[Psychological stress factors in erectile dysfunctions. Causal models and empirical results].

In this paper the role of psychosocial factors in erectile dysfunction is examined in two different ways: (1) Current approaches to the causation of psychogenic erectile dysfunctions are reviewed and discussed. (2) Empirical results from a large unselected sample of sexually dysfunctional men are presented and compared to a sample of functional men. Concerning etiological models the traditional unidimensional dichotomous concepts (psychogenic versus organic) of erectile dysfunction have to be abandoned and replaced by two-dimensional models that are able to take the clinical reality into accou

Balancing elder care responsibilities and work: two empirical studies.

It was hypothesized that workers with elder care responsibilities, compared with noncaregiving workers, would be more likely to suffer physical symptoms of stress and to be absent more days. In Study 1, 67 (32 noncaregivers, 35 caregivers) employees of a financial institution were interviewed. In Study 2, 169 (118 noncaregivers, 51 caregivers) employees of a health care institution completed a questionnaire. In Study 1, caregivers were absent more days than noncaregivers, and caregiver status was related to the incidence of weight loss or gain, drowsiness, and inability to sleep. In Study 2, c

The relationship of childhood physical and sexual abuse to adult illness behavior.

A sample of 275 undergraduate subjects reported (i) their history of sexual and physical abuse in childhood and (ii) their health care utilization, somatization, and hypochondriasis as an adult. Physical and sexual abuse were recalled by largely separate groups, physical abuse predominating in males and sexual abuse in females. Both types of abuse were followed by a greater number of hospital admissions and surgical procedures in adulthood. Although sexual, and especially physical, abuse were associated with recall of emotionally disturbed parenting, this could not explain their influence on h

Pseudoseizure manifestations in two preschool age children.

We report two patients with epilepsy with pseudoseizures at age 6 years. Both presented with intractable staring spells. Pseudoseizures were provoked and aborted by suggestion, leading to the diagnosis. In both patients, evidence of a neuropsychological disturbance was later found and psychotherapy started. Monitoring of intractable staring episodes is recommended prior to escalating antiepileptic drug levels or resorting to polytherapy. In addition, differentiation from other non-epileptic phenomena is necessary to initiate proper therapy.

Measurement of conscious symptom exaggeration by questionnaire: a clinical study.

This study compared measures of conscious exaggeration of symptoms by 157 patients with chronic pain who were involved in personal injury litigation with 106 patients not seeking compensation who were also attending a multidisciplinary pain clinic. Pain severity was assessed using a visual analogue scale and the adjectival check-list of the McGill Pain Questionnaire. There was no difference between the two groups on scores obtained on the Conscious Exaggeration (CE) scale; it was found that there was a significant correlation between CE scores and trait anxiety, hostility, state anxiety, and p

The role of tonic immobility during re-experiencing trauma in PTSD treatment.

Tonic immobility (TI) is a profound paralysis that may occur during extreme stress. Previous studies have found that TI during trauma was associated with poorer recovery from posttraumatic stress disorder (PTSD). Importantly, TI can re-occur during re-experiencing the trauma (TIre-exp). It is unclear whether TIre-exp also hinders recovery from PTSD or declines along with other PTSD symptoms after treatment. We examined whether pre-treatment self-reported TIre-exp would predict PTSD symptoms after trauma-focused treatment, including related constructs as depression and dissociation. and investi

Phenomenology and psychiatric correlates of pseudocataplexy.

STUDY OBJECTIVES: Pseudocataplexy is a rare functional neurological disorder that mimics cataplexy, pathognomonic for narcolepsy type 1 (NT1). We describe the psychiatric comorbidity and personality traits of patients with pseudocataplexy versus NT1 cases. METHODS: The case-control observational study enrolled consecutive patients with pseudocataplexy and a control group of age-matched consecutive NT1 patients. The diagnostic work-up included a structured interview, 48-hour polysomnography, multiple sleep latency test, cataplexy provoking test, and hypocretin-1 measurement in cerebrospinal fl

Pain and Analgesic Utilization in Medically Underserved Areas: Five-Year Prevalence Study from the Rochester Epidemiology Project.

PURPOSE: There is a paucity of data on pain diagnoses and analgesic utilization in medically underserved areas (MUAs). This study compared the prevalence of pain diagnoses and analgesic medication use between MUAs and non-medically underserved areas (N-MUAs) in Southern Minnesota and Western Wisconsin using the Rochester Epidemiology Project (REP) database. METHODS: Five-year prevalence per 100 people (January 1, 2011 to January 1, 2016) was extracted from the REP database for multiple pain diagnosis variables and analgesic medications. Primary outcomes included comparison of five-year preval

Pain associated with specific anxiety and depressive disorders in a nationally representative population sample.

OBJECTIVE: To examine in a nationally representative sample (a) the differential association of specific anxiety and depressive disorders defined according to DSM-IV with pain disorder (PD) and pain symptoms, and (b) whether pain-associated anxiety and depressive disorders and their comorbidity have different implications in terms of impairment, disability, health care utilization, and substance use. METHOD: A nationally representative community study was conducted in Germany. Symptoms, syndromes and diagnoses of mental disorders, and pain were assessed in N = 4,181 participants aged 18-65 ye

Trauma prevalence and somatoform symptoms: are there specific somatoform symptoms related to traumatic experiences?

