BACKGROUND: For hand surgeons, in addition to anamnesis and clinical examination, imaging is the essential component in diagnostics and the basis for therapy planning. The focus on the special field of the "hand" and the adjacent joints is limited with regard to the anatomical extent, but with regard to the possible clinical pictures it encompasses the entire range of pathological changes.
CLINICAL ISSUE: Which imaging techniques support the goal of maintaining the functionality of the hand?
PERFORMANCE: The carpal bones can be represented by computer tomography without superimposition. The

The Motec cementless modular metal-on-metal ball-and-socket wrist arthroplasty was implanted in 16 wrists with scaphoid nonunion advanced collapse (SNAC; grades 3 or 4) and 14 wrists with scapholunate advanced collapse (SLAC) in 30 patients (20 men) with severe (grades 3 or 4) post-traumatic osteoarthritis of the wrist. The mean age of the patients was 52 years (31 to 71). All prostheses integrated well radiologically. At a mean follow-up of 3.2 years (1.1 to 6.1) no luxation or implant breakage occurred. Two wrists were converted to an arthrodesis for persistent pain. Loosening occurred in on

Matti-Russe grafting has proven effective in the surgical treatment of scaphoid non-union. Old or recurrent cases as well as the small proximal fragment give rise to problems. Stabilization with the Herbert screw in addition to cancellous bone grafting has proven effective in such problematical cases. Proximally located fractures also constitute an indication for primary surgery. This does not only improve the chances of healing, but also substantially shortens the period of immobilization. We have treated 34 non-unions and 12 fractures with this method between 1989 and 1990; these were mainly

A retrospective study evaluated the function of thirty-six patients treated by radius-scaphoid-lunate arthrodesis for painful posttraumatic radiocarpal arthritis from 1982 through 1987, and determined whether the procedure created arthritis or other functional problems in the remaining joints. Thirty-one men and five women with a mean age of 41 years were studied. The standard surgical technique employed iliac crest bone graft and internal fixation. Seven patients required revision of the proximal fusion to complete wrist fusion because of pain; arthritic changes in the midcarpal joint had bee

To visualise the rupture of the scaphoid bone it will be sufficient--as has been proven--to perform two additional projections from the navicular series besides the standard x-rays of the wrist joint, namely, with the hand in writing-pen position and in hyperpronation. With this procedure, almost all (99%) of fractures detected during first examination can be diagnosed. To prove the occurrence of fractures of the wrist besides those of the os naviculare, specific x-rays are employed in accordance with the results of an on-target clinical examination. Such x-rays are, in particular: lateral rad…

The aim of this study was to evaluate the results of percutaneous screw fixation of scaphoid waist fractures in our surgery unit by comparing them with recent data from the literature. This was a retrospective, descriptive study conducted between January 2010 and September 2016. Patients operated due to a recent scaphoid fracture and treated by percutaneous anterograde screw fixation with a Herbert screw were included in this study. Our cohort was a sub-set from a scaphoid fracture group; other fracture sites, other treatment techniques and fractures associated with scapholunate ligament lesio

Perilunate injuries occur after an impact trauma to the wrist. The recognition of these lesions and immediate treatment are the two essential conditions to ensure the best possible outcome. The first therapeutic step is the restoration of anatomical joint relationships of the carpus followed by percutaneous pinning or internal fixation required for maintaining the congruence this segment. Despite correct diagnosis and approach in these cases the prognosis is often encumbered by decreased range of motion, loss of grip strength and finally the onset of osteoarthritis. Scapholunate dissociation i

This study investigated the effects of loading and posture on mechanical properties of the transverse carpal ligament (TCL). Ten fresh-frozen cadaver arms were dissected to expose the TCL and positioned in the load frame of a servo-hydraulic testing machine, equipped with a load cell and custom made indenters. Four cylindrical indenters (5, 10, 20, and 35 mm) loaded the TCL in three wrist postures (30° extension, neutral and 30° flexion). Three loading cycles with a peak force of 50 N were applied at 5 N/s for each condition. The flexed wrist posture had significantly greater TCL stiffness (40

We randomly allocated 60 consecutive patients with fractures of the waist of the scaphoid to percutaneous fixation with a cannulated Acutrak screw or immobilisation in a cast. The range of movement, the grip and pinch strength, the modified Green/O'Brien functional score, return to work and sports, and radiological evidence of union were evaluated at each follow-up visit. Patients were followed sequentially for one year. Those undergoing percutaneous screw fixation showed a quicker time to union (9.2 weeks vs 13.9 weeks, p < 0.001) than those treated with a cast. There was a trend towards a hi

