L4-to-L4 nerve root transfer for hindlimb hemiplegia after hypertensive intracerebral hemorrhage.

There is no effective treatment for hemiplegia after hypertensive intracerebral hemorrhage. Considering that the branches of L4 nerve roots in the lumbar plexus root control the movement of the lower extremity anterior and posterior muscles, we investigated a potential method of nerve repair using the L4 nerve roots. Rat models of hindlimb hemiplegia after a hypertensive intracerebral hemorrhage were established by injecting autogenous blood into the posterior limb of internal capsule. The L4 nerve root on the healthy side of model rats was transferred and then anastomosed with the L4 nerve ro

[Spinal repair of ventral root avulsions after brachial plexus injuries: Towards new surgical strategies?].

BACKGROUND AND PURPOSE: Avulsion of nerve roots associated with the brachial plexus results in dramatic lesions with a prognosis which remains poor to this day. These lesions are considered as involving the central nervous system and therefore not amenable to surgical repair. However, the results of many experiments in animals have shown that if continuity can be re-established between the cervical cord and a denervated muscle or the distal end of its nerve, spinal motor neurons can regrow into a peripheral nerve graft, ultimately leading to the restoration of functional contraction. Neuromechanical regulation of respiratory motor output in ventilator-dependent C1-C3 quadriplegics.

To evaluate the role of phrenic and sternocleidomastoid afferents as alternate sources of inhibitory feedback during mechanical ventilation, we studied five C2-C3 quadriplegics with sensory denervation of the rib cage and diaphragm, six C1-C2 quadriplegics with additional loss of sensory feedback from the neck muscles, and seven normal subjects. We compared the return of inspiratory muscle activity [the recruitment threshold (PCO2RT)] during mechanical ventilation between subject groups after stepwise increases in end-tidal PCO2 (PETCO2) either by increasing the inspired fraction of CO2 (FICO2

Skin sodium content as a predictor of blood pressure response to renal denervation.

Patients with treatment resistant hypertension (TRH) are known to have elevated sodium (Na) content in muscle and skin. Renal denervation (RDN) emerged as an adjacent therapeutic option in this group of patients. This analysis aimed at evaluating whether tissue Na content predicts blood pressure (BP) response after RDN in patients with TRH. Radiofrequency-device based RDN was performed in 58 patients with uncontrolled TRH. Office and 24-h ambulatory BP were measured at baseline and after 6 months. To assess tissue Na content Na magnetic resonance imaging (Na-MRI) was performed at baseline prio

Variability in the development of synkinesis in a rabbit facial nerve axotomy model.

OBJECTIVE: Weakness and synkinesis (involuntary cocontraction of different muscle groups) are common sequelae after facial nerve injury. We describe a rabbit model of facial nerve axotomy and repair, which can be used to study such sequelae and propose a grading tool to assess the facial movement outcomes. Using this rabbit model, we assess the effect of delaying facial nerve repair on the quality of the clinical result. METHODS: A total of 15 rabbits (30 facial halves) were divided into 4 groups: control, facial nerve main trunk axotomy and immediate repair, axotomy and repair at 2.5 weeks a

Respiratory Profile in Patients After Liver Transplantation.

BACKGROUND: Patients in the immediate post-operative liver transplantation (LxT) period can develop respiratory and functional complications. In the postoperative months, these functions tend to improve. Nevertheless, there are few studies that evaluate precisely and specifically respiratory function in post-LxT long-term after surgery. The objective of the study was to describe the respiratory profile of patients 1 to 6 months after LxT, accompanied by LxT outpatients. METHODS: We included patients between 25 and 60 years old. We excluded patients with chronic renal or cerebrovascular impair

Feasibility of catheter-based renal nerve ablation and effects on sympathetic nerve activity and blood pressure in patients with end-stage renal disease.