There is still insufficient knowledge on the subject of possibly specific patterns of somatoform symptoms related to sexual or nonsexual traumatizations. Using standardized questionnaires, a sample of 892 patients consecutively admitted to a psychotherapy outpatient clinic were evaluated for psychological symptoms in general, for somatoform symptoms and for history of traumatizations. Any severe lifetime trauma was reported in 67.8% of the total sample. Somatoform symptoms were notably more prevalent in traumatized patients when compared with nontraumatized patients. Descriptive data analysis

[Psychosomatic aspects of chronic pelvic pain syndrome. Psychometric results from the pilot phase of an interdisciplinary outpatient clinic].

BACKGROUND: Chronic pelvic pain syndrome (CPPS) presents as a multicausal disorder. Complex interactions of psychological factors with somatic dysfunctions are crucial to the development and maintenance of CPPS. AIM: This study characterized the patient cohort from a psychosomatic perspective. MATERIAL AND METHODS: Subjects with CPPS were recruited from an interdisciplinary CPP outpatient clinic. Sociodemographic data, symptoms (National Institutes of Health Chronic Prostatitis Symptom Index, NIH-CPSI) and pain-related factors (Short Form of the McGill Pain Questionnaire, SF-MPQ) as well as

The epidemiology of multiple somatic symptoms.

BACKGROUND: The risk factors for a high total somatic symptom count are unclear; and it is not known whether total somatic symptoms count is a predictor of impaired health status. METHOD: A prospective population-based cohort study in North West England. Randomly sampled residents (1443 participants; 58% response) completed questionnaires to determine number of somatic symptoms (SSI), health status and a wide range of risk factors; 741 completed questionnaires 1 year later. We used logistic regression to identify risk factors for high SSI at follow-up and for persistently high SSI. We used AN

Patient-doctor interaction, psychobehavioural characteristics and mental disorders in patients with suspected allergies: do they predict "medically unexplained symptoms"?

In approximately 20% of patients with suspected allergies, no organic symptom explanation can be found. Limited knowledge about patients with "medically unexplained symptoms (MUS)" contributes to them being perceived as "difficult" and being treated inadequately. This study examined the psychobehavioural characteristics of patients presenting for a diagnostic allergy work-up. Patients were interviewed and completed various self-rating questionnaires. Patient-Doctor interaction was evaluated, and the organic explicability of the patients' symptoms was rated by allergists. Patients with vs. thos

Primary care perspectives on generalized anxiety disorder.

Recently, there has been increased interest in the impact and treatment of anxiety disorders. However, one type of anxiety disorder, generalized anxiety disorder (GAD), has received less attention than other disorders, such as panic disorder, despite the prevalence and amenability of this disorder to treatment in the primary care setting. Rates of GAD have been found to be between 2.8% and 8.5%, with a median prevalence of 5.8%-at least twice the rate reported in the National Comorbidity Survey. Up to one third of patients presenting to primary care clinics with somatic complaints had a mood o

Promoting mindfulness in psychotherapists in training influences the treatment results of their patients: a randomized, double-blind, controlled study.

BACKGROUND: All therapists direct their attention in some manner during psychotherapy. A special form of directing attention, 'mindfulness', is recommended. This study aimed to examine whether, and to what extent, promoting mindfulness in psychotherapists in training (PiT) influences the treatment results of their patients. METHODS: The therapeutic course and treatment results of 124 inpatients, who were treated for 9 weeks by 18 PiTs, were compared. The PiTs were randomly assigned to 1 of 2 groups: (i) those practicing Zen meditation (MED; n = 9 or (ii) control group, which did not perform m

Traumatic events and post-traumatic stress disorder in the community: prevalence, risk factors and comorbidity.

OBJECTIVE: Lifetime and 12-month prevalence of traumatic events and DSM-IV post-traumatic stress disorder as well as risk factors and comorbidity patterns were investigated in a representative community sample (n = 3021, aged 14-24 years). METHOD: Traumatic events and PTSD were assessed with the Munich Composite International Diagnostic Interview (CIDI). RESULTS: Although 26% of male subjects and 17.7% of female subjects reported at least one traumatic event, only a few qualified for a full PTSD diagnosis (1% of males and 2.2% of females). Traumatic events and PTSD were strongly associated w

Inherited and early-life vulnerability in functional neurological disorder and functional somatic symptoms: A retrospective cohort and case-control study from a large electronic health record database.

BACKGROUND: Biological factors affecting motor, sensory, and integrative processing may predispose to functional neurological disorder (FND) and functional somatic symptoms. While early-life influences have traditionally been conceptualized within psychological frameworks, the contribution of early-life biological vulnerabilities to symptom expression remains unclear. METHODS: Using TriNetX, a large electronic health records database, we assessed proportions of inherited, congenital, perinatal, and neurodevelopmental diagnoses, capturing a range of early-life conditions. We compared 188,868 i

Trajectories of persistent physical symptoms in primary care: A secondary analysis of the PsicAP randomized clinical trial comparing CBT versus treatment as usual.

BACKGROUND: The course of persistent physical symptoms (PPS) is heterogeneous, yet little is known about distinct symptom trajectories and their baseline predictors across treatment conditions in primary care. This study aimed to identify PPS trajectories and their associated baseline characteristics in patients receiving treatment as usual (TAU) or transdiagnostic group cognitive behavioral therapy (TDG-CBT). METHODS: Latent Class Growth Analysis (LCGA) was used to identify trajectories of PPS (PHQ-15) over 12 months in 497 participants from the PsicAP randomized clinical trial (TAU: n = 242
