACKGROUND: Temporomandibular joint ankylosis is one of the most distressing clinical condition resulting in loss of jaw function and impairment of growth. Ankylosis is a greek word meaning "Stiff Joint". It is observed that in humans the Sternoclavicular joint and Temporomandibular joint are similar morphologically and histologically as they are the only two synovial joints covered with fibrocartilage. This similarity of the joints have encouraged the surgeons to use SCG as an alternative to costochondral grafts. AIMS: The purpose of this article was to evaluate the feasibility of using ster

Intraoperative Real-Time Fluorescence Labeling of Degenerated Facial Nerves with Bevonescein.

BACKGROUND: Bevonescein (ALM-488), a nerve-targeted peptide-dye conjugate, enables intraoperative fluorescence of degenerated peripheral nerves because of its extracellular matrix-binding mechanism. In contrast, myelin-based nerve-targeting agents would not be expected to label degenerated nerves because of demyelination. The authors compare the intraoperative fluorescence of chronically degenerated murine facial nerves produced by bevonescein and myelin-binding dye oxazine-4 and discuss its utility in peripheral nerve reconstruction. METHODS: Sixteen wild-type mice underwent transection of t