BACKGROUND AND OBJECTIVES: Sympathetic activation is a hallmark of ESRD and adversely affects cardiovascular prognosis. Efferent sympathetic outflow and afferent neural signalling from the failing native kidneys are key mediators and can be targeted by renal denervation (RDN). Whether this is feasible and effective in ESRD is not known. DESIGN, SETTING, PARTICIPANTS AND MEASUREMENTS: In an initial safety and proof-of-concept study we attempted to perform RDN in 12 patients with ESRD and uncontrolled blood pressure (BP). Standardized BP measurements were obtained in all patients on dialysis fr

Neuronal nitric oxide synthase inhibitor, 7-nitroindazole, delays motor dysfunction and spinal motoneuron degeneration in the wobbler mouse.

Gene mutations of superoxide dismutase (SOD) have been discovered in familial amyotrophic lateral sclerosis (ALS). Neuronal nitric oxide synthase (NOS), endothelial NOS and 3-nitrotyrosine immunoreactivities are selectively increased in the spinal motoneurons of sporadic ALS. Other study suggests that 3-nitrotyrosine immunoreactivity is enhanced in the spinal motoneurons of sporadic and familial ALS patients. The hypothesis is postulated that increased production of radical species, such as superoxide and peroxynitrite, may cause motoneuron degeneration in ALS. There are increased amounts of n

Protective glottic closure: biomechanical effects of selective laryngeal denervation.

Glottic closure constitutes the primary mechanism for prevention of intradeglutitive and postdeglutitive aspiration. Laryngeal paralysis therefore exerts a considerable impact on deglutition, yet little is understood regarding the biomechanical effects of selective denervation on the laryngeal protective function. We measured the glottic closing force (GCF) in each of 6 male, 40-kg Yorkshire pigs 1) after selective unilateral superior laryngeal nerve (SLN) section; 2) after selective unilateral recurrent laryngeal nerve (RLN) section; and/or 3) after combined SLN-RLN section as both right and

Origin of cardiac-related synchronized cardiac sympathetic nerve activity in anaesthetized cats.

To study the origin of cardiac-related rhythm in cardiac sympathetic nerve activity (CSNA), ECG, aortic pressure and CSNA were recorded when cardiac interval was changed by artificial pacing, or when the aortic nerve was stimulated after baroreceptor denervation in anaesthetized cats. CSNA was averaged by using the R-wave of ECG, or stimulus pulse as a trigger. Delay times from arterial pulse or stimulus pulse to the onset and half amplitude of inhibition and to the maximal inhibition were measured from the averaged data. The delay of inhibition in CSNA was constant and independent of pacing i

The carotid body in the motorneuron response to protriptyline.

Protriptyline (PRT) has been shown to preferentially stimulate upper airway inspiratory motorneurons relative to phrenic activity in hyperoxic hypercapnia in the decerebrate cat via a carotid body-independent mechanism. Since previous studies indicated that carotid body stimulation results in preferential activation of upper airway respiratory muscles during both hypercapnia and hypoxemia, we hypothesized that if PRT preferentially stimulated upper airway motorneurons, the mechanism of action might involve the carotid body. We investigated the effect of PRT on carotid body function by comparin

Neural regulation on the active sodium-potassium transport in hypokalaemic rat skeletal muscles.

C.N.S.-induced suppression of muscle Na-pump activity was studied in fast 'twitch' muscle, extensor digitorum longus, of hypokalaemic rats which were fed a K-deficient diet for 0-9 weeks. The results were compared with those of slow 'tonic' muscle, soleus, reported previously. K-deficient diet caused blood hypokalaemia and a considerable K+ loss and Na+ accumulation in the skeletal, heart and smooth muscles. The cellular K+ loss was in the order of soleus greater than extensor digitorum longus greater than diaphragm greater than duodenum greater than auricle greater than ventricle; C.N.S. orga

Ighmbp2 mutations and disease pathology: Defining differences that differentiate SMARD1 and CMT2S.