BACKGROUND: Nondisplaced scaphoid fractures treated with prolonged cast immobilization may result in temporary joint stiffness and muscle weakness in addition to a delay in return to sports or work. Fixation of scaphoid fractures with a percutaneous cannulated screw has resulted in a shorter time to union and to return to work or sports. The purpose of this prospective, randomized study was to compare cast immobilization with percutaneous cannulated screw fixation of nondisplaced scaphoid fractures with respect to time to radiographic union and to return to work.
METHODS: Twenty-five full-tim

As with the practice of all medicine, an early accurate diagnosis is essential. The same applies after injury. The diagnosis should be made at the earliest opportunity--this should be done as soon as an appropriate expert is able to examine the patient, assess adequate radiographs and, where necessary, order special investigations such as a bone scan. By this means, distal radial fractures, injuries of the radioulna joint, dislocated lunates, Bennett's fractures, dorsal capsular wrist sprains, avulsion fractures etc. can be managed accordingly. Those patients in whom clinical examination, plai

BACKGROUND: The objective of this study was to examine the effect of wrist rotation on the scapholunate interval in the posteroanterior radiograph and to identify radiographic landmarks on the posteroanterior projection that can be used to assess position.
METHODS: Eleven healthy cadaveric wrists were radiographed in the neutral position and subsequently were rotated and imaged from 30° pronation to 30° supination in 10° intervals. At each interval, the scapholunate interval was measured as well as the following landmarks: (1) the visible perimeter of the base of the hook of the hamate; (2) t

BACKGROUND: Scaphoid nonunion (SNU) presents substantial functional limitations, frequently necessitating surgical intervention to restore wrist mobility and prevent degenerative changes. While the traditional approach to managing SNUs has relied on open fixation combined with bone grafting, the universal necessity of grafting, especially in stable nonunion (NU), is increasingly questioned. Emerging evidence indicates that graft-less fixation can deliver favorable outcomes, with notable healing observed in such cases.
AIM: To compare the outcomes of graft-less vs graft-augmented fixation in s

BACKGROUND: Functional disorders of the hand are generally investigated first using conventional radiographic imaging. However, X-rays (two-dimensional (2D)) provide limited information and the information may be reduced by overlapping bones and projection bias. This work presents a three-dimensional (3D) hand reconstruction method from biplanar X-rays.
METHOD: This approach consists of the deformation of a generic hand model on biplanar X-rays by manual and automatic processes. The reference examination being the manual CT segmentation, the precision of the method was evaluated by a comparis

BACKGROUND: Four-corner fusion (4CF) is a known treatment option for degenerative wrist conditions. Different techniques may be used and there is no general consensus on best implant. As such, it was the purpose of the current systematic review to compare fusion rates and outcomes depending on the fixation technique.
METHODS: The systematic review was registered in the international prospective register of systematic reviews (PROSPERO): CRD42020164301. It followed the PRISMA guidelines. Original articles were screened using the following databases: Cochrane Central Register of Controlled Tria

PURPOSE: To evaluate the diagnostic utility of scaphoid dorsal subluxation on magnetic resonance imaging (MRI) as a predictor of scapholunate interosseous ligament (SLIL) tears and compare this with radiographic findings.
METHODS: Thirty-six MRIs were retrospectively reviewed: 18 with known operative findings of complete Geissler IV SLIL tears that were surgically repaired, and 18 MRIs performed for ulnar-sided wrist pain but no SLIL tear. Dorsal subluxation of the scaphoid was measured on the sagittal MRI cut, which demonstrated the maximum subluxation. Independent samples t tests were used

This report is of a 32 year old man who presented with complains of pain, swelling and deformity of right wrist of four weeks duration. He gave history of road traffic accident four weeks back leading to injury to right wrist; Preoperative radiographs and C.T. scan images were suggestive of trans-scaphoid dislocation of the proximal row of wrist. A volar and dorsal approach were used to reduce this complex dislocation but was not successful. Wrist arthodesis was performed after doing proximal row carpectomy. One year follow-up of the patient showed fair result with grip strength of 85% to cont

PURPOSE OF THE STUDY: A complete rupture of the scapholunate (SL) interosseal ligament results in palmar flexion of the scaphoid and dorsiflexion of the lunate that leads to disintegration of the carpal circle and the development of scapholunate dissociation with dorsal intercalary segment instability. If the injury is not treated properly or early, the abnormal position of the scaphoid and lunate results in degenerative changes of the wrist shown by X-ray and referred to as scapholunate advanced collapse (SLAC). The aim of this study was to evaluate the results of our method used for reconstr