Mutations in the Immunoglobulin mu DNA binding protein 2 (IGHMBP2) gene result in two distinct diseases, SMA with Respiratory Distress Type I (SMARD1) and Charcot Marie Tooth Type 2S (CMT2S). To understand the phenotypic and molecular differences between SMARD1 and CMT2S, and the role of IGHMBP2 in disease development, we generated mouse models based on six IGHMBP2 patient mutations. Previously, we reported the development and characterization of Ighmbp2D564N/D564N mice and in this manuscript, we examine two mutations: D565N (D564N in mice) and H924Y (H922Y in mice) in the Ighmbp2H922Y/H922Y a

Characterizing focal muscle signal on MRI in flexor-pronator muscles' status post ulnar collateral ligament reconstruction.

OBJECTIVE: Focal high signal is commonly seen about the flexor pronator mass on MRI in some patients after ulnar collateral ligament (UCL) reconstruction of the elbow. The etiology of this high signal is unclear and not described in literature. The hypothesis is that the edema we see on post-operative MRI is related to palmaris longus graft harvest rather than secondary to other causes of muscle edema such as denervation or muscle strain. METHODS AND MATERIALS: An IRB waiver-approved, retrospective search of our radiology database was conducted using the keywords "ucl," "elbow," and "reconstr

Early vulnerability to ischemia/reperfusion injury in motor terminals innervating fast muscles of SOD1-G93A mice.

In mouse models of familial amyotrophic lateral sclerosis (fALS), motor neurons are especially vulnerable to oxidative stresses in vitro. To determine whether this increased vulnerability also extends to motor nerve terminals in vivo, we assayed the effect of tourniquet-induced ischemia/reperfusion (I/R) injury on motor terminals innervating fast and slow hindlimb muscles in male G93A-SOD1 mice and their wild-type littermates. These mice also expressed yellow fluorescent protein (YFP) in motor neurons. We report that in SOD1-G93A/YFP mice the motor terminals innervating two predominantly fast

PEG-fusion of viable sciatic nerve isografts restores axonal structure and behavioral recovery after segmental-loss sciatic nerve injuries in Lewis rats.

Segmental-loss peripheral nerve injuries (SL-PNIs) often produce severe deficits in sensory/motor functions and voluntary behaviors. The current gold standard for repair is neurorrhaphy of a cable autograft that (1) produces donor site morbidity; (2) results in rapid Wallerian degeneration (WD) of severed distal nerve segments; (3) relies on slow (1-2 mm/day) axonal regeneration; (4) does not prevent atrophy of denervated muscles and sensory structures; and (5) results in poor to non-existent recovery of sensory/motor functions and voluntary behaviors, especially with longer segmental-loss gap

Preferential motor reinnervation is modulated by both repair site and distal nerve environments.

To restore function after nerve injury, axons must regenerate from the injury site to the periphery, then reinnervate appropriate end organs when they arrive. Only 10 % of adults who suffer nerve injury will regain normal function, often because axons regenerate to functionally inappropriate targets (Brushart, 2011). The peripheral destination of these axons is largely determined by the pathways they enter at the site of nerve repair. To improve clinical outcomes, it is thus critical to improve the accuracy of axon pathfinding. In rodents, motor axons regenerating in mixed nerve preferentially

Chronic motoneuronal activation enhanced axonal regeneration and functional recovery after brachial plexus injury.

BACKGROUND: Brachial plexus injury (BPI) leads to significant impairment of upper limb motor function, primarily due to progressive atrophy of denervated muscles resulting from the slow rate of axonal regeneration. Therefore, identifying strategies to accelerate axon extension is of critical importance. METHODS: In this study, we first established a mouse model of brachial plexus injury and employed chemogenetic approaches to specifically activate C6 spinal motoneurons. We then assessed axonal regeneration and motor function recovery in the injured mice through behavioral tests, morphological
